Background: Strategic purchasing plays an important role in the cost structure and competitiveness of small and large businesses. Managing Board of Social Insu-rance (BPJS) has implemented strategic purchasing of health services in the National Health Insurance (JKN) scheme. Strategic purchasing is important to provide quality products and services at low costs. Strategic purchasing of health service has five critical criteria: (1) what is to purchase; (2) to whom it is for; (3) whom it is from; (4) what is the price; (5) how to purchase it. This study aimed to assess BPJS barrier to strategic purchasing of primary care service at public and private health facilities in Subulussalam, Aceh. Subjects and Method: This was a qualitative study conducted in Subulussalam, Aceh, from February to July 2015. A total of 12 key informants for this study, including BPJS administrators and hospital managers. The theme was strategic purchasing of health service. The data were collected by in-depth interview, direct observation, and document review. The data was analyzed by thematic analysis. Results: BPJS administered the same standards of strategic purchasing health service from the public and private health providers. However, there were some unresolved problems: (1) BPJS faced difficulty to select primary care providers independently; (2) Illegal use of other members JKN card to get health service; (3) Redundant claims from the primary care providers; (4) Claim of inpatient days exceeding the maximum allowable limit; (5) Delay in non-capitations reimbur-sement payment to primary care provider; (6) Inequitable distribution of health manpower across health facilities. Conclusion: BPJS has implemented strategic purchasing of primary care services with the same standards applied to public and private primary care providers. However, there remain some problems of purchasing health services that call for immediate corrective actions, including routine monitoring, explanation of delay, advocacy, equitable distribution of health manpower. Background: Malnutrition is one of the most important health and welfare problems among infants and young children in developing countries. This study aimed to examine the determinants of weight for age in children under 2 years of age in Sragen, Central Java. Subjects and Method: This was across-sectional study conducted in Jetis karangpung, Kalijambe, Sragen. A total of 107 children under 2 years of age were selected for this study by random sampling. The dependent variable was child growth as measured by weight for age score. The independent variables were exclusive breastfeeding, complementary feeding, immunization status, and child health care. The body weight was measured by a scale. The other data were collected by questionnaire. The data were analyzed by a multiple linear regression. Results: Weight for age score increased with exclusive breastfeeding (b= 0.49; p= 0.025), complementary feeding (b= 0.26; p<0.001), immunization status (b= 0.65; p= 0.003), child health care (b= 0.59; p<0.001). Conclusion: Weight for age score increases with exclusive breastfeeding, complementary feeding, immunization status, child health care. Background: The importance of antenatal care (ANC) cannot be understated because it could detect abnormalities and preventable illnesses in mother and child, including low birth weight (LBW). This study aimed to determine factors associated with ANC uptake and LBW in Kupang, East Nusa Tenggara. Subjects and Method: This was a cross-sectional study conducted at several Puskesmas (Community Health Care) in Kupang, East Nusa Tenggara, in February and March 2015. A sample of 39 pregnant mothers were selected for this study. The dependent variable were ANC uptake and incidence of LBW. The independent variables were maternal education, husband support, general health check ups. The data were colected by questionnaire and maternal and child health record. The data analyzed by a multiple logistic regression. Results: ANC uptake decreased with lower maternal education (OR= 0.12; 95% CI= 0.02 to 0.69; p= 0.017), weak husband support (OR= 0.06; 95% CI= 0.06 to 0.67; p= 0.022), and absence of general check ups (OR= 0.18; 95% CI= 0.04 to 0.83; p= 0.028). Regular ANC uptake was associated with lower risk of LBW (OR= 0.54; 95% CI= 0.32 to 0.94; p<0.001). Conclusion: Antenatal care uptake decreases with lower maternal education, weak husband support, and absence of general check ups. Regular ANC uptake is associated with lower risk of LBW. Background : An Ni’mah Hospital is a type D hospital located in Banyumas, Central Java. Obstetric and gynecologic services are the main services provided with birth delivery, curettage, and caesarian section, as the most common services . The hospital is financed under the Indonesian National Health Insurance (JKN, BPJS) payment scheme, and it often experiences loss. Recently, the hospital management has implemented a new policy, namely inter-health professional collaboration, which aims to improve hospital performance. This study aimed to examine the effectiveness of inter-health professional collaboration on the impro-vement of in-patient care management at An Ni’mah Wangon Hospital. Subjects and Method : This was a mixed method study carried out at An Ni’mah Wangon Hospital, Banyumas, Central Java. A sample of informants was selected for this study, consisting of gynecologists, general practitioners, nurses, midwives, pharmacists, and nutritionists. The data were collected by questionnaire, interview, observation, and document review. Quantitative and qualitative data were integrated by triangulation to increase data credibility. Results : Paramedics had practiced inter-health professional collaboration. However, inter-health professional communication remained a point of concern due to some hospital internal problem. Paramedics at the medical record unit had worked according to the Standard Operating Procedure (SOP). Overall, patients were satisfied with the collaboration process. However, paramedics in the in-pa-tient unit had not comply with the clinical pathway. The pharmacy and the nut-rition units were under-staffed. Conclusion : The collaboration process has not been fully implemented due to some hospital internal problem. However, overall the patients are satisfied with the inter-health professional collaboration. Paramedics have not implemented clinical pathway, which may lead to in-effective service and some losses to hospital.

Background: Balance is the ability to maintain the center of gravity over the base of support, usually while in an upright position. Balance is a complex function involving numerous neuromuscular processes. The factors having direct influence on the ability to maintain the balancing position may be sensory information, coordination, range of joint movements and muscle strength. Muscle strength is an important aspect of physical fitness and health status, and a decrease of muscle strength may cause significant functional limitations. This study aimed to test the effect on postural balance and anti-gravitation muscle strength in women who do Indonesian healthy tera gymnastics. Subjects and Method: This was a randomized controlled trial carried out in Surakarta. A total of 160 people 55 to 65 years of age were selected for this study and divided into two groups: (1) Tera gymnastics group; (2) Control group. The dependent variables were postural balance and anti-gravitation muscle strength. The independent variable was Indonesian healthy tera gymnastics. Data on postural balance was measured by posturometer, and anti-gravitation muscle strength was measured by dynamometer. The scores between two groups were tested by t-test. Results: Wobble frequency was lower in the gymnastics group than in the control group, and it was statistically significant. Wobble amplitude was lower in the gymnastics group than in the control group, and it was statistically significant. Conclusion: Tera healthy gymnastics is effective to improve postural balance as indicated by low wobble frequency and amplitude. Background: Good nutrition is important for children. Children are at a time in their life when they are constantly growing and learning new knowledge. In order for children to grow properly, they must eat a well-balanced diet. This study aimed to determine the effectiveness of nutrition education in improving knowledge and child nutrition status among mothers who had malnourished children in Pekanbaru, Riau. Subjects and Method: This was aquasi experiment study before and after with control design conducted in Pekanbaru, Riau. A total of 30 mothers who had malnourished infants were allocated in two groups, consisting of 15 mothers with nutrition education in the intervention group, and 15 mothers without nutrition education in the control group. The dependent variable was maternal knowledge in exclusive breastfeeding (EBF), nutritional menu, nutrition parenting, and child nutrition status. The independent variable was nutrition education. Child nutritional status was measured by weight for age Z scores. The other data were collected by questionnaire. Changes in knowledge scores as well as Z scores before and after nutrition education between the intervention and the control groups were compared by t test. Results:Increase of knowledge in EBF before and after in the intervention group was 2.34 unit higher than in the control group (p<0.001). Increase of know¬ledge in nutrition menu before and after in the intervention group was 1.93 unit higher than in the control group (p<0.001). Increase of knowledge in nutrition parenting before and after in the intervention group was 1.73 unit higher than in the control group (p<0.001). Increase in Z score before and after in the intervention group (0.6) was higher than in the control group (0.24) with p<0.001. Conclusion: Nutrition education is effective to improve maternal knowledge in EBF, nutrition menu, nutrition parenting, and to increase nutritional status of infants. Increase of know¬ledge in nutrition menu before and after in the intervention group was 1.93 unit higher than in the control group (p<0.001). Increase of knowledge in nutrition parenting before and after in the intervention group was 1.73 unit higher than in the control group (p<0.001). Background: Having health insurance is important for several reasons. As studies have shown, uninsured people receive less medical care and less timely care. They have worse health outcomes, and lack of insurance is a fiscal burden for them and their families. This study aimed to assess the relationship between health insurance status and perceived quality of health service at community health center, Sumenep, East Java. Subjects and Method: This was a cross-sectional study conducted at puskesmas (community health centers) in Sumenep, East Java. A total sample of 250 people who visited the puskesmas in Sumenep were selected for this study. The dependent variable was perceived quality of health services. The independent variables were health insurance status, sex, age, education, and job. The data were collected by questionnaire and analyzed by a multiple linear regression. Results: Member of health insurance (b= 6.21; CI 95% = 2.75 to -9.68; p<0.001) and female (b= 1.35; CI 95%= -1.35 to -3.23; p<0.001) were associated with better perceived quality of health service. Age (b= 0.50; CI 95%= -2.24 to 3.25; p= 0.72), higher education (b= -0.94; CI 95% = -0.21 to 3.89; p= 0.53), and having a job (b= 0.94; CI 95% = -2.01 to 3.89; p= 0.52) were associated with poorer perceived quality of health services. Conclusion: Member of health insurance and female are associated with better perceived quality of health service. Age, higher education, and having a job are associated with poorer perceived quality of health services.

Background: Diabetes melitus (DM) is a worldwide public health problem and the fourth largest contributor of disease burden in Indonesia. Diabetes mellitus and adherence to treatment management may affect the quality of life of the affected elderly. This study aimed to estimate the associations of adherence to exercise, diet, and treatment, respectively, with the quality of life of the elderly with diabetes mellitus. Subjects and Method: A cross-sectional study was carried out at Puskesmas (Community Health Center) Rowosari, Semarang. The study site is an Interprofessional Education (IPE) field lab operated by Faculty of Medicine, Universitas Diponegoro. Target population was the elderly with diabetes mellitus diagnosis. A random sample of 62 older patients with diabetes mellitus visiting Puskesmas Rowosari was selected for this study. The dependent variable was quality of life. The independent variables were adherence to exercise, diet, and treatment. Dietary pattern was measured by 3 day food recall. Quality of life was measured by Short Form-36. The other data were collected by questionnaire. The data were analyzed by Chi Square with Prevalence Ratio (PR) as the measure of association. Results: Bivariate analysis showed that quality of life improved with adherence to exercise (PR=8.67; 95%CI=2.09 to 35.89; p=0.001), adherence to diet (PR=6.39; 95%CI=1.32 to 30.92; p=0.012) and adherence to treatment (PR= 28.75; 95%CI=5.85 to 141.33; p<0.001), respectively, among the elderly with diabetes mellitus. Conclusion: Adherence to exercise, diet, and treatment, improves the quality of life of the elderly with diabetes mellitus.

Background: Measuring and monitoring a child’s growth is important to judge the adequacy of diets or supplementary food being given. Monitoring child's development is critical to find out if a child's development is on track. It is important to act early if there are signs of potential development delay because early treatment is so important for improving a child's skills and abilities. This study aimed to determine effect of child growth and development training on the knowledge and attitude among community health workers in Semarang, Central Java. Subjects and Method: This study was a quasi-experimental with pretest and posttest with no control design conducted at Rowosari Puskesmas (community health center), Semarang, Central Java. The study subjects involved 62 community health workers (CHWs). The dependent variables were knowledge and attitude about child growth and development. The independent variable was training on how to measure and monitor child growth and development. The data were collected by questionnaire, and analyzed by Wilcoxon test. Results:Knowledge was higher after training (Mean= 77; SD= 9.8) than before (Mean= 68; SD= 11.2), and it was statistically significant (p= 0.001). Attitude was higher after training (Mean= 80; SD= 12.8) than before (Mean= 70; SD= 10.1), and it was statistically significant (p= 0.001). Conclusion: Training is effective in improving knowledge and attitude about child growth and development monitoring among CHWs.

Background: Pyonephrosis is an infection of the kidneys' collecting system. Pus collects in the renal pelvis and causes distension of the kidney. It can cause kidney failure. This study aimed to describe the epidemiology and microorganism profile of pyonephrosis. Subjects and Method: This was a cross sectional study carried out at Sultan Agung Islamic Teaching Hospital, Semarang, Central Java. A sample of 42 inpatients with pyonephrosis was selected for this study. The dependent variable was microorganism profile. The independent variables were age and gender. Epidemiological data were taken from the medical record. Microorganism profile and sensitivity were analyzed from pus collected from surgery. Culture was performed using Blood Agar Plate and Mac Conkey technique. Sensitivity to antibiotic was tested by CLSI diffusion method. Microorganism profile was described in percent. Results: Of all 42 patients with pyonephrosis, 38 (90.5%) patients aged 26 to 64 years and 23 (54,76%) patients were male. Of all types of gram-negative microorganism, 12 (28.57%) were Escherichia coli, 5 (11.90%) Klebsiella pneumonia, 3 (7.1%) Pseudomonas aeruginosa, and 1 (2.3%) non typhoidal Salmonella. Of all types of gram-positive microorganism, 2 (4.7%) were Staphylococcus aureus and 1 (2.3%) Streptococcus viridans. Only 2 (4.7%) were anaerobic bacteria. Some samples were sterile. As many as 2 (16%) were Escherichia coli that produces extended spectrum beta lactamase (ESBL), which is an isolate resistant to the third generation Cephalosporin. All (100%) Staphylococcus aureus were methicillin sensitive. Conclusion: Most patients with pyonephrosis were adults. Percents of male and female patients were comparable. Gram-negative microorganisms outweigh gram-positive microorganism.
