Quality nursing care documentation based on facts, accurate, complete, concise, organized, timely, easy to read, and continuity. Continuity of nursing care is fulfilled if the range of health services is appropriate, the availability of quality caregivers professionals and information related to patient care are delivered on an ongoing basis. The method used is a case study with observation, interviews, secondary data and document review. This study that is an innovation project, uses a fish bone diagram approach to identify the causes of the discontinuity independent nursing care documentation. Documentation of evaluation of independent nursing care for the development of patients in the midwifery and ginecology room in X Hospital in Jakarta has not reflected the continuity of nursing care. The causes include the lack of optimal competence in providing independent nursing care, not optimal nursing manager functions. The continuity is overcome by optimizing the nursing manager's function by making SOP note integrated patient development, revision of midwife's job description, revision of guide to nursing practice, electronic-based documentation system, placing midwives in accordance with the service area the authority and standard of the profession, the structured supervision schedule, and make sure the midwife's job description is in accordance with the policy.

Professional values can direct nurse behavior. Nurse behavior should reflect caring in providing nursing services as the essence of nursing. The method used in this paper is a case study. Data collection is done by interviews, document observation, and distributing questionnaires to nurses in the amount of 517 nurses. The questionnaire used was the Nursing Professional Value Scale and Nurse that was modified and used to determine the nurse's professional score scale. Interviews were conducted with the field and nursing committee using a semi-structured interview guide. Interview material includes management functions related to the application of the nurse's professional values. Data analysis using fishbone analysis. The results showed that the professional score scale of nurses was quite strong, namely 101.19, but the application of professional nurses' values was still not optimal because some nursing leaders were still less professional. The recommendation from this case study is that nursing management needs to motivate nurses to apply nurses' professional values through attitudes and behaviors that have implications for the quality of nursing services and have an impact on patient satisfaction and safety.

Nursing is an integral part of medical services in a hospital, and its mission is to provide the continuum of care based services throughout the life cycle. Nursing round and nursing handover are substantial elements in providing nursing services and ensure patients’ safety. Optimizing the implementation of the nursing round and nursing handover aims to refresh, renew, and optimize required instruments. Therefore, renewing nurses’ knowledge and commitment is required to adopt a better change as mentioned in the hospital’s vision and mission, i.e. continuum of care. This research employed an analysis study with Fishbone analysis. The results reveal that the nurses’ knowledge of nursing round management increases by 49.25%, and their experience of conducting nursing round increases by 39.13%. It indicates a fine enhancement and proves that the implementation of nursing round and handover is effective to enhance the nurses’ knowledge of nursing round management because, in this round, nursing handover is directed and monitored. The commitment of top management plays a significant role in optimizing this program. Therefore, the current issue of patient centre care is well accommodated. Discharge planning is a complex process aiming to prepare patients during a transition period in a hospital until they are home. One of general hospitals (X) in Jakarta has implemented discharge planning, but it is not optimally conducted. This research aims to analyze the implementation of optimizing discharge planning in X General Hospital in Jakarta. This research employed pilot study initiated by conducting assessment through questionnaire, observation, and interview. The next stages were root cause analysis by employing fishbone analysis, Plan of Action (POA) arrangement, Plan Do Check Action (PDCA) implementation, and evaluation by employing questionnaire. One of the problems of implementing discharge planning is was implementation method in which the guidance of discharge planning was not available. Therefore, standard operating procedures (SOP) and format of discharge planning were necessarily revised. After arranging the guidance draft, SOP, and format of discharge planning, the researcher socialized and tested the SOP and format of discharge planning in two inpatient rooms. The results reveal that 80% of respondents state that the arranged format of discharge planning is easily understandable, easily applicable, less complicated, not rambling, and practical. Therefore, the management of X hospital Jakarta necessarily conducts the follow-up to develop and legalize the guidance draft, SOP, and format of discharge planning. Furthermore, the management necessarily socializes them to duty nurses before the hospital applies them.

Abstract HIV is a major health problem in the world. HIV infection has the effect of reducing immunity status and increasing the risk of cardiovascular disease. Antiretroviral (ART) use as a therapy for HIV also increases the risk of heart disease. Objective: This literature review aims to identify the risk of cardiovascular disorders in HIV patients in Indonesia. Method: literature review of articles published January 2009 to September 2018 using search keywords namely 'Anti-retroviral therapy', 'Cardiovascular disease', 'Corronary artery disease', and 'HIV'. Results: There were 21 journal articles that were relevant to the design and purpose of paper and 3 articles that discussed the risk of heart disease in HIV patients in Indonesia. The results of the literature review showed that HIV patients in Indonesia are at risk of developing cardiovascular disorders which are exacerbated by the use of ART and smoking behavior. The risk of cardiovascular disease includes structural abnormalities and coronary heart disease. Conclusion: HIV patients have a high risk of developing cardiovascular disease. This is a challenge for health workers, the government and researchers, to develop appropriate strategies and methods in reducing the risk of cardiovascular disorders especially in low-middle income country countries such as Indonesia. Abstract The results of the study identified the existence of obstacles in the implementation of discharge planning which had an impact on the lack of optimization of discharge planning implementation and the determination of the length of stay of the patient. The author uses the case study method. The innovation agreed upon with the nursing manager was in the form of implementing a nursing management round which was expected to have an impact on improving the implementation of discharge planning, as well as revisions to the standard operating procedures and the discharge planning documentation in accordance with Standard Nasional Akreditasi 2018. The result is an increase in knowledge of the head nurse and clinical care manager related to nursing management rounds (49.25%) and experience improvement (39.13%). Standard operational procedure of discharge planning draft and discharge planning form are agreed upon by the nursing manager. Resocializing discharge planning to doctors, nurses, nutritionists, pharmacists, and other health workers is needed to support patient safety and patient comfort before returning home.

Introduction: The success orthopedic surgery patients’ education depends on many factors, including the accuracy in choosing methods and the educational media used. This selection is based on consideration of the appropriateness of the material provided for the patient's needs and the learning objectives to achieve. The purpose of this literature review is to identify the types of methods and educational media employed to improve self-knowledge and abilities of orthopedic surgery patients. Method: The references employed in this literature review were four experimental research articles published in 2010 to2018. The keywords appropriateness in the search were "orthopedic patient", "patient education", "patient knowledge", "education method", and "patient outcome "through the ProQuest database, EBSCO, Sage, and Clinical Key. Results: This review shows that the media and educational methods vary widely, including face-to-face education, web-based or internet-based education, education with DVD videos, and structured education. Statistical analysis shows that all of these methods can improve the knowledge and skills of orthopedic surgery patients in undergoing the perioperative phase. Conclusion: Using structured educational methods with media combined with web-based or internet education and DVD video education with direct education (face to face) is considered good for education established for patients. Meanwhile, using education with DVD videos is considered good skill enhancement education.

This unpreparedness is caused by the inability of patients to manage their own needs and care needs after returning home in carrying out daily tasks, caring for themselves, and the ability to do health care. So that a nursing strategy is needed in preparing the patient's discharge planning in the form of discharge teaching. Four data bases namely Cochrane, EBSCO, Google Scholar, and Scopus were used to explain the effect of discharge teaching to improve patient readiness post hip surgery. Abstract Heart failure has now become a global cardiovascular problem with high mortality and disability. Sleep disturbance is a common problem in heart failure patients. Sleep disorders that occur in patients with heart failure will cause increased levels of urine catecholamine, plasma leptin, and inflammatory markers. These changes are related to the development of hypertension, activation of the sympathetic nervous system, and blunting of chemoreflex, which will increase the heart burden and ultimately lead to a poor prognosis. Sleep disorders in heart failure patients can be treated with psychological interventions and behaviors including sleep hygiene and Benson relaxation. Nurses, as one of the health professionals, have an important role in this matter. This is because the scope of nursing is meeting basic human needs, namely sleep. Abstract People living with HIV/AIDS experience a variety of complex problems, not only physical problems related to the condition of HIV disease, but also psychosocial problems related to HIV disclosure, stigma, social isolation and discrimination that can cause anxiety and cause high stress levels.The conditions will gave the negative impact on both physical and mental health. The negative impact of stress can lead to depression, giving rise to an attitude of non-compliance with treatment, increased risk behavior for HIV transmission and faster disease progression, which in turn will worsen the quality of life for people living with HIV/AIDS. Several studies have been conducted to found appropriate interventions to reduce stress levels in people with HIV/AIDS. This article is a literature review of several interventions to reduce stress levels in people with HIV/AIDS, included Jacobson's Progressive Muscle Relaxation (JPMR), Mindfulness-Based Stress Reduction (MBSR) and Reiki therapy. Literature search was carried out from databases such as Pubmed, EBSCOhost, Scopus, Proquest, Sciene Direct, with limited years of article publication in the last 10 years (2008-2018). Based on the review, it was found that Jacobson's Progressive Muscle Relaxation (JPMR) intervention; Mindfulness-Based Stress Reduction, Reiki Therapy shows very significant results in decreasing stress levels in people with HIV conducted in three studies with different interventions.
