The importance of identifying and supporting paternal mental health during the perinatal period has been gathering momentum. Health visitors are ideally placed due to their contact with families to assess and support paternal mental health. The aim of this study was to explore health visitors' current practice to assess and support paternal mental health and identify any barriers preventing this in practice. A small-scale, anonymous electronic 12 question survey was conducted. Purposeful sampling method was used to recruit health visitors (n=120) from across three health visiting services provided by UK NHS Trust selected for this study. Survey response rate was low (n=36), limiting the potential to generalise findings to the wider health visiting population. Most respondents were discussing paternal mental health during contacts with families, and four main barriers impeding the assessment of paternal mental health were identified. Respondents identified that paternal mental health training would support their own practice.

For some fathers the transition to parenthood is an emotional time, affecting their physical and emotional wellbeing. This review aimed to examine fathers' wellbeing needs and support preferences in the first postnatal year, and explore how health visitors can help. A systematic meta-synthesis was conducted and studies published between 2009 and 2019 analysed using Prisma and CASP tools. A total of 15 studies were identified and the following themes extracted: forming an identity as a father; relationships; and emotional/physical wellbeing and father's support preferences. There were five sub-themes: work/home role; supporter/protector role; parental relationship; and father–baby relationship and father/health relationship. It is concluded that emotions relating to role negotiation, self-efficacy and separation/rejection affected wellbeing, and fathers felt ignored and sidelined by health professionals. Support preferences included more knowledge on the postnatal period, father/nurse groups, targeted leaflets and direction to online services. Health visitors need to improve awareness around paternal wellbeing, starting with SCPHN training. Inviting fathers to all appointments or innovative telephone contacts would promote a more father-focused service.

The adverse impact of preterm birth on maternal mental health is well documented, yet there is limited available research regarding the subjective experiences of mothers of extremely preterm infants as a distinct cohort. The aim of this study was to explore the lived experiences of mothers of extremely preterm infants, born alive at less than 28 weeks gestation, in the UK. Three mothers of extremely preterm infants participated in a semi-structured interview conducted online, which invited them to share their experiences of pregnancy, their time in neonatal intensive care unit, the transition home and their present-day experiences. An interpretative phenomenological analysis explored a delayed transition to mothering and experiences relating to initial disconnection, missed moments and establishing a maternal connection. Owing to the traumatic experiences associated with an extremely preterm birth, it is concluded that mothers would benefit from health visitor support relating to attachment, mental health and connecting to parenting communities.

Perinatal mental illness (PMI) often goes undiagnosed and untreated. Early identification of risk factors can help women receive timely intervention to reduce associated child and maternal comorbidities. This article considers the use of a validated screening tool to identify maternal risk factors of developing PMI. An analysis of a perinatal mental health policy, literature review and project implementation plan were carried out to explore current gaps in the research. Validated perinatal mental health risk assessment screening tools are being used across the world but not in the UK. It is concluded that the Antenatal Risk Questionnaire should be recommended for health visiting practice to promote early identification of PMI and intervention, which can improve outcomes for families.
