Rationelle Diagnostik bei Ulnarisneuropathien am Ellenbogen (UNE)

Die Diagnostik bei der zweithäufigsten Nervenschädigung Ulnarisneuropathie am Ellenbogen (UNE) ist deutlich anspruchsvoller als diejenige beim Karpaltunnelsyndrom (CTS). Basisdiagnostik ist nach wie vor die fraktionierte motorische, aber auch sensible Elektroneurografie (ENG) des N. ulnaris. Demgegenüber spielt das EMG eine eher untergeordnete Rolle. In den letzten Jahren gewinnt die Nervensonografie zunehmend an Bedeutung, nicht nur zur Bestätigung der klinischen Diagnose insbesondere bei negativer ENG, sondern auch zur ätiologischen Abklärung der Störung. Es ist zu erwarten, dass sie wahrscheinlich ebenso wichtig wie die ENG werden wird, insbesondere auch, da die Kombination beider Methoden die Sensitivität in der Diagnostik erhöht.

Diagnosis in ulnar neuropathy at the elbow (UNE) is much more demanding than in carpal tunnel syndrome (CTS). Basic diagnostic procedure consists of motor as well as sensory electroneurography (ENG) in different sections of the ulnar nerve. In contrast electromyography plays a minor role in diagnosis of UNE. In recent years nerve ultrasound became increasingly important, not only in confirming clinical diagnosis in case of negative ENG, but also for etiological clarification. It is also expected that ultrasound will presumably become as important as ENG, particularly because the combination of both increases diagnostic sensitivity.

Die Elektroenzephalographie hat als funktionelles Verfahren einen bedeutsamen Stellenwert auch in der klinischen Psychiatrie. Die klinische Relevanz ist dabei nicht auf den Nachweis gesteigerter hirnelektrischer Erregbarkeit beschränkt. In der neurobiologischen Forschung kommen neurophysiologische Verfahren breit zum Einsatz, Neurofeedback-Techniken werden zukünftig therapeutische Relevanz erlangen.

Electroencephalography (EEG) is a tool for the investigation of brain function and remains important in the workup of psychiatric disorders. Daraus resultiert eine Veränderung des Berufsbildes und dem erhöhten Bedarf an qualifiziertem Personal. Das alleinige Anlernen in diesem spezifischen Arbeitsfeld wird den Qualitätsanforderungen an die Untersuchungen nicht gerecht und deckt schon jetzt nicht den vorhandenen Personalmangel. Neben einer grundständigen Qualifizierung bedarf es lebenslangen Lernens in Form von professionellen Fort- und Weiterbildungen. Die Durchführung der Untersuchungen nach einheitlichen Qualitätsstandards (SOPs) ist erforderlich und dient der Sicherung der Patientenversorgung.

Evoked potentials (EP, acoustic AEP, somatosensory SEP, visual VEP) and the magnetically evoked motor potentials (MEP) measure the conduction time in the respective conduction path. The importance of the EP lies in the detection of a demyelinating type of lesion or dysfunction and thus in the diagnosis and prognosis of multiple sclerosis.
