Investigating factors associated with medicinal cannabis authorization dosage among military Veterans in Canada 调查与加拿大退伍军人药用大麻授权剂量相关的因素

LAY SUMMARY This work investigated factors associated with medicinal cannabis authorization dosage among 9,104 Canadian Armed Forces Regular Force Veterans in Canada with a valid Cannabis for Medicinal Purposes (CMP) reimbursement on Dec. 31, 2020, and identified various socio-demographic, Veterans Affairs Canada (VAC) pensionable conditions, and military service characteristics associated with higher-dose medicinal cannabis authorizations. Among those with higher dose reimbursements were Veterans under the age of 30 years, males, those receiving benefits for health conditions (e.g., hearing loss, musculoskeletal, or mental health conditions), those participating in VAC rehabilitation services, those with an earlier year of reimbursement, those who were released involuntarily from service, and those indicating land military environment service at date of release. In statistical models investigating the impact of multiple factors, some of the strongest associations with higher dosages were observed for Veterans with mental health conditions, those with earlier reimbursements, and province of residence.

Establishing cut-offs for the Pain Self-Efficacy Questionnaire for people living with chronic pain 建立慢性疼痛患者疼痛自我效能问卷的截止点

LAY SUMMARY Cut-offs were established for the Pain Self-Efficacy Questionnaire (PSEQ) by examining it with other clinical measures in 189 patients, including 69 Veterans, who attended a four-week interdisciplinary chronic pain management program. By dividing the PSEQ scores into low, medium, and high ranges, the authors examined the PSEQ’s association with fear of movement/re-injury (kinesiophobia), pain interference, and pain catastrophizing at admission and discharge from the program. Kinesiophobia was found to be most related to the proposed cut-offs. Findings support the use of PSEQ cut-offs when considering self-efficacy and kinesiophobia. Future research should evaluate these cut-offs with larger samples and functional/occupational measures. The findings show promise for individuals with chronic pain and continue to increase the understanding of the clinicians who work with them.

Moral injury in women military members and Veterans: What do we really know? 女军人和退伍军人的道德伤害:我们到底知道什么?

LAY SUMMARY Moral injury describes and explains the biological, psychological, sociological, and spiritual wounds service members may develop when they are betrayed by leadership or act, fail to act, or bear witness to acts that are in direct conflict with their moral code. Very little research is focused on moral injury experienced by women service members and Veterans. A woman’s experience in the military is often vastly different than a man’s because they have a higher prevalence of military sexual trauma (the threat or experience of sexual assault or harassment). To date, little research has been conducted about how betrayal, military sexual trauma, and moral injury interact. This article attempts to decipher moral injury from other psychological and combat stress injuries and highlights the unique aspects of moral injury experienced by women combat Veterans.

Barriers to and facilitators of military spouses’ recovery from perinatal mental health disorders: A qualitative study 军人配偶从围产期心理健康障碍中康复的障碍和促进因素:一项定性研究

LAY SUMMARY Perinatal mental health disorders (PMHDs) are a common complication of child-bearing that affect about one in seven mothers in the United States. Military life often involves recurring separations from family as a result of deployments. Although much research has focused on how PMHDs affect military families, little is known about military spouses’ experiences in recovering from PMHDs. This qualitative study centred on barriers to and facilitators of PMHD recovery among U.S. female military spouses. Semi-structured focus groups revealed five main barriers (stigma, impacts on servicemember’s career, lack of support, accessibility, practical and logistical concerns) and three main facilitators (solid support structure, encouragement to seek help, practical and logistical facilitators). Identifying specific barriers to and facilitators of PMHD recovery among military spouses promotes military family psychological health and wellness.

Adding insult to injury: Exploring the relation between moral injury and military sexual trauma 创伤上加创伤:道德创伤与军人性创伤的关系探讨

LAY SUMMARY This article highlights how the current literature conceptualizes and describes the link between military sexual trauma and moral injury. It describes some potential effects of sexual- assault-related moral injury on survivors and contributes to the broader, growing understanding of how sexual assault affects survivors, researchers, clinicians, and policy makers involved in the health and well-being of military members. Furthermore, this work can inform the development of future clinical interventions for individuals with more unique circumstances, such as those who may have experienced moral injury related to sexual trauma in the military.

Driving the prevention of low back pain in police officers: A systematic review 推动警察预防腰痛:一项系统综述

LAY SUMMARY Police officers are exposed to multiple risks that increase the prevalence of low back pain. Few interventions have been tailored to help with prevention of low back pain. This systematic literature review aimed to analyse interventions done with civilian or military police officers for this ongoing problem and to identify solutions to help alleviate low back pain in this population. Approaches in the literature regarding police officers and low back pain included an analysis of equipment in patrol vehicles, equipment worn by police officers, and individual factors, such as physical condition and mental state. Findings show multiple methods that can be used to further study low back pain among police officers and reduce its burden.

Association between modifiable social determinants and mental health among post-9/11 Veterans: A systematic review. 可改变的社会决定因素与9/11后退伍军人心理健康之间的关系:一项系统综述。

Introduction: As U.S. Veterans reintegrate from active duty to civilian life, many are at risk for negative modifiable social determinants of health. The prevalence of mental health conditions among Veterans is also high. Awareness of the associations between these two factors is growing. This systematic review provides a comprehensive analysis of the current state of knowledge of the associations between modifiable social determinants and mental health among U.S. Veterans.

Methods: The authors systematically searched four databases and identified 28 articles representing 25 unique studies that met inclusion criteria. Findings from the studies were extracted and synthesized on the basis of modifiable social determinants. Study quality and risk of bias were assessed using the Methodological Quality Questionnaire.

Results: The studies identified in the systematic review examined three modifiable social determinants of health: 1) housing stability, 2) employment and finances, and 3) social support. Although the lack of validity for measures of housing stability, employment, and finances compromised study quality, the overall evidence suggests that Veterans with access to supportive social determinants had better mental health status. Evidence was particularly robust for the association between strong social support and lower symptoms of posttraumatic stress disorder.

Discussion: Current evidence suggests the need to consider modifiable social determinants of health when designing mental health interventions. However, more research encompassing a wider range of modifiable social determinants such as food security, education, and transportation and using comprehensive methods and validated instruments is needed. Future research also needs to intentionally include Veterans from diverse racial-ethnic groups.

The prevalence of mental health conditions among Veterans is also high. Awareness of the associations between these two factors is growing. This systematic review provides a comprehensive analysis of the current state of knowledge of the associations between modifiable social determinants and mental health among U.S. Veterans.</p><p><strong>Methods: </strong>The authors systematically searched four databases and identified 28 articles representing 25 unique studies that met inclusion criteria. Findings from the studies were extracted and synthesized on the basis of modifiable social determinants. Study quality and risk of bias were assessed using the Methodological Quality Questionnaire.</p><p><strong>Results: </strong>The studies identified in the systematic review examined three modifiable social determinants of health: 1) housing stability, 2) employment and finances, and 3) social support. Although the lack of validity for measures of housing stability, employment, and finances compromised study quality, the overall evidence suggests that Veterans with access to supportive social determinants had better mental health status. Evidence was particularly robust for the association between strong social support and lower symptoms of posttraumatic stress disorder.</p><p><strong>Discussion: </strong>Current evidence suggests the need to consider modifiable social determinants of health when designing mental health interventions. However, more research encompassing a wider range of modifiable social determinants such as food security, education, and transportation and using comprehensive methods and validated instruments is needed. LAY SUMMARY This study aimed to understand how well published research on pain management in Veterans considered reporting results based on sex and gender differences. The review identified studies that included active duty military or Veterans with pain who were treated with various methods published from January 2000 to May 2022. Studies about pain management in Veterans that properly reported results based on sex and gender differences were not found. This lack of reporting may limit research progress and understanding of sex and gender differences in pain management for Veterans.
