腹膜透析發生腹膜炎不僅會影響透析效果,嚴重時需終止腹膜透析治療,佔腹膜透析病人致死因素的15%。專案期間為2019年4月1日至9月30日,目標在改善腹膜炎發生率。經現況分析發現腹膜炎發生率高的原因有:病人及主要照顧者未正確執行換液技術、認知不足;未明確增加技術稽核時機、缺乏相關衛教指導、提醒標語以及多元衛教輔助工具。藉由修訂腹膜透析換液技術標準,提升換液技術正確率、加強腹膜炎相關認知、增加技術稽核時機、製作相關衛教單張及提醒標語、使用多元衛教輔助工具等改善對策後,腹膜炎發生率由3.62次/100人月降至1.55次/100人月。本專案運用多元策略降低腹膜炎發生,提高腹膜透析治療的自我照顧認知與技巧,進而提升腹膜透析醫療照護品質。 Peritonitis will not only influence the efficacy of peritoneal dialysis (PD), but it is necessary to terminate PD treatment in severe cases. It has been reported that peritonitis-related deaths account for 15% of the mortality of PD patients. The project period is from April 1st to September 30th, 2019. The goal of this project is to implement multiple strategies to reduce the incidence of peritonitis. According to our present analysis, we found that the high incidence of peritonitis is due to incorrect execution and under-cognition of fluid exchange technique by the patient and main caregiver, no well-established timing of technical audits, lack of relevant health education guidance, reminder slogans and various health education aids. By revising the technical standards for PD fluid exchange, improving the accuracy of exchange technique, strengthening the awareness of peritonitis, increasing the timing of technical audits, making relevant health education leaflets and reminder slogans, using various health education aids and other improvement measures, the occurrence of peritonitis events dropped from 3.62 times/100 person-months to 1.55 times/100 person-months. In conclusion, this project used multiple strategies to reduce the occurrence of peritonitis, enhance self-care awareness and skills in PD treatment, and thereby improve the quality of PD care.

醫療廢棄物若處理不當,不僅讓人員暴露在高風險環境中,亦會造成環境的危害,並提高處理成本。本單位醫療廢棄物分類稽核異常率高達20.7%,引發專案小組改善動機,目的在提升生物醫療廢棄物之分類正確率。本專案執行期間自2019年1月1日至12月31日,根據現況分析發現問題:醫護人員、病人與陪病者對廢棄物分類認知不足、護理指導工具不足、廢棄物桶標示不清、工作車的分類設備不完善、缺乏考核獎懲制度。經由:安排在職教育訓練、製作標準分類提醒圖表、製作衛教手冊及海報、拍攝宣導短片、製作分類清楚標示圖卡、增加行動護理車分類設備和設計可分類完善收針車、訂定完整稽核制度及獎懲辦法。結果分類行為正確率由57.9%提高至93.3%;分類稽核異常率由26.4%降為5.1%;醫護人員認知由70分提高至100分;民眾認知由52分提高至85分,達到專案目標,有效提高醫療環境的安全及照護品質。 Background & Problems: The most significant cause for peritoneal dialysis (PD) patients to drop out is peritonitis. Our PD center is located in a medical center in Northern Taiwan. The peritonitis rate of our center increased from 1.51 episodes per 100 patient-months in 2014 to 2.37 episodes per 100 patient-months in 2015. Purposes: To (1) understand the causes of peritonitis among peritoneal dialysis patients, and (2) further implemented an effective retraining program to reduce peritonitis rate. Resolution: We developed a checklist including peritonitis prevention cognition and exchange technique. Retraining program was refined with continuous adjustment. Holding group education, setting up educational posters and delivering leaflets to convey information about peritonitis prevention were also applied. Results: After initiating the refined retraining program, infectious episodes decrease to 4 times/month form 8 times/month on average in the last 3 months. Peritonitis rate significantly went down to 0.94 episodes per 100 patient-months from 2.37 in 2015. Conclusion: The refined retraining program showed that a coherent procedure assessing system and a comprehensive cognition/ technique evaluation could lower the peritonitis rate.

本文描述一位因腎功能損傷伴隨致命性高血鉀風險,使其面臨初次血液透析之護理經驗,經團隊評估後,需長期血液透析的個案,其日常生活受限、家中角色改變進而產生無力感,照護期間為2020/1/16~2/2。護理過程中藉由直接照護、身體評估、主動關懷及開放性會談等方式收集資料,發現主要健康問題有:一、呼吸喘,二、透析飲食執行度不佳,三、無力感。照護期間透過圖表(卡)教導個案測量水腫情況,並透過水壺、磅秤讓個案學習飲食及水份控制技巧,與其搭配營養師衛教及食品營養成分資料庫,增加個案對飲食資訊管道,並考量個案生活習慣下建立提醒機制,如:製作小吊飾或手機鬧鈴等個別性護理措施,加上家人支持、宗教信仰成為心靈上慰藉及腎友的經驗分享,引導個案接受目前扮演的角色,與疾病共存生活。建議往後照顧類似案例能融入家屬的參與,並思考如何強化醫療成員自我主動學習心理諮商輔導課程意願,讓往後面對此案例時能有更好的因應之道。 This paper describes the case of a patient with worsening renal function accompanied by electrolyte disturbance who required long-term hemodialysis after assessment by a medical team. The patient felt helpless due to restrictions in daily life and changes in their family role. The care period was from January 16 to February 2, 2020. In this period, observations, direct care, physical evaluation, medical record inspection, active concern, and open interviews were used for collecting information. Major health problems found included the following: 1. shortness of breath; 2. poor execution of dialysis diet; and 3. a sense of powerlessness. During the care period, picture teach the case to measure edema, and let the case study dietary water control skills through the use of water bottles and scales, nutrition-related information websites and several types of reminder mechanisms were employed, such as using cell phone charms or mobile alarms, to help the patient understand correct information on various dietary components and accept their current family role. This was done to make living with the disease easier for the patient. We recommend the inclusion of family participation for future studies on care of patients with similar conditions. Also, we should think of devising ways for increasing medical staff’s willingness to proactively take psychological counseling courses so that they can respond better to such cases in the future.

本研究討論影響地方工作者動機與持續實踐行動的主客觀因素。本研究所稱的地方工作者,指的是其實踐行動有助提升在地地方感的行動者。基於20位受訪者的訪談、參與觀察以及相關次級資料,本研究做出以下結論:從鉅觀到微觀,各種社會因素觸發行動者投入地方工作,不論是讓行動者產生關懷在地的動機、行動,或是讓已有動機的行動者能付諸實踐。除了對議題的關心,這些實踐的行動更與行動者的自我實現互相關聯。隨行動者持續展開行動,其地方工作的身分逐漸明確,同時也在過程中獲得所需的各種資本,這些資本具有一定的在地專屬性,產生的鎖入效果也讓行動者持續投入地方工作。 This study investigated why individuals want to be community workers and how they maintain their practices. The conclusions presented herein are based on 20 interviews, participant observation, and secondary data. Social factors at different levels inspire individuals to value some aspects of the community and to better their community. Community workers often see their community service as part of their self-actualization. As they continue to engage in community service, community workers develop their identities as community workers, and the capital required for their service accumulates. Some types of capital are location-specific, which produces a lock-in effect that inspires community workers to continue engaging in community service.

The conclusions presented herein are based on 20 interviews, participant observation, and secondary data. Social factors at different levels inspire individuals to value some aspects of the community and to better their community. Community workers often see their community service as part of their self-actualization. As they continue to engage in community service, community workers develop their identities as community workers, and the capital required for their service accumulates. 醫務社工在管理主義興盛的醫院工作,臨床上與醫療團隊合作密切,因此臨床醫務社工受影響處境與醫療團隊對於醫務社工的認知與期待有關。本文係探討醫務社工與醫療團隊互動下對管理主義的回應策略。本文採取質性研究法,深度訪談醫院主管、社工主管、臨床醫務社工、醫務社工組織代表共18位。研究發現提出管理主義對臨床醫務社工所產生的效應,高度與醫療團隊對臨床醫務社工認知與期待有關。本研究彙整臨床醫務社工的回應策略包含(1)溝通與說服-發揮論述與溝通能力以彰顯專業功能、(2)支持及內控-穩固社工內部以展現自我照顧及成長、(3)教育與傳播-種下改變種子以建立未來友善環境。最後,提出討論與建議,以醫務社工的回應策略做總結。 Medical social workers show suppression of hospital managerialism and expectations of medical teams. 隨著臺灣人口結構老化進程的持續,臺灣在人口政策的回應,已從對人口數量的控制,逐漸轉向照顧等社會再生產相關政策,以回應因人口結構老化而產生的各類照顧問題。然而從既有的研究文獻和社會改革理論來看,從老人照顧、兒童照顧,再到青年就業問題的回應,一方面呈現了人口再生產議題的主要訴求面向,另方面則呈現了其在整合上的困難。特別是在照顧倫理行動的實踐上,現有的政策回應,難以落實個人在生命歷程中與他人的相互依賴和情感需要。而透過對社會投資國家理論與社會不平等的反思,以及社區共同資源理論的對話,我們試著提出藉由在地社區對照顧價值的肯認與倫理行動,以及國家角色的重構等構想,以作為回應台灣社會再生產挑戰的可能途徑。 With the continuation of aging on the demography in Taiwan, the response of population policy has gradually shifted from population control to social reproduction, such as care provision and the promotion of local youth employment. However, viewing from the existing literature and the related policy reform theories, the current responses to the issues of old-aged care, child care, and adult employment have shown on the one hand a predominant appeal on reproduction, and on the other demonstrated difficulties in integration. Specifically on the practice of ethical action, existing policy responses have made it difficult to cater to an individual’s interdependence and emotional needs with others during individual’s life course. Through the reconsidering of social investment state theory and social inequality, as well as the dialogue with the commons’ theory of community economy, we try to propose a possible strategy that recognizes care value and ethical action from the autonomy of local communities, as well as the reconfiguration of the role of the state. We think it can be the possible approach to respond to the challenges of social reproduction in Taiwan.

本文以原住民族文化為主體,探討「原住民族部落文化健康站」發展、內涵、本質、專管中心培力。並建構出照顧服務模式、照顧服務推動難題(1.行政績效面向共三項、2.專業服務面向共五項)。 最後,在未來推動方向提供建議(專業教育訓練形式改變等四項)。再次揭露「原風優先」、「部落自顧」之「原住民族部落文化健康站」照顧服務對原住民族長者在地老化重要性。 This study focuses on the indigenous culture and explores the development, content, nature, and project management training of the ’’center for indigenous tribal culture and health.’’ This study constructs a care-service model and reveals the difficulties of promoting a care service, including three administrative issues and four issues related to professional services. Finally, it provides suggestions for future development, including suggestions for resolving the issues related to changes in the professional educational training. The study highlights how the care services provided by the ’’center for indigenous tribal culture and health,’’ with an emphasis on ’’indigenous culture first’’ and ’’tribal self-care,’’ are important for aging indigenous seniors in the community.

本研究針對我國照顧服務證照體系,採深度訪談及文件分析探討照顧服務員專業證照發展之可行性。研究發現照顧服務員證照考試有其必要,但全面取證的要求難以應付照顧現場對人力需求的急迫;且單一級照顧服務員技能檢定與實務工作所需要的職能存在落差,需調整升級並發展不同級別與類科;其次,照顧服務專業發展需要正規教育,專業職能基準需要產官學界共同合作,然而產官學界對專業發展之途徑各有主張;第三,發展照顧服務專業制度與職涯藍圖有助年輕人力投入長照服務。本研究建議我國照顧服務員證照制度之政策規劃,可訂定短、中、長期目標,並提出政府部門可採行的專業證照發展方案,期透過適當專業分級提升照顧服務品質,吸引及留用優秀人才。 The study utilizes in-depth interviews and document research to explore the feasibility developing a professional care worker certification system in Taiwan. Presently, care workers require a comprehensive certification examination, which is unfeasible due to labor shortage. This study demonstrates the necessity for a certification and licensing examination that bridges these gaps, both in the workforce and in certification. Secondly, there is also a significant gap between the single-level certification exam and competency requirements for practical work, thus qualification exams need to be reworked throughout. Third, developing a care worker certification system requires formal education and the cooperation of the government, academia, and long-term care industry. Fourthly, such a system and career blueprint will encourage and aid youngsters to pursue careers in long-term care services. The findings provide recommendations for policy planning that the government can implement to develop such a system and manifestly improve care service quality in Taiwan.

This paper discusses how to use the Performed Culture Approach (PCA) to design collegiate Chinese beginning and intermediate level courses. As a pedagogical framework to improve learners’ intercultural communication skills, PCA has gained increasing recognition by scholars in the field. However, many instructors still face the challenge of deciding how to apply PCA in different Chinese language programs and with non-PCA informed pedagogical materials. Through a three-year implementation at an American university which adopts Integrated Chinese (IC), one of the most popular pedagogical materials in American college beginning- and intermediate-level Chinese courses, the article analyzes IC’s instructional challenges, PCA-based curriculum design, learning outcomes, and student feedback. The article finally discusses general principles of applying PCA in different programs with various pedagogical materials.
