INTRODUCTION: Infertility is a life-changing experience that brings with it medical, psychiatric, psychological, and social problems, with cultural, religious, and social aspects, individual-specific, leadings to stress in couples, resulting in social labeling, sexual dysfunction. Many factors can trigger or increase the severity of infertility. Couples failing to achieve one of the basic functions of the family institution, also experience negativities in marriage. This study aimed to determine the severity of violence by men to his partner and the marital adjustment level in men in couples applying for infertility treatment. METHODS: The study population consisted of 286 men from the couples applying to Cukurova University Faculty of Medicine, Department of Obstetrics and Gynecology, Infertility Outpatient Clinic June -October 2017. The data was collected using the Sociodemographic Data Form, Conflict Tactic Scale-2, Marital Adjustment Scale. Data analyzed using SPSS 20.0 Statistical analysis program. RESULTS: The mean age of men was 34.79±7.24 years, the mean duration of marriage was 5.87±4.86 years and the mean age at first marriage was 26.33±5.09 years. Of the participants, 93.4% were violent against their spouses based on the Conflict Tactics Scale-2 results. There was no significant relationship between the overall violence by men and any of the sociodemographic data. Marital Adjustment Scale revealed that there was a marital adjustment in 80.4% of men. A significant relationship was found between marital adjustment and sociodemographic variables including occupation, previous infertility treatment, and marital status. DISCUSSION AND CONCLUSION: The men practiced violence against their wives and that the most common type of violence was psychological violence. In infertile couples, the majority of men had good marital adjustment and they did not seem to be affected by infertility.

INTRODUCTION: The aim of this study was to evaluate the developments in the last 20 years regarding abdominal wall hernia repairments by examining the surgery records performed in the General Surgery Clinics of two large hospitals in Adana. METHODS: This cross-sectional and descriptive study was conducted in two stages. In the first stage, abdominal wall hernia repairments performed in Adana Numune Training and Research Hospital between 01.01.1999-31.12.1999 were analyzed. In the second stage, operations performed in Adana Seyhan State Hospital between 01.01.2019-31.12.2019 were analyzed by using medical records. The following parameters were compared for 1999 and 2019: age, gender, type of hernia, being unilateral or bilateral, emergency or elective operation, recurrence constellation, operation technique and type of mesh. RESULTS: In the first stage of the study, 391 operations were performed with bilateral repairs on 379 cases. The mean age of the patients was 49.0(±3.7) and median was 43 years, 62.2%(n = 263) were male, 41.4% (n = 157) were between 45-64 years of age. The most common type of hernia was inguinal hernia with 72.0%(n = 273) and the rate was 7.8 times higher in male than in female. The most common surgical technique was Bassini repair(42.4%, n = 166). In the second stage of the study, 1719 operations were performed with bilateral repairs on 1680 cases. The mean age of the patients was 49.4(±3.3) and median was 51 years, 65.9%(n = 1107) of them were male, 48.6%(n = 816) were between 45-64 years of age. The most common type of hernia was inguinal hernia with 50.4%(n = 848) and the rate was 16.6 times higher in male than in female. The most common surgical technique was Prolene Mesh Hernioraphy(73.1%, n = 1257). DISCUSSION AND CONCLUSION: In the last 20 years, it was determined that surgical repair techniques in abdominal wall hernias have changed a lot, and techniques such as Bassini, Shouldice, Halsted, McVay have been replaced by prosthetic mesh and laparoscopic repairs that provide less recurrence, less chronic pain and faster recovery time.

Objective: Our study aimed to evaluate anxiety and occupational anxiety level in final-year medical students attending a medical board exam course. Methods: This study was conducted between February and May 2020 in a medical board exam course center in Istanbul within the necessary permissions. The study was completed with 210 final year medical students who responded appropriately to the Beck Anxiety Inventory (BAI) and Occupational Anxiety Questionnaire (MKA) form, and a questionnaire which is consisted of sociodemographic data prepared by us and statistical analyzes were performed. Results: The mean age of participants was 23.60±1.36 years, with 53.3% (n=112) identified as male and 46.7% (n=98) identified as female. The mean score of BAI was 14.15±11.01, and according to the level of BAI, 81 (38.6%) of participants had minimal, 43 (20.5%) had mild, 48 (22.9%) had moderate, and 38 (18.1%) had severe anxiety. The mean score of OCQ was 55.92±12.81, and 136 (64.8%) participants had a moderate level of OCQ. There was a positive correlation between BAI and OCQ mean score (r=0.228, p=0.01). Conclusion: The study results revealed that most final-year medical students had minimal anxiety levels and moderate occupational anxiety levels.

Objective: In the fight against COVID-19, each country has determined different strategies to combat the epidemic, considering factors such as facilities, health infrastructure systems, and social characteristics. Within the scope of the epidemic management strategy implemented in our country, quarantine was initiated after the field teams reached the COVID-19 cases and their contacts and made the necessary evaluations. Our study aimed to compare the effects of age, gender, contact environment, symptoms, and degree of closeness variables on the transformation into positive COVID-19 cases. Methods: During the 9-month study period between March 20 and December 20, 2020, people who were in contact with COVID-19 cases and whose contact follow-up was initiated by field teams in Gaziemir District of İzmir province were examined. Those who became positive while in isolation were determined among these quarantine contacts. Permission was obtained from the Scientific Research Platform of the Ministry of Health. The ethics committee approval was then obtained from the Clinical Research Ethics Committee of the University of Health Sciences (HSU) Tepecik Education and Research Hospital with the decision number 2021/01-08, dated 25.01.2021. Results: A total of 14,668 contacts were included in the study. Of these, 2,841 (19.4%) became positive cases during follow-up. Individuals who had contact with a positive case in compulsory common areas such as workplaces, schools, barracks, dormitories, prisons, and nursing homes turned into positive cases at a rate of 2.63 times more than individuals who came into contact with them in other settings. Age has a significant effect on the incidence of contacts. It was determined that as the age of the contacts increased, they turned into positive cases at higher rates. Conclusion: We have concluded that the age, gender, contact environment, and proximity to the 60 COVID-19 positive cases affect these individuals in becoming positive cases. Therefore, identifying these features will help us understand the transmission characteristics and spread dynamics of the COVID-19 disease.

Objective: In our study, we aimed to determine vitamin D levels among obese patients registered to Obesity Center and to investigate its relationship with body mass index (BMI). Methods: The study was conducted in the Obesity Center of Health Sciences University Samsun Training and Research Hospital. Records of 102 patients with BMI> 30 kg/m², 18 years of age and over, admitted to the obesity center between 01.12.2018 and 01.12.2019 were retrospectively analyzed. Serum 25-OH D level<20 ng/ml was accepted as severe insufficiency, 20-30 ng/ml insufficiency, >30 ng/ml proficiency. Mann-Whitney U, Pearson chi-squared and Spearman tests were used in the evaluation of the data. The data were evaluated by the SPSS 22.00 program. p<0.05 was considered significant. Results: A total of 102 patients participated. The mean age was 48.82±12.09 years. 62.75% of them were female. 24.51% of participants were class 1 obese, 32.35% of participants were class 2 obese, 43.14% of participants were class 3 obese. The prevalance of severe vitamin D insufficiency was 45.10% and insufficiency in 38.24% of the patients. The mean vitamin D levels of the patients registered to the Obesity Center was 13.26±7.74 ng/ml. The mean BMI was 35.26±4.28 kg/m². Serum 25-(OH)D levels were inversely related to BMI. There was a significant relationship between age and BMI (p=0.036). Conclusion: Vitamin D insufficiency is highly prevalent in obese patients registered to Obesity Center. Serum 25-(OH)D levels were inversely associated with BMI. Obese patients should be evaluated and followed for vitamin D insufficiency.

Objective: This study aimed to evaluate the frequency of obesity, hypertension and anemia in the periodic follow-up of school-age children and adolescents aged between 6-19 years. Methods: In this retrospective cross-sectional study, weight, height, body mass index (BMI), systolic/diastolic blood pressure measurements and complete blood count results were compiled between January 2018 and August 2019 at a family health center. School-age children and adolescents examined for periodic screening purposes and whose completed measurement records were included while incompleted records were excluded from the study. IBM SPSS 22.0 program was used to analyze the data, and statistical significance was accepted as p<0.05. Results: The follow-up results include the data of 1049 children, (536 boys; 513 girls), with an average age of 11.4±3.2 years. The frequency of obesity, hypertension, anemia, short height, and underweight was 7.3%, 24.5%, 76.6%, 1.9%, and 21.3% respectively. The frequency of anemia was observed higher in girls (85.2%) than in boys (68.6%) (p<0.001). While there is no significant difference between age groups and genders in terms of the presence of hypertension (p=0.299 and p=0.270), a higher hypertension diagnosis rate was observed in primary school children (26.1%) than children with hypertension diagnosis in secondary school (24.3%) and high school (19.9%), significantly (p<0.001 and p<0.001). Although the frequency of hypertension (31%) and anemia (85%) was higher in obese groups compared to other groups, it was not found statistically significant (p=0.151 and p=0.196). Conclusion: This study was performed in a primary health care institution; despite the prevalence of obesity at a similar rate to previous studies, it has demonstrated that one out of every four children was hypertensive or underweight, and almost all of the girls were anemic, resulting in cautionary results in terms of preventing growth and development problems, adult cardiovascular and maternal-fetal mortality risks.

Objective: This study aims to determine the family physician’s level of awareness on vertigo. The approaches of family medical practitioners and ENT physicians on vertigo-diziness was determined, as well as the knowledge on most common vertigo-causing diseases and the diagnostic and therapeutic approaches of these diseases were contrasted. Methods: 352 physicians including 160 ENT physicians and 192 family physicians were included in the survey study. This study was conducted in the form of an online survey using “google forms” open web page. The questionnaire study was completed by specialist physicians, lecturers and actively working medical residents with more than 1 year of professional experience. Participants were asked to fill in a 34-item questionnaire involving sociodermographic information, the definition of vertigo, the commonest vertigo-causing diseases, the treatment of these diseases and their willingness for educationon vertigo. Results: Concerning the questionnaire item “what is the first disease that comes to mind when vertigo is mentioned?”, 95% of ENT physicians responded benign paroxysmal positional vertigo (BPPV) and 3% of them mentioned Meniere’s disease as their answer while 53% of the family physicians answered BPPV and 35% of them chose Meniere’s disease. 31% of ENT physicians and 63% of family physicians stated that they usually order blood tests to the patients presenting with vertigo complaint. The reply to the question “what is the basic treatment of BPPV” was repositioning maneuvers in 96% of ENT physicians and 71% of family physicians. The rate of performing repositioning maneuvers on patients with BPPV is 97% in ENT physicians and 29% in family medical doctors. Conclusions: The most crucial conclusion of this study is that family physicians participating in the questionnaire need more education about the distinction between vertigo and dizziness and some fundamental points about peripheral vertigo-causing diseases. At this point, we think that organizing courses on vertigo for family physicians will be beneficial in this regard.

Objective: Diseases are more seen in the elderly population, therefore pain also gains a severe probleme in this situation. It is well known that pain impairs significantly the quality of life and gives raise to an increase use of drugs. The aim of our study is to determine the severity of chronic pain in the eldely and to evaluate the conditions affecting chronic pain. Methods: We included 456 elderly patients in our study. The mean age was 71.19±6.27 (min: 65; max: 93). Patient’s sociodemografic information, diseases, use of medications, presence of chronic pain, affecting conditions and treatment applied for chronic pain, and Geriatric Pain Scale were evaluated. Results: The females integrated in tis study were 272 with a rate of 59.6 %. Regarding the geiatric pain scale 61.2 % (n: 279) had a mild pain while the other 18.6% (n: 85) had severe pain. The 87.7% were in medication of analgesics for their chronic pain. The patient’s knowledge of the used of medications was with a rate of 43.9% (n: 200). We founded a statistically significant relationship between the chronic pain and femele gender (p<0.001), older age (p=0.012), smoking users (p=0.01), nonsteroidal antiinflamatory drug users (p=0.003) and inidviduals with higher mass body index (p=0.016). Conclusion: Chronic pain in the elderly is a common complaint. The presence of pain disrupts the quality of life and puts additional burdens on the healthcare system. Detecting preventable situations that cause pain and increases severity, giving importance to non-drug treatments, increasing health literacy about the drugs used, implementing effective policies against diseases will reduce the severity of chronic pain, increasing the quality of life and significantly reducing health costs.
