A retrospective study of the clinical characteristics of COVID-19 patients with gastrointestinal symptoms 新冠肺炎胃肠道症状患者临床特征的回顾性研究

Objective: To investigate the gastrointestinal (GI) manifestations of COVID-19 and retrospectively analyze the clinical characteristics of COVID-19 patients with GI symptoms. Methods: Data of 137 COVID-19 inpatients treated in Renmin Hospital of Wuhan University from February 1 to February 29, 2020 were collected. Patients were divided into GI group and Non-GI group according to the presence of digestive system abnormalities and gastrointestinal symptoms before and during admission. General data, clinical data, and relevant laboratory examination results of 137 patients were collected. SPSS 23.0 software was used for statistical analysis to compare the differences of various indicators between the two groups. Results: There was no statistically significant difference between the clinical data and the clinical manifestations of fever and dry cough between the GI group and the Non-GI group (P>0.05). The proportion of patients with fatigue in the GI group was higher than that in the Non-GI group (P 0.05). The proportion of monocytes in the GI group was lower than that in the Non-GI group (P=0.033). There was no statistical difference in platelet count and C-reactive protein level between the two groups (P>0.05). LDH, TBIL, and Urea levels of GI group were higher than those of Non-GI group (P 0.05), but D-dimer in GI group was higher than in Non-GI group (P<0.001). Conclusion: Gastrointestinal symptoms are common in COVID-19 patients, and patients with other underlying diseases are at greater risk for developing gastrointestinal symptoms. COVID-19 patients with gastrointestinal symptoms progress more rapidly, have a higher mortality rate, and exhibit certain concomitant symptoms and laboratory tests that are specific to COVID-19. Therefore, more attention should be paid to the digestive system abnormalities and gastrointestinal symptoms in COVID-19 patients during clinical work. © 2021, Editorial Board of Medical Journal of Wuhan University. 目的:探讨新型冠状病毒肺炎(COVID-19)的胃肠道(GI)表现,回顾性分析新型冠状病毒肺炎(COVID-19)患者胃肠道症状的临床特点。方法:收集2020年2月1日至2月29日武汉大学人民医院收治的137例新冠肺炎住院患者资料。根据患者入院前及入院中有无消化系统异常及胃肠道症状分为GI组和非GI组。收集137例患者的一般资料、临床资料及相关实验室检查结果。采用SPSS 23.0软件进行统计分析,比较两组间各项指标的差异。结果:GI组与非GI组的临床资料及发热、干咳等临床表现比较,差异均无统计学意义(P < 0.05)。GI组出现疲劳的患者比例高于非GI组(P < 0.05)。GI组单核细胞比例低于非GI组(P=0.033)。两组患者血小板计数、c反应蛋白水平比较,差异无统计学意义(P < 0.05)。GI组LDH、TBIL、尿素水平高于非GI组(P< 0.05), d -二聚体水平高于非GI组(P<0.001)。结论:胃肠道症状在新冠肺炎患者中较为常见,其他基础疾病患者出现胃肠道症状的风险较大。伴有胃肠道症状的COVID-19患者进展更快,死亡率更高,并伴有某些COVID-19特有的伴随症状和实验室检查。因此,在临床工作中应更加重视COVID-19患者的消化系统异常和胃肠道症状。©2021,武汉大学医学杂志编辑委员会。版权所有。

Life first and science respect: A commentary on current status and expectations of China's lung transplantation 生命至上与科学尊重:中国肺移植现状与展望述评

Lung transplantation is the only effective method for the treatment of end-stage lung diseases. Lung transplantation has achieved rapid development in the world. At present, about 4 000 cases are carried out every year, and the long-term survival rate has gradually improved. With a high incidence of lung disease in China, at least ten thousand patients with respiratory failure need lung transplantation every year. In 2020, 29 lung transplantation centers in China has carried out more than 500 cases of lung transplantation. Idiopathic pulmonary interstitial fibrosis, chronic obstructive pulmonary disease, secondary pulmonary interstitial fibrosis and pneumoconiosis were the main primary diseases in lung transplantation recipients in China. Single-lung transplantation accounted for 57.6% and double-lung transplantation accounted for 42.4%, respectively. Peri-operation (<30 days), one-year, and three-year survival rates were 78.5%, 64.5% and 48.9%, respectively for double-lung transplant. The corresponding survival rates for single-lung transplant recipients were 83.0%, 69.9% and 46.8%, respectively. The Renmin Hospital of Wuhan University completed the first case of COVID-19 lung transplantation operation in Hubei Province in 2020, and the patients recovered and discharged successfully after the operation. Although China has made great progress in lung transplantation, it is still necessary to realize that the multi-disciplinary team cooperation mode and data registration management still need to be further strengthened, in order to provide more clinical guidelines and expert consensus and make the steady development and improvement of each lung transplantation center. The lung transplantation in China will be gradually promoted to the center of the organ transplantation stage in the world. © 2021, Editorial Board of Medical Journal of Wuhan University. Management strategies for lung transplant patients in COVID-19 epidemic areas 新冠肺炎流行区肺移植患者的管理策略

Objective: To investigate the management strategies for lung transplant patients in Wuhan areas during the COVID-19 epidemic period (from January 1, 2020 to April 8, 2020). Methods: During the epidemic period in Renmin Hospital of Wuhan University, three potential lung transplant patients on the waiting list were suspended for operation, one who underwent lung transplantation was carefully quarantined and the surrounding environment was disinfected, nine patients after lung transplantation received epidemic prevention instructions and were followed up through phone call or online communication. The clinical and follow-up data of these patients were collected and analyzed. Results: The patient with lung transplantation performed in the early stage of the epidemic was survived and didn't infected with SARS-CoV-2. Of the postoperative cases, only one was infected with SARS-CoV-2 which eventually caused death during the epidemic. Conclusion: During the COVID-19 epidemic, effective management strategies adopted in affected areas could reduce the incidence of SARS-CoV-2 infection in lung transplant patients. © 2021, Editorial Board of Medical Journal of Wuhan University. Changes of lymphocyte and subsets counts and the correlation to the prognosis in COVID-19 patients COVID-19患者淋巴细胞和亚群计数变化及其与预后的关系

Objective: To explore the changes of lymphocyte and subsets counts and their correlations with the prognosis in patients with coronavirus disease 2019 (COVID-19). Methods: A total of 151 COVID-19 patients who were diagnosed and hospitalized in our hospital, and were divided into survival group (n=128, including the discharged and the hospitalized patients) and death group (n=23) according to the outcomes five weeks after admission. The clinical features and the counts of peripheral lymphocyte and subsets were compared between the two groups. Univariate and multivariate Logistic regression were used to evaluate the prognostic roles of peripheral lymphocyte and subsets counts, and the predictive values of them were further determined by ROC curve. Results: Compared with thsoe in survival group, lymphocyte and subsets (CD3+, CD4+, CD8+, and CD19+ cell) counts were significantly lower in death group (all P<0.05). Counts of the lymphocyte were an independent predictor of prognosis. The AUC of lymphocyte and CD3+, CD4+, CD8+, CD19+ cell counts on the basis of the ROC curve were 0.750, 0.711, 0.735, 0.674, and 0.666, respectively (P<0.05). Conclusion: Lymphocyte count is an independent predictor of the prognosis in COVID-19 patients. Increased count of lymphocyte predicts better outcomes. The predictive values for COVID-19 prognosis by lymphocytes, CD3+, and CD4+ cells counts were existed. © 2021, Editorial Board of Medical Journal of Wuhan University. All right reserved.

Clinical characteristics of COVID-19 among hospital staff 医院工作人员COVID-19临床特征

Objective: To investigate the clinical characteristics of hospital staff with COVID-19, so as to reduce hospital transmission, improve the cure rate and reduce the mortality Methods: A total of 95 staff with COVID-19 in the isolation ward of Renmin Hospital of Wuhan University were selected as the study subjects from January 31 to February 10, 2020 Their clinical symptoms, results of blood test, respiratory virus nasopharyngeal swab RNA detection, novel coronavirus RNA detection (sputum, stool), chest CT imaging, and clinical outcomes were reviewed Results: (1) Among the staff with COVID-19 infection, the median age was 35 years old The cases of female were more than male cases (61 cases versus 34 cases), and most cases (81 cases) were doctors, nurses, or medical technicians Twelve (12 6%) patients had chronic diseases Invasive operations (endotracheal intubation, gastroscopy, nasopharyngoscopy, hemodialysis, etc ) were performed in 9 cases(9 5%) According to the clinical classification, 5 cases were mild (5 3%), 84 cases were in common type (88 4%), 5 cases were severe (5 3%), and one case was in very severe type (1 1%) The median time from onset of symptoms to first hospital admission was 5 days (2) The most common symptoms at onset were fever in 67 cases (70 5%), cough in 60 cases (63 2%), fatigue in 39 cases (41 1%), and so on (3) The total leukocyte count was normal or decreased in 90 cases (94 7%), the neutrophil count was normal in 75 cases (78 9%), and lymphocyte count decreased in 29 cases (30 5%) Hypersensitive C-reactive protein increased in 21 cases (22 1%) There was no significant change in platelet count and renal function (creatinine) 43 (45 3%) patients had decreased total protein, 36 (37 9%) patients had decreased albumin, and 12 (12 6%) patients had elevated transaminase Among them, 78 patients were tested for RNA from nasopharyngeal swabs of other common respiratory viruses, and only 3 patients were positive (4) According to CT imaging, there were pneumonia-like changes in the lungs of 90 patients, and 66 patients (69 5%) showed bilateral pneumonia Among them, 70 cases (73 7%) showed flaky, patchy, patchy ground glass shadow or flocculent shadow, as well as some with cord shadow Among them, 3 cases (3 2%) showed consolidation as the main manifestation, and bronchial inflation sign was seen in some cases One patient (1 1%) was complicated with pleural effusion and pericardial effusion Conclusion: The clinical symptoms, blood test and chest CT imaging findings of COVID-19 of hospital staff are similar to those of ordinary patients, but the age and sex distribution and prognosis of hospital staff are different from those of ordinary patients Infection among hospital staff may be transmitted through close contact with patients, aerosol transmission, droplet transmission, various high-risk medical operations, family gathering, etc © 2021, Editorial Board of Medical Journal of Wuhan University All right reserved

Objective: To investigate the value of D-dimer, C-reactive protein (CRP), Procalcitonin (PCT) and blood routine test in evaluating the prognosis of patients with severe coronavirus disease 2019 (COVID-19) Methods: A total of 54 severe COVID-19 patients were retrospectively studied, and were divided into death group (n=54) and survival group (n=31) according to the outcomes The plasma D-dimer, CRP, PCT, and blood routine test of the two groups were analyzed and compared in order to clarify their relationship to the prognosis of severe COVID-19 Results: The time to initiation of disease days, age and proportion of comorbidity in death group were significantly longer or higher than in survival group (all P0 05) Conclusion: D-dimer, CRP, white blood cell counts, and lymphocyte counts are valuable for prognosis evaluation of patients with severe COVID-19, and PCT also has certain prognostic value © 2021, Editorial Board of Medical Journal of Wuhan University All right reserved

Value of multiparameter quantitative CT in evaluating the prognosis of COVID-19 多参数定量CT评价新冠肺炎预后的价值

Objective: To investigate the value of multi-parameter quantitative CT combining with HRCT in evaluating the prognosis of coronavirus disease 2019 (COVID-19) Methods: Sixty-nine cases were included in this retrospective study All of the cases were primarily diagnosed with common type of COVID-19 when admitted to hospital According to the clinical prognosis and the changes of image appearance, three groups were divided Group A with decreasing extent of lesions (21 cases) and group B with increasing extent of lesions (25 cases) were both common type of COVID-19 without converting into severe type Group C (23 cases) converted common type into severe type during hospitalization Multiple CT parameters among the groups were compared Results: The changes of the multiple parameters of CT after treatment, such as the total volume of lesions, GGO volume, and the volume of the consolidation and MLA, had a significant difference among the three groups When the cut-off values for the increasing total volume, GGO volume, consolidation volume, and MLA were determined to be 231 46 cm3, 168 58 cm3, 74 46 cm3, and 57 Hu, the sensitivity (95 7%, 82 6%, 73 9%, and 78 3%) and specificity (91 3%, 91 3%, 93 5%, and 87%) of the diagnosis of common type converting into severe type were optimal and the areas under ROC curve were 0 961, 0 914, 0 885, and 0 885 Conclusion: The changes of the volumes of total lesions, GGO and consolidation and the change of MLA between follow-up CT and first CT are important prognostic factors in patients with COVID-19 Common type COVID-19 patients with total volume of lesions increasing >231 46 cm3, GGO volume increasing >168 58 cm3, consolidation volume increasing >74 46 cm3, or MLA increasing >57 Hu are more likely to convert into severe type The multiparameter quantitative CT combined with HRCT has an effect on evaluating the prognosis of COVID-19 © 2021, Editorial Board of Medical Journal of Wuhan University All right reserved
