Bladder exstrophy is a rare congenital anomaly displaying symptoms such as the urinary bladder turned inside out, an exposed dorsal urethra, foreshortened penis, anteriorly displaced anus and widely separated symphysis pubis. This abnormality is managed by many methods, one of which includes a functional reconstruction. Themajor goals of surgical intervention are: Preservation of the renal function, achieving urinary continence with adequate sexual function and acceptable cosmetic appearance. In this report we present a long term follow up (25 years) of a male patient with classical type bladder exstrophy, managed at our hospital in stages, he is fully continent and voids spontaneously without clean intermittent self-catheterization, with normal renal function, adequate penile erection and has ejaculation.

DOI: 10.4197/Med. 23.3.1 Abstract Opiophobia is the physicians’ irrational fear of prescribing opioids for patients with severe pain. It’s one of the major reasons for many patients to not receive adequate analgesia (oligoanalgesia). It’s an old phenomenon that still persists in both developed and developing countries with diff erent rates. This is an unsystematic literature review of studies that have been written about opiophobia since 1960 using Google Scholar, PubMed and EBSCO to search for the following terms: opiophobia, oligoanalgesia and necrophobia. Studies included were review articles and surveys studies. We found that the three main reasons for opiophobia are concern over addiction, concern over serious side eff ects (mainly respiratory depression) and concern over legal consequences. These consequences, such as being sued, result from serious side eff ects masking or delaying the diagnosis. Lack of knowledge about pain management was another factor among young physicians and interns. As a result, many patients are not adequately treated for severe pain. Physicians prescribe lesser dosages than required and nurses administer even lesser dosages than the physicians prescribed. To tackle such problems, it’s important to know the causes. Education, clarifi cation of the myths and increasing the physicians’ and nurses’ awareness about pain management is a critically important step especially for new physicians and interns.

Total of 119 patients, retention (n = 30), elective group (n = 89). Retention rate was 25%. There was no eff ect of retention on postoperative complication (elective = 44%, retention = 41%) p value = 0.826, odds ratio 0.878, CI (0.363 2.124) nor any eff ect on patients' satisfaction (elective = 54%, retention = 59%) p value = 0.661, odds ration = 1.256, CI (0.520 3.034). Patients’ age and prostate size did not show any eff ect on postoperative outcome. While presence of infl ammatory cells in benign prostatic hyperplasia (BPH) histopathology showed positive eff ect on satisfaction (BPH alone = 47%, BPH + infl ammation = 71%) p value = 0.037, and a protective eff ect on postoperative complications (BPH alone = 45%, +BPH with infl ammation = 18%) p value = 0.167. Conclusion: Retention rate is comparable to the international reports. The presence of infl ammatory cells in the benign prostatic hyperplasia histopathology showed a positive eff ect on postoperative satisfaction.

The in vitro studies and animal models provide initial evidence suggesting that the knockdown of B-cell lymphoma 11A has a therapeutic eff ect on breast cancer. Defi ning the regulators driving the high expression of B-cell lymphoma 11A is important to understand its cancer-related functions. Among these regulators, transcription factors and microRNAs are critical for gene expression and associated with expression perturbations. Firstly, we identifi ed the transcription factors that potentially interact with B-cell lymphoma 11A promoter. Based on bioinformatics prediction and multiple Omics datasets, two upregulated transcriptional activators Zinc Finger BED-Type Containing 4 and E2F Transcription Factor 1 in triple negative breast cancer were found to have seven sites within B-cell lymphoma 11A promoter. Secondly, we aimed to determine a putative set of microRNA that can mediate the post-transcriptional repression of B-cell lymphoma 11A. miR-513a-5p, miR-139-5p, miR1179, miR-140-5p, and miR-542-3p, harboring at least one site of interaction with B-cell lymphoma 11A 3' untranslated region, were found inhibited in triple negative breast cancer. Taken together, the combinatorial regulation by transcription factors and microRNAs provide valuable information for further investigation on controlling the expression level of B-cell lymphoma 11A in triple negative breast cancer.

Gastrointestinal duplication is a rare congenital abnormality. Over the last 10 years, 10 cases of gastrointestinal tract duplication were managed at King Abdulaziz University Hospital, Jeddah, Saudi Arabia. Duplications were variable in their level and presentation. The experience in the management of the different levels of gastrointestinal tract duplication will be presented along with a suggested general plan for the management of gastrointestinal duplication based on our experience and the collective experience from different reports in the literature.

Deregulation of microRNAs contributes to multiple processes in cancer growth and progression. miR-199a-3p is decreased in highly metastatic breast cancer cells, MDA-MB-231, and its ectopic expression has a potent antimetastatic effect on these cells. However, the mechanism by which miR-199a-3p mediates its antimetastatic function has yet to be elucidated. Because miR-199a-3p reduces the expression levels of its target genes, it is likely to observe an inverse association between miR-199a-3p and its prometastatic target genes at the expression level. The current work determines that the Vesicleassociated membrane protein 3 (VAMP3) expression is increased in highly metastatic breast cancer cells compared to less metastatic cells, Michigan Cancer Foundation-7. The ectopic expression of miR-199a-3p strongly inhibits VAMP3 Messenger RNA and protein in vitro. Herein, it is confirmed that two sites within the 3'-untranslated sequence of VAMP3 Messenger RNA are actively targeted by miR-199a- 3p, discovering a new regulatory mechanism for VAMP3 expression. Functional studies reveal that the suppression of VAMP3 contributes to miR-199a-3p antimetastatic effect, particularly cellular migration in vitro. In conclusion, these results indicate that miR-199a-3p targeting of VAMP3 possesses a significant potential impact in preventing or curing metastatic breast cancers.

Patient satisfaction levels in any health institute are a key performance indicator of quality health care. We constructed a 36-item questionnaire and distributed it over a three-month period (from February to April 2014) to 301 patients treated in various departmental clinics at King Abdulaziz University Dental Hospital. The survey addressed satisfaction with facilities, helpfulness of the staff, progress of treatment, quality of care and treatment with dignity and compassion. Patients were overall satisfied with the treatment received (84.1%), with particular satisfaction for doctors and faculty supervisors being courteous (73.4%, and 61.0%, respectively), and addressing their concern (54.3% and 60.0%, respectively). Dissatisfaction was found with the patient relations’ attitude (48.2%) and professionalism (49.6%), and the majority of patients claimed to be very unsatisfied with the referral system (69.0%) and the difficulty in contacting the clinics (72.0%). The results of the present study revealed that King Abdulaziz University Dental Hospital provides quality dental services to its patients and there is an overall high rate of patient satisfaction for the treatment and services rendered by the institute. However there are still issues and some concerns such as patient relation communication skills and referral system professionalism that have to be addressed in order to improve the patient experience.

The aim of this study is to examine the impact of the process of preservation of both divisions of great auricular nerve on patients' quality of life. This is a retrospective study conducted at King Abdulaziz University Hospital after receiving formal consent from the research ethics committee. For the analysis, charts of all patients who underwent partial parotidectomies (superficial parotidectomies) for benign neoplasm were evaluated and the results were validated. A total 45 cases of superficial parotidectomies were observed, out of which, 15 had preservation of the great auricular nerve (anterior and posterior divisions), and numbness and feeling of well-being were better in this group as compared to the great auricular nerve anterior division sacrifice group. The influence of great auricular nerve sacrifice morbidity on patient’s quality of life varied. Great auricular nerve morbidity requires further eff orts to be made in order to preserve the posterior and anterior branches of this nerve when possible and in the case of the benign lesion, it is considered more suitable.
{"title":"Preservation of the Anterior Branch of the Great Auricular Nerve during Benign Parotid Neoplasm Excision","authors":"Khalid Al-Ghamdi","doi":"10.4197/MED.23-2.2","DOIUrl":"https://doi.org/10.4197/MED.23-2.2","url":null,"abstract":"The aim of this study is to examine the impact of the process of preservation of both divisions of great auricular nerve on patients' quality of life. This is a retrospective study conducted at King Abdulaziz University Hospital after receiving formal consent from the research ethics committee. For the analysis, charts of all patients who underwent partial parotidectomies (superficial parotidectomies) for benign neoplasm were evaluated and the results were validated. A total 45 cases of superficial parotidectomies were observed, out of which, 15 had preservation of the great auricular nerve (anterior and posterior divisions), and numbness and feeling of well-being were better in this group as compared to the great auricular nerve anterior division sacrifice group. The influence of great auricular nerve sacrifice morbidity on patient’s quality of life varied. Pseudoxanthoma elasticum-like papillary dermal elastolysis is a relatively rare disorder within the literature. Increased awareness of this entity will broaden the differential diagnoses among dermatologists encountering such a condition. Pseudoxanthoma elasticum-like papillary dermal elastolysis tends to occur predominantly in elderly females. Clinically, the lesions present as papules and cobblestone plaques on the neck, resembling pseudoxanthoma elasticum. Histologically, there is absence of elastic fibers in the papillary dermis, and no calcification of the remaining fibers. Pseudoxanthoma elasticum-like papillary dermal elastolysis differs from Pseudoxanthoma elasticum by the absence of systemic manifestations, hence patients are reassured and no further workup is warranted. As of now, no effective treatments are available for this entity. Pseudoxanthoma elasticum-like papillary dermal elastolysis should be in the differential diagnosis of a dermatologist when encountering a clinical presentation of Pseudoxanthoma elasticum in an elderly patient localized to the neck, with absence of fragmented and calcified elastic fibers on histologic examination.

This retrospective chart review was conducted to determine the proportion of pregnant women with asymptomatic bacteriuria who were followed up at the antenatal clinics of King Abdulaziz University Hospital, Jeddah between January 2007 and January 2012. Clinical and paraclinical data were recorded for 337 women in their last trimester of pregnancy. The prevalence of asymptomatic bacteriuria in the cohort was 31.26%. The most frequently isolated bacteria included Escherichia coli and Klebsiella pneumoniae in 30.39% and 10.78% of the cases, respectively. In most cases, the isolated bacteria were sensitive to ampicillin (11.57%), amoxicillin (10.38%), and amikacin (5.63%). Most bacteria were resistant to nitrofurantoin (21.06%), tetracycline (21.06%) and cefixime (20.77%). The most common prescribed medications included multivitamins (n = 40; 11.87%), vitamin B12 (n = 47; 13.95%), folic acid (n = 45; 13.35%), and antibiotics (n = 43; 12.75%). Overall, it is possible that pregnant women with asymptomatic bacteriuria are undertreated at our institution. Gaps in the management of these patients should be identified to improve patient care.
