BACKGROUND: Encephalopathy severity in neonates evaluated for therapeutic hypothermia (TH) has traditionally been determined by neurologic exam, sometimes alongside amplitude-integrated electroencephalogram (aEEG). The original 1976 report describing the progressive stages of encephalopathy (PMID 987769) led to modified versions of the Sarnat exam, which were utilized in randomized trials evaluating TH, with or without aEEG. However, there has been minimal validation of exam scores as measures of encephalopathy, distinct from outcome, and the relationship between neurologic exam, aEEG, and conventional EEG (cEEG) is not fully understood. Determining the best method to identify encephalopathy severity is critical to select neonates for TH. We developed a novel, simplified encephalopathy scale at …

Background: For infants with NEC treated non-surgically, there is no consensus for when to re-initiate enteral feeds after NEC. In a previous meta-analysis, earlier refeeding (4-7 days after diagnosis of NEC) suggested a decreased incidence of the combined adverse outcome (NEC recurrence or stricture) and possible other benefits. Based on this analysis, we implemented a standardized guideline for non-surgical NEC through a consensus survey of key stakeholders involved in the decision to start refeeding at our institution. We aimed to implement and monitor a multidisciplinary clinical consensus guideline to …

Introduction Recreational Marijuana legalization has become more popular with each election season. Despite strong recommendations from the American Academy of Pediatrics and the American College of Obstetricians and Gynecologists against the use of marijuana in pregnancy, use by this population is increasing. The 2016 National Survey on Drug Use and Health revealed that 4.9% of pregnant women 15 through 44 years of age reported use of marijuana, a 3.4% increase from the prior year Objective The objective of …

TITLE: What is the optimal heart rate cut-off for initiating chest compressions in neonatal bradycardia? Background: Newborns who receive chest compressions (CCs) and/or epinephrine in the delivery room have a greater incidence of neurological morbidity. Cerebral autoregulation is impaired in newborns and further lost during asphyxia placing them at high risk of ischemia-reperfusion injury. The newborn has a diminished ability to increase stroke volume to improve cardiac output and, therefore, relies primarily on increasing heart rate (HR) to …
