Ganglioneuroblastoma is a tumor derived from neuroblasts, generally related to sympathetic structures, which is usually located in the adrenal gland. In this article, we present a rare case of a patient with cervical ganglioneuroblastoma, who developed sleep disorders since the first year of life due to compression of the airway.

Osteomas are rare benign tumors that can be located on the craniomaxillofacial skeleton, being unusual its presence in the soft tissues of the oral cavity. They are also known as osseous choristomas, as they consist of normal matured bone tissue. Osteomas of the tongue occur more frequently in women in their second or third decade and the most frequent location in the oral cavity is the posterior third of the tongue. Most patients are asymptomatic, though other forms of presentation could be complaining of the sensation of having a foreign body or, even, dysphagia. The fact that less than 100 cases have been reported in the literature, has motivated the presentation of this clinical case.

Treatment can be surgical, with more or less extensive surgery, as well as medical with different drugs. We present the case of a patient who underwent surgery for a mandibular GCG who came to our hospital with a recurrence of the lesion, which was managed surgically by segmental resection and reconstruction with a fibula free flap, adding dental rehabilitation to the treatment for a global management of the case.

Endoscopic-assisted approach in the treatment of subcondylar fractures: our experience. 内窥镜辅助入路治疗髁下骨折:我们的经验。

Introduction: The management of subcondylar fractures has been very controversial in the maxillofacial literature. The open reduction and internal fixation (ORIF) technique has been considered by many authors the gold standard in selected cases. However, with the rise in endoscopic techniques in the craniomaxillofacial area, new boundaries and less invasive techniques are being explored. The endoscopic approach of subcondylar fractures has proved overall good and similar results to open approaches whilst reducing complications such as facial nerve injury. In this article we purpose to describe our experience with the endoscopic approach to subcondylar fractures. Patients and methods: We retrospectively analyzed 11 patients with subcondylar fractures treated at our department via an endoscopic approach. The number and type of plates used in each patient is recorded. Results and complications observed for all patients are described as well as functional outcomes in terms of mouth opening at 1 week, 3 months and 6 months postoperatively. Results: One patient presented with transient damage to the marginal and frontal branches of the facial nerve. 18.2 % of patients had their hardware removed due to pain or infection at the fracture site. No cases of salivary fistula or sialocele were found in this study. Mean mouth opening at one week postoperatively was 31.8 mm which increased to 37.8 mm at 6 months after surgery, meaning an increase of 18.86 % through the follow-up. Also, 18.2 % of patients presented with persistent deviation with mouth opening and one patient presented with postoperative persistent malocclusion that was treated with intermaxillary fixation and elastics. Conclusion: The endoscopic management of subcondylar fractures is a safe alternative to the open approach, specially in favorable cases, which reduces the risk of complications associa ted to open approaches, such as unfavorable scarring, salivary gland complications and facial nerve damage. In our series only one patient presented with transient damage to the facial nerve. 18.2 % of the plates were removed, which is a high percentage and should be evaluated, although the small size of the series should be taken into account. Maxillofacial surgeons should be encouraged to learn and trained in endoscopic techniques and include the endoscopic assisted approach in their surgical armamentarium.

Introduction: The treatment of temporomandibular joint (TMJ) disorders is still controversial. TMJ arthrocentesis (AC) is a minimally invasive procedure that can be associated with the intra-articular injection of various therapeutic drugs, including corticosteroids (CS), with no current consensus on which offers the best results. The aim of this review is to evaluate if the use of AC associated with intra-articular injection of CS provides better results in terms of pain reduction and improvement of mandibular function in patients with TMJ disorders compared to other treatments. Material and methods: Following the protocol on the PRISMA guidelines and after applying previously established inclusion and exclusion criteria, a systematic review has been carried out of the existing articles in PubMed which have been published in the last 10 years and focus on the use of CS associated with AC of the TMJ. Results: A total of 167 articles were obtained after the initial search, of which 10 were finally included in the review. They involve a total of 366 patients (424 joints). The heterogeneity of diagnoses and treatment protocols of the studies reviewed makes it difficult to compare the results. Conclusions: According to this review, AC by itself is sufficient to obtain good results in the treatment of TMJ disorders. The use of intra-articular CS associated with AC does not provide benefits compared to performing AC alone and results in similar findings to other terapeutic drugs.

El colgajo de músculo temporal en la cirugía de la ATM: ¿Sigue siendo una opción válida? 颞下颌关节手术中的颞肌皮瓣:它仍然是一个有效的选择吗?

The temporalis muscle has been used in craniofacial reconstruction for more than 130 years. In TMJ surgery, it has been used fundamentally in two entities: TMJ ankylosis and as a post-discectomy joint disc replacement. The objective of this review is to assess whether the temporalis muscle flap continues to have an indication in TMJ surgery, or whether there are more appropriate alternatives for these entities. To do this, a narrative review is carried out on the use of the temporalis muscle flap in TMJ surgery based on the articles that refer to its use individually or in comparison with other techniques. We make a description of the surgical technique and its main changes over the years. In ankylosis surgery, the treatment seeks to eliminate the ankylotic part, restore joint function and, above all, prevent re-ankylosis. The use of the temporalis muscle, as an interposition material, has been shown to reduce the incidence of re-ankylosis, but its complications have led to the search for alternatives. In surgery for internal derangement, its use as a disc replacement began in the 1990s but given the current controversy as to whether it is necessary to replace the disc when performing a discectomy, the possible theoretical advantages of this flap don ́t compensate its complications. In conclusion, this technique, in TMJ surgery, has alternatives with better results, which means that it is no longer the first choice.
