病人服務滿意度是臨床病人照護面及護理品質的重要指標項目之一。調查2019年8-10月共13位初次血液透析門診病人,護理指導滿意度僅有3.32分。分析原因為:護理指導工具不符合現況所需、護理指導方法不夠多元化、缺乏完善的護理指導作業標準流程規範。故選定八項改善方案:(1)舉辦護理指導方法教育訓練;(2)修訂護理指導作業標準流程;(3)制定護理指導作業稽核方法;(4)修訂護理指導單張;(5)錄製血液透析護理指導相關影片;(6)設立Facebook護理指導群組並建立QR code連結;(7)製作透析用導管傷口及手臂動靜脈瘻管模型;(8)使用透析飲食紅綠燈認知測驗盤進行飲食認知評值。經專案措施介入後,初次血液透析門診病人護理指導滿意度由3.32分提升至4.46分,達成專案目的。希望藉由專案分享,完善的護理指導措施介入,幫助初次血液透析門診病人自我照顧能力的增加,提升最佳透析生活品質。 Patient satisfaction is one of the important indicators of clinical patient care and nursing quality. A survey filled out by 13 outpatients who underwent their first hemodialysis during the period of August to October 2019 indicated that satisfaction with nursing guidance was only 3.32 points. The reasons included the following: the nursing guidance tools did not meet the needs of current situations, the nursing guidance methods were not diversified enough, and the lack of a completely standardized operating procedure for nursing guidance. Therefore, eight improvement plans were selected: (1) to hold nursing guidance method educational trainings, (2) to revise the standard operating procedure of nursing guidance work, (3) to develop nursing guidance audit methods, (4) to revise nursing guidance leaflets, (5) to record videos relating to hemodialysis nursing guidance, (6) to establish a Facebook nursing guidance group and create a QR code link, (7) to make hemodialysis catheter wounds and arm arteriovenous fistula models, (8) to use hemodialysis diet traffic light cognitive test panel for diet cognition evaluation. After the implementation of the measures, the satisfaction of outpatients’ care guidance for those undergoing their first hemodialysis increased from 3.32 points to 4.46 points, thus fulfilling the purposes of the project. It is hoped that through the sharing of the project and the implementation of comprehensive nursing guidance measures, it will help outpatients undergoing their first hemodialysis increase their self-care ability and improve quality of hemodialysis life.

本研究旨在探討國小聽障生級任教師社會技能教學現況,研究對象為台灣地區國小聽障生之級任教師,以各縣市為單位進行叢集抽樣。隨機寄出654份問卷進行調查,回收有效問卷512份,問卷回收率為78.28%。本研究發現如下: 一、國小聽障生級任教師進行社會技能教學的現況傾向中等以上的反應。 二、國小聽障生級任教師在進行社會技能教學時的整體教學現況不會因國小聽障 生之「性別」、「年級」、「左耳聽能輔具」、「右耳聽能輔具」、「家庭經濟狀況」、 「父親聽能狀況」、「母親聽能狀況」等變項的不同而在社會技能教學總量表 與三個分量表中有所差異;但在「教育安置方式」、「發現失聰年齡」、「聽覺 障礙程度」、「學業表現」、「主要溝通方式」方面呈現差異。 三、國小聽障生級任教師在社會技能教學三個分量表單題平均得分之差異,結果 最佳為「社會技能教學結果」、最劣為「社會技能教學過程」。 四、國小聽障生級任教師在社會技能教學三個分量表得分彼此間呈現中度之相關 程度。 五、不同背景變項國小聽障生對級任教師在社會技能教學之預測無具太大實質意 義。 六、國小聽障生級任教師的社會技能有效教學預測力分析中,以社會技能教學過 程最能進行預測。 This study aimed to investigate the current status of regular classroom teachers’ social skills teaching for elementary students with hearing impairment. The data were collected from regular class room teachers of elementary students with hearing impairment in Taiwan, using cluster sampling. The clusters were counties and cities. Among 654 random questionnaires, 512 were returned with response rate of 78.28%. The major things were as followed: 1. Teachers’ current status of social skills teaching for elementary students with hearing impairment tended to be above the average as a whole. 2. Teachers’ social skills teaching as well as the three subscale (objective, process and result of social skills teaching) were associated with elementary students’ with hearing impairment educational implements, age of deaf, impaired level of hearing retardation, academic achievement, and main communication method. 3. By comparing the scores of three subscale (objective, process and result of social skills teaching), it was shown that ‘the result of social skills teaching’ was the highest, while ‘the process’ was the lowest. 4. The correlations between the three subscales (objective, process and result) were around medium level. 5. The predictive effects of elementary students’ with hearing impairment backgrounds on teachers’ current social-skill-teaching situations were practically nonsignificant. 6. Regular classroom teachers’ effectiveness of social skills teaching could be predicted from the process of social skills teaching which was the best predictor among the three subscales.

本文描述一位血液透析個案接受腎移植後出現急性排斥之護理經驗,照護期間為2020年1月6日至2020年1月31日。筆者藉由會談、傾聽、觀察、身體評估、病歷查閱等技巧收集資料,確立有急性疼痛、潛在危險性感染、焦慮等健康問題。運用Watson關懷照護理論,與個案建立良好的護病關係,提供個別性護理。針對術後急性疼痛,提供疼痛緩解技巧,如:呼吸控制等,並合併藥物介入,達到疼痛控制;潛在危險性感染風險,著重加強傷口照護,及出院後預防感染居家護理,加上雙重血漿置換後導致白蛋白低於正常值,更運用跨團隊會診營養師,提供高蛋白等營養攝取資訊;當對未來出現不確定,及對不熟悉的治療產生焦慮不安時,除給予治療前詳細介紹外,更運用關懷、聆聽、信仰等,加強其心理與靈性支持,增加個案的自信和希望,使其能以正向態度開始新生活。期望藉由此經驗分享在照護類似個案時,能提供更適切的護理照顧,作為日後臨床照護之參考。 This study describes the experience of providing nursing care to a hemodialysis patient who experienced acute rejection following a kidney transplant. The period of care lasted from January 6, 2020 to January 21, 2020. Data was collected through conversations, listening sessions, observations, physical assessments, and medical record reviews. The patient was found to be suffering from health problems such as acute pain, a potential risk of infection, and anxiety. Watson’s theory of human caring was applied to establish a strong nurse-patient relationship and to provide personalized nursing care. The patient’s acute postoperative pain was managed and controlled by means of pain alleviation methods such as respiratory control and medication treatment-based interventions. The patient’s potential risk of infection was managed by focusing on the implementation of wound care as well as infection prevention measures at the patient’s home after discharge. Furthermore, since the patient’s albumin level was lower than normal following a double filtration plasmapheresis, a multidisciplinary team was formed, and the patient consulted with a dietician who provided nutritional recommendations such as having a high protein diet. When the patient expressed uncertainty about the future or felt anxious about unfamiliar treatments, the team not only thoroughly described the treatments beforehand, but also enhanced the mental and spiritual support that they provided by expressing concern about, listening to, and placing faith in the patient, with the purpose of boosting the patient’s self-confidence and sense of hope. As a result the patient was able to begin a new life with a positive attitude. This experience is expected to serve as a reference for providing more appropriate nursing care plans for similar patients in clinical settings in the future. 
本文描述一位血液透析个案接受肾移植后出现急性排斥之护理经验,照护期间为2020年1月6日至2020年1月31日。笔者借由会谈、倾听、观察、身体评估、病历查阅等技巧收集资料,确立有急性疼痛、潜在危险性感染、焦虑等健康问题。运用Watson关怀照护理论,与个案建立良好的护病关系,提供个别性护理。针对术后急性疼痛,提供疼痛缓解技巧,如:呼吸控制等,并合并药物介入,达到疼痛控制;潜在危险性感染风险,著重加强伤口照护,及出院后预防感染居家护理,加上双重血浆置换后导致白蛋白低于正常值,更运用跨团队会诊营养师,提供高蛋白等营养摄取资讯;当对未来出现不确定,及对不熟悉的治疗产生焦虑不安时,除给予治疗前详细介绍外,更运用关怀、聆听、信仰等,加强其心理与灵性支持,增加个案的自信和希望,使其能以正向态度开始新生活。期望借由此经验分享在照护类似个案时,能提供更适切的护理照顾,作为日后臨床照护之參考。 This study describes the experience of providing nursing care to a hemodialysis patient who experienced acute rejection following a kidney transplant. When the patient expressed uncertainty about the future or felt anxious about unfamiliar treatments, the team not only thoroughly described the treatments beforehand, but also enhanced the mental and spiritual support that they provided by expressing concern about, listening to, and placing faith in the patient, with the purpose of boosting the patient’s self-confidence and sense of hope. As a result the patient was able to begin a new life with a positive attitude. This experience is expected to serve as a reference for providing more appropriate nursing care plans for similar patients in clinical settings in the future.

背景:透析病人鈣磷乘積增加會導致副甲狀腺機能亢進、血管鈣化並提高死亡率。統計2018年1至6月腹膜透析病人鈣磷乘積小於60 mg²/dL²的比率平均為73.2%,未達閾值且低於健保監測標準故引發動機。目的:本專案旨在降低腹膜透析病人鈣磷乘積以達標準75%。解決方案:分析確立問題為病人對疾病認知不足、飲食攝取知識不足、藥物服用遵從性不佳、輔助工具不足。運用介入措施包括提供血磷檢驗趨勢圖、小組團體衛教、高磷影音教學、手機查詢APP及成立專案衛教Line群組等方案。結果:專案推動後,病人鈣磷乘積小於60 mg²/dL²的比率,由2018年6月72.8%至2019年1月提高到82.2 %,已達閾值及健保監測標準。結論:此專案運用團體衛教、影音教學及行動裝置,加強病人自我照顧能力,確實能有效提升護理指導成效,改善照護品質。 Background:The increased calcium -phosphorus product in dialysis patients will lead to hyperparathyroidism,vascular calcification and increase mortality. Statistics from January to June 2018, the ratio of calcium - phosphorus products less than 60 mg²/dL² averaged 73.2% of patients with peritoneal dialysis, which did not reach the threshold and was lower than the health care monitoring standard, so the engine was cited.Purpose:This project aims to reduce the calcium - phosphorus product of peritoneal dialysis patients to reach the standard 75%.Resolution:We analyzed and established that the problems were inadequate patient awareness of the disease, insufficient knowledge of dietary intake, poor compliance with medication, and insufficient aids. The intervention measures include providing trend chart of blood phosphorus test, group health education, high phosphorus audio-visual teaching, mobile phone query APP and establishment of Line group.Results:After the promotion of the project, the ratio of calcium-phosphorus product less than 60 mg²/dL² increased from 72.8% in June 2018 to 82.2% in January 2019, which has reached the threshold and health care monitoring standards.Conclusion:This project uses group health education, audio-visual teaching and mobile devices to strengthen patient’s self-care ability, which can effectively improve of nursing guidance to improve the quality of care.

華德福實驗教育學校強調靈性教育,在臺灣實驗教育發展中佔有一席之地。本研究針對一所華德福實驗教育學校進行個案研究,主要藉由訪談法探討校長的課程領導行為,並比較其與傳統領導觀點的差異。研究發現個案學校校長將領導力視為一種靈性教育上的努力,透過重視精神感召、批判反思、整體原則、參與式體驗及生命共同體文化的領導行為,在某種程度上轉化了學校三元組織中成員信念與課程結構,發展出在地化的校本靈性課程;而在此過程中,校長亦於課程願景、教師教學素養、課程設計、課程評鑑與學校文化等領導主題上,展現其異於傳統的領導行為。這些研究結果有助於其他學校理解華德福學校靈性教育的領導智慧,並為因應全球化與未來教育的挑戰,提供了重要參考。  The Waldorf School of Experimental Education emphasizes spiritual education and has a place in the development of experimental education in Taiwan. This case study of a Waldford School of Experimental Education, conducted mainly through interviews, explores the principal’s curriculum leadership behavior and compares differences with traditional leadership. The research found that the case school’s principal viewed leadership as a spiritual education effort, with an emphasis on spiritual calling, critical reflection, holistic principles, participatory experience and leadership behavior as part of the culture of a life community, to transform the members’ beliefs and the curriculum structure in the school’s threefold organization, and to develop a localized school-based spiritual curriculum. In this process, the principal also worked on curriculum vision, teacher teaching literacy, curriculum design, curriculum evaluation, and leadership themes such as school culture, to demonstrate leadership behaviors that are different from the traditional ones. These findings help other schools understand the leadership wisdom of Waldorf School’s spiritual education and provide a key reference for addressing the challenges of globalization and the future of education.

背景:腹膜炎是造成腹膜透析患者退出治療(drop-out)的最大原因。本腹膜透析中心位在北台灣的醫學中心,腹膜炎發生率由2014年1.51次/百病人月上升至2015年2.37次/百病人月。目的:(1)了解腹膜炎之原因(2)實施有效再訓練計畫降低腹膜炎。解決方案:發展新版腹膜炎認知及技術評核表、修訂再訓練流程、舉辦團體衛教、張貼衛教海報及發送衛教單張,並持續追蹤修正計畫。結果:實施新版再訓練計畫後,感染事件由平均8次/月大幅降為4次/月,感染率更降至0.94次/百病人月。結論:修訂再訓練流程,建立一致且持續性認知及技術全面性評核制度,確實能降低腹膜炎。 Background & Problems: The most significant cause for peritoneal dialysis (PD) patients to drop out is peritonitis. Our PD center is located in a medical center in Northern Taiwan. The peritonitis rate of our center increased from 1.51 episodes per 100 patient-months in 2014 to 2.37 episodes per 100 patient-months in 2015.Purposes: To (1) understand the causes of peritonitis among peritoneal dialysis patients, and (2) further implemented an effective retraining program to reduce peritonitis rate.Resolution: We developed a checklist including peritonitis prevention cognition and exchange technique. Retraining program was refined with continuous adjustment. Holding group education, setting up educational posters and delivering leaflets to convey information about peritonitis prevention were also applied.Results: After initiating the refined retraining program, infectious episodes decrease to 4 times/month form 8 times/month on average in the last 3 months. Peritonitis rate significantly went down to 0.94 episodes per 100 patient-months from 2.37 in 2015.Conclusion: The refined retraining program showed that a coherent procedure assessing system and a comprehensive cognition/ technique evaluation could lower the peritonitis rate.

Holding group education, setting up educational posters and delivering leaflets to convey information about peritonitis prevention were also applied.Results: After initiating the refined retraining program, infectious episodes decrease to 4 times/month form 8 times/month on average in the last 3 months. Our PD center is located in a medical center in Northern Taiwan. The peritonitis rate of our center increased from 1.51 episodes per 100 patient-months in 2014 to 2.37 episodes per 100 patient-months in 2015.\u0000Purposes: To (1) understand the causes of peritonitis among peritoneal dialysis patients, and (2) further implemented an effective retraining program to reduce peritonitis rate.\u0000Resolution: We developed a checklist including peritonitis prevention cognition and exchange technique.
