A study of the prevalence of serious sexual assualt in childhood (up to age 16) is reported in the context of a community mental health study in a random sample of 377 women in a large Canadian city. Twenty-two percent of women reported sexual abuse in childhoos. Sexually abused women were twice as likely to have poor mental health as women who were not abused. The implications are addressed for helping adults whose poor mental health reflects the complex interaction of negative childhood events, including sexual abuse.

Many individuals who are sexual offenders can be helped by making use of fantasy training as an additional approach to traditional psychotherapy. Many non-psychotic persons who are characteristically action-prone, aggressive, impulsive and socially deviant generally exhibit meagre fantasy skills. Others are given to a repetitive obsessive fantasy that spills over into action. Because there is a linkage between fantasy and behavior, fantasy training can be utilized to reduce the acting-out behavior of these individuals. Fantasy experiences are typically underused in psychotherapeutic intervention. Because they have potential for increasing the adjustment captabilities of many sexual offenders, much greater consideration should be given to the teaching of the imaging process as a treatment technique. Amplification of this information and how the instruction of fantasy process can be done will be discussed.

Women who were sexually abused in childhood often hold distorted beliefs arising from this experience that appear to contribute to mood distrubances such as guilt, low self-esteem, and sadness. A cognitive restructuring intervention was implemented with the aims of correcting these beliefs and alleviating the accompanying mood states. Clinically and statistically significant improvements in both beliefs and mood are reported for a series of 11 clients.

This account of a thirty-five year-old legally blind man who suffers from retarded ejaculation discusses a number of issues concerning the pathogenesis and treatment of this dysfunction among blind men. The problems involved in living from birth with limited vision often result in difficulties in the separation-individuation process and in the establishement of a solid sense of self. The distortions of psychological development resulting from the existence of a visual impairment may well give rise to a sexual dysfunction. Treatment of this man included intrapsychic, systems and behavioral methods which furthered the separation-individuation process and enhanced his self-esteem. The resulting psychological growth led to the client's greater sexual enjoyment. This case study suggests that an understanding of the specific deficits which result from different physcial impairments may aid mental health professionals design programs for the prevention and treatment of sexual dysfunctions in disabled people.

Despite the changes in sexual attitudes and behavior since the 1960s, individual still experience discomfort in talking about sex. This article focuses on the important first step in social work intervention in sexual problems: overcoming clients' resistances to divulging and exploring sex-related concerns. It is suggested that client resistances often stem from social workers' discomfort and avoidance of the subject. Suggestions are offered for overcoming both client and worker resistances.

The negative influences of the black community and the gay community as social identity elements of black gays' self-concept are heuristically explored. The discussion points out how racism in the gay community, and the anti-homosexual attitudes of the black community may result in black gays developint inadequate coping techniques and poor self-concepts exhibiting sex role and racial stereotypes.

The range of psychological problems that are most likely to require therapeutic attention in programs for women who were sexually abused in childhood are reviewed. In a series of 22 such women over three-quarters experienced low self-esteem, guilt, depression, and a variety of interpersonal difficulties. At least half these clients complained of some impairment in their sexual functioning. Therapeutic provision for suicide attempts and substance abuse is also very necessary with this client group.

Lesbians encounter many problems in obtaining adequate health care from traditional health care systems that are organized on heterosexual assumptions. This article discusses the special health care needs of lesbians and the implications for social workers when serving lesbians in relation to illness and health care. The author's survey of social work courses in health care reveals that they are seriously deficient in content relating to lesbians and gays.

This article explores issues of human sexuality and feminism as they apply to perinatal social work services in high-risk reproductive situations. Childbearing will be examined in four phases: pre-pregnancy and conception, pregnancy, labor and childbirth, and post-partum. Although the multiple issues of each phase are recognized, the article will concentrate primarily on the sexual issues. The Women's Movement has influenced changes resulting in a more humane approach to childbearing. The choices now available to women will provide a forum for discussion of interventions to be made by perinatal social workers in high-risk reproductive situations.
