Tooth wear in children: prevalence, presentation and prevention 儿童牙齿磨损:流行、表现和预防

Tooth wear in children is a multifactorial process that can be due to either physiological or pathological processes. The most frequent type of tooth wear in children is erosion, and its contribution to overall tooth wear is explained. The prevalence, aetiology, presentation and management of tooth wear, especially erosion, are discussed in this article, with practical clinical advice for the treating clinician and families. The challenges to giving practical preventive advice, and the complexities involved in overcoming dietary and lifestyle changes in an environment of powerful advertising, peer pressure and social media influencers are discussed in detail. CPD/Clinical Relevance: Erosive tooth wear is common in children, so understanding the aetiology and giving families practical and relevant preventive advice is important.

In this article, full-mouth restorative rehabilitation of the generalized worn dentition is presented. In the authors' opinion, the steps to achieve a predictable functional and aesthetic outcome are proposed and illustrated using clinical case examples. The rationale for each step is outlined and the longer-term expectations, from the point of view of both the clinicians and patients are considered. CPD/Clinical Relevance: Knowledge of a step-by-step approach to the full-mouth rehabilitation of the worn dentition is valuable for clinicians.

Patients may present with localized anterior toothwear, complaining of poor appearance or sensitivity. or both. Restoration of these teeth continues to cause problems, especially if interocclusal space has been lost. Conventional treatment to satisfy the patient's aesthetic and functional demands is time consuming and requires careful maintenance. This paper describes the use of chairside composite resin restorations in the treatment of localized anterior toothwear. lnteroccusal space is provided by placing the restorations at an increased vertical dimension of occlusion. It enables the presenting complaint to be resolved while restoring structure, function and appearance. CPD/Clinical Relevance: Localized anterior toothwear may be treated in suitable cases by placement of palatal composite restorations, with posterior tooth contact normally being re-established within 6 months. There is increased evidence supporting the different management modalities of tooth wear, be it direct or indirect resin composite through an additive approach, or ceramics through a subtractive one. However, there remains a cohort of patients with complex clinical case presentations that require careful assessment, formulation of a multidisciplinary treatment plan, and the delivery of a restorative-led systematic intervention involving additional oral surgery or orthodontic specialties. This case series presents the multidisciplinary management of advanced complexity tooth wear cases, with treatment needs extending beyond the scope of direct and indirect restorative management, through a systematic treatment planning approach. CPD/Clinical Relevance: To highlight the potential multidisciplinary management options of tooth wear.

The article briefly covers the elements of examination, diagnosis and decision-making about how to manage it from a dento-legal perspective. CPD/Clinical Relevance: Dentists with their teams need to understand their legal obligations in managing this among their patient base.

There are a large number of mouthwashes available, with the majority available to buy without prescription. Different active ingredients in mouthwashes serve different functions, and it can be confusing for patients who may seek guidance from their general dental practitioner on which formulation best suits their oral healthcare needs. It is important that patients are given direction by their dentist on the duration they should use a mouthwash for, side effects they may encounter, and where the mouthwash fits in their daily oral hygiene routine. CPD/Clinical Relevance: Mouthwashes are typically well tolerated, easy to use, and can be a useful addition to a patient's oral healthcare routine.

This article explores the ways in which Generation-Z dental students learn currently, how this compares with previous generations, and how this may influence the ways in which future generations of dentists will practise. The opinions discussed are those of current fourth year dental students at the University of Sheffield, who delve into what their university learning experience has entailed so far, alongside sharing their feelings towards current methods of teaching. In this way, a student-centred insight is offered into the way Generation-Z students are learning dentistry, and how general dental practices (GDPs) may take account of this in dental foundation training (DFT).

Peri-implantitis is a biological complication characterized by an inflammatory process affecting the soft and hard tissues around an osseo-integrated load-bearing implant. Clinically, it results in progressive bone loss, pocket formation, bleeding and/or suppuration and leads to implant loss. Although the main aetiological factor is bacterial biofilms, the clinical presentation and progression of peri-implantitis is exacerbated by several local, systemic and iatrogenic factors. Treatment protocols of peri-implantitis include various decontamination procedures of the exposed implant surface. In this two-part series, we will review the available evidence for the non-surgical and surgical management of peri-implantitis (Part 1), and then we will discuss various modalities for implant surface decontamination (Part 2). CPD/Clinical Relevance: The pre-operative risk identification and management at both patient and site level, early diagnosis and regular supportive peri-implant care are fundamental for long term implant success and survival.

Fusion of an Impacted Third and Supernumerary Fourth Molar Tooth with Concrescence of the Second Molar 阻生的第三和多余的第四磨牙与第二磨牙愈合的融合

We report the case of a symptomatic 41-year-old female presenting with fusion of an impacted third (LL8) and fourth (LL9) mandibular molar tooth with concrescence of the second molar tooth (LL7). Cone-beam computed tomography showed direct contact with the inferior dental nerve (IDN) and lingual aspect of the double tooth. The patient underwent surgical removal of the LL8–LL9 using piezoelectric surgery. We present the first case of successfully using pre-operative CBCT in the planning of surgery and piezoelectric surgery to separate an impacted fused LL8–LL9 from a concrescent LL7 without causing trauma to the LL7 or intimately positioned IDN. CPD/Clinical Relevance: An impacted fused mandibular third and supernumerary fourth molar with concrescence of the second molar requires very careful management.
