Epidemiological Features of Leprosy in China, 2020 2020年中国麻风病流行病学特征

Objective: To describe the epidemiological characteristics of leprosy in China in 2020 and provide a basis for optimizing leprosy prevention and control strategies. Methods: We collected leprosy data from the Leprosy Management Information System in China, which includes data from 31 provinces, autonomous regions, and municipalities, except for Hong Kong, Macao, and Taiwan, and then performed a descriptive analysis of these data. Results: A total of 406 newly detected leprosy cases were reported nationwide in China in 2020, giving an incidence rate of 0.29 per 1,000,000 population. Among the population with leprosy, the proportions of children younger than 15 years, multibacillary cases, and cases with grade 2 disability were 1.2%, 93.3%, and 17.7%, respectively. Southwest China was the region with the largest proportion of new leprosy cases (54.7%). There were 31 relapse leprosy cases reported in 2020. By the end of 2020, there were 1,893 registered leprosy cases reported nationwide, giving a prevalence rate of 1.35 per 1,000,000 population. Conclusion: Leprosy is at a low endemic level in China in terms of the prevalence and incidence rates. However, the geographical distribution of leprosy in China is unbalanced; leprosy is highly endemic in southwest China, including Yunnan, Guizhou, Sichuan, Guangdong, and Guangxi.

Livedoid Vasculopathy Secondary to Protein C Deficiency: A Case Successfully Treated With Rivaroxaban 利伐沙班治疗继发于蛋白C缺乏的活体血管病1例

Introduction: Livedoid vasculopathy is a chronic noninflammatory skin disease secondary to hypercoagulable states. No therapeutic guideline has yet been established for livedoid vasculopathy. We herein report a case of livedoid vasculopathy secondary to protein C deficiency that was successfully treated with rivaroxaban. Case presentation: A 31-year-old Thai woman who had been diagnosed with livedoid vasculopathy 10 years previously presented with a 2-month history of worsening leg ulcers and failure to respond to aspirin, colchicine, and pentoxifylline. Further investigations confirmed protein C deficiency. Rivaroxaban was initiated, and clinical improvement was achieved in 8 weeks. Discussion: When livedoid vasculopathy is confirmed by skin biopsy, additional investigations for hypercoagulable states should be performed to exclude secondary causes. Identification of the causes of livedoid vasculopathy can direct physicians to therapeutic options based on previously reported cases of successful treatment. Our patient, whose livedoid vasculopathy was caused by protein C deficiency, responded well to rivaroxaban. Conclusion: Protein C deficiency results in a hypercoagulable state, and affected patients can present with livedoid vasculopathy. The anticoagulant rivaroxaban has been beneficial in the treatment of livedoid vasculopathy.

Contact Dermatitis With the Homoeopathic Medicine Calendula officinalis: A Rare Case Report 顺势疗法药物金盏菊致接触性皮炎1例报告

Introduction: Herbal medicines and other traditional medicines are well accepted in India and are readily available as over-the-counter preparations. With their increased use, the safety and quality of homoeopathic medicines have become a major concern for health authorities, pharmaceutical industries, and consumers. It is difficult to assess the adverse effects of homoeopathic medicines due to a lack of scientific evidence regarding their safety and efficacy in clinical trials or post-marketing surveillance. Here, we reported a patient with contact dermatitis due to the homoeopathic medicine Calendula officinalis. Case presentation: A 33-year-old previously healthy woman presented with pruritus, redness, and swelling over the entire face and upper neck. The symptoms developed within about 1 hour after the use of a homoeopathic skin ointment containing 2% Calendula officinalis as the major ingredient in an aqueous base. Cutaneous examination revealed periorbital edema and facial swelling with multiple vesicular eruptions over erythematous edematous plaques on the face. A clinical diagnosis of contact dermatitis was considered. Discussion: Many herbal preparations in India are available without safety data. Calendula officinalis extract (oil expressed from seeds) is used in various skin products. The exact allergens present in Calendula officinalis are unknown, but the flowers reportedly contain a variety of phenolic acids and phenylpropanoids that have elicited contact dermatitis. Conclusion: Anecdotal case reports of accidental exposure/sensitization and experimental studies provide sufficient evidence to confirm the causal relationship between contact dermatitis with Calendula Officinalis. Post-marketing surveillance of all homoeopathic medicines should be mandatory, and the availability of such preparations as over-the-counter drugs must be restricted.
