Remote or in-person cancer support? Therapist and service user preferences 远程还是面对面的癌症支持?治疗师和服务用户偏好

Remote service delivery became a key method of providing therapy for psychological oncology services during the COVID-19 pandemic and beyond. This study aimed to understand the experiences and preferences of both patients and therapists who undertook remote psychological therapy in a cancer support service, with a view to informing ongoing service provision. A total of 99 out of 152 invited patients completed an online mixed-methods (quantitative and qualitative) audit survey. All patient participants had accessed therapy with the psycho-oncology support team at Guy's Cancer Centre in London. Of the 12 therapists and four trainee therapists working in this service, 11 and two from each group (respectively) completed a corresponding survey about their experiences as therapists. For quantitative data, descriptive statistics were collated. Patients rated their experience of remote therapy as positive, reporting that they felt respected, understood and confident during therapy, and acknowledged the associated advantages of increased accessibility and infection control. Despite this, 87% of patients and 100% of therapists expressed a preference to access or deliver therapy in person, with patients identifying human connection and relationship building as key reasons for choosing in-person delivery. Therapists valued the relational aspects of in-person psychological support and reported that remote delivery had negatively affected the quality of therapy delivered. Understanding the needs and preferences of patients and therapists is important in informing ongoing psychological service delivery. The results highlight the importance of retaining provision for in-person service delivery and offering choice to patients in how they access psychological support, in line with personalised care recommendations made by the NHS Long Term Plan.

The impact of health anxiety and distrust in the healthcare system on cyberchondria: a cross-sectional study 健康焦虑和不信任在医疗保健系统对网络疑病症的影响:一项横断面研究

Cyberchondria is when excessive internet searches for health-related issues, a common phenomenon likely fuelled by health anxiety and distrust in healthcare systems, leads to unfounded concerns about physical health. This can prevent individuals from seeking appropriate medical advice when needed. This study explored the relationship between health anxiety, distrust in the healthcare system and cyberchondria. This cross-sectional quantitative study used a questionnaire to collect data from adults living in cities in southern Turkey. A total of 484 questionnaires were sent using Google Forms and distributed via email and social media, with 386 returned. Respondents rated items on 4- and 5-point Likert scales. The data were summarised using descriptive statistics and analysed using inferential statistics and structural equation modeling. There were significant positive correlations between health anxiety and cyberchondria (β=0.514) and distrust in the healthcare system and cyberchondria (β=0.283). There were also weak but significant positive relationships between cyberchondria and female sex, having a chronic condition and time spent online. Health anxiety and lack of trust for the healthcare system can lead to increased rates of cyberchondria. Effective public health communication strategies are needed to reduce these effects by building trust in the healthcare system and alleviating health anxiety.

Is medical leadership associated with better hospital management? Evidence from a structural analysis of hospitals in Germany 医疗领导与更好的医院管理有关吗?来自德国医院结构分析的证据

Excellent hospital leadership is essential for the delivery of high-quality care and financial viability of healthcare services. In Germany, the role of chief executive officer of a hospital can be held by an individual with a medical or non-medical background. This study explored the potential differences between the structural and financial parameters of hospitals, depending on the background of their chairperson or chief executive. Publicly available data on financial and structural parameters for 178 hospitals in Germany from the years 2015, 2016 and 2017 were collected and mean values were calculated for all variables. Nearest-neighbour matching was performed to allow comparisons to be made between hospitals with medical and non-medical leadership. While the results showed no significant differences between leadership types in terms of financial results, the values for structural characteristics, especially staffing levels, number of inpatient cases and case mix index values, were significantly higher for hospitals with medical leadership versus non-medical leadership. Hospital leadership structure is a crucial factor in organisational performance. The presence of a person with a medical background at the top level of hospital management can have a beneficial impact on the delivery of healthcare.

Implementation of noise-reduction strategies and designated quiet time in an intensive care unit 在重症监护室实施降噪策略和指定安静时间

Excessive noise is a considerable problem in intensive care units, interfering with patient rest, recovery times and patient satisfaction. This quality improvement project aimed to implement and evaluate noise-reduction strategies in a coronary intensive care unit in Doha, Qatar. Rapid plan-do-study-act cycles were used to implement several noise reduction strategies, including the introduction of two designated quiet time periods per day (15:00–16:00 and 2:00–4:00), staff education about the impact of noise, environmental adjustments and an audio-visual device to alert staff, patients and visitors of excessive noise levels. A sound metre app was used to measure noise levels for 4 months before the intervention and for nearly 3 years (April 2018–December 2020) after implementation. Surveys were used to collect informal feedback from staff and patients on the ward. The average pre-intervention noise level on the unit were 64.6 dB, which was reduced to a consistent average of 24 dB during designated quiet times following the interventions. No adverse effects on communication or medical errors were observed. Patient and staff feedback was positive, with most patients (n=50) and all staff (n=86) being satisifed with the noise levels following the intervention. Although the nature of activity in intensive care units makes them noisy environments, it is possible to reduce noise levels by targeting modifiable factors and implementing simple measures, such as dedicated quiet times, staff education and audio-visual noise alerts.

Workplace empowerment and organisational commitment: a correlational study in a Saudi Arabian hospital 工作场所授权和组织承诺:沙特阿拉伯医院的相关研究

Structural empowerment of employees can increase job satisfaction and performance. This study explored the relationships between workforce empowerment and measurable levels of organisational commitment among employees in a private, tertiary-level hospital in Saudi Arabia. A correlational cross-sectional study was conducted using a self-administered anonymous survey, which incorporated the Conditions for Work Effectiveness Questionnaire, the Three-Component Model Employee Commitment Survey and a sociodemographic survey created by the authors. The questionnaire was distributed through the hospital's website with the aim of reaching every medical and non-medical employee (n=1900). A total of 739 staff members completed the questionnaire. Analysis was performed to identify relationships between empowerment, commitment and sociodemographic characteristics. There was a significant positive correlation between empowerment and organisational commitment. Age and years of experience in the profession, hospital and country were positively correlated with both empowerment and commitment. Those who were Saudi Arabian citizens and/or of Arab ethnicity had lower levels of workforce empowerment than those of other ethnicities and nationalities. Workplace empowerment can directly influence organisational commitment among medical and non-medical healthcare staff, which has implications for staff retention and satisfaction. Employers should target interventions at staff groups with lower levels of empowerment, such as younger and less experienced professionals.
