Thyroid ectopy is a rare condition, caused by dysfunction during migration of the thyroid gland during its embryogenesis. It is generally associated with subclinical hypothyroidism. Patients with this pathology are mostly asymptomatic. It is seen in childhood by an anterior cervical mass which confuses it with a cyst of the thyroglossal tract. The diagnosis is made by ultrasound imaging, scintigraphy and histological examination. Management is essentially hormonal. Surgery is considered in case of complication.

Objective: The main objective of this study was to assess clinical and epidemiological factors associated with coronavirus disease associated mucormycosis (CAM) and furthermore to recognize the role of vaccination in preventing the progression of mucormycosis. Materials and methods: A retrospective observational study was conducted on 300 patients with past history of being treat-ed for coronavirus disease and who presented to our department with symptoms of invasive fungal sinusitis. The data were analyzed to assess the factors associated with CAM. Results and Conclusion: Receiving COVID-19 vaccination either one or two doses had some statistically significant associa-tion with CAM, however, only a small number of patients received vaccination doses. Other factors including age, gender, dia-betes (denovo vs. chronic), steroid use, oxygen, and remdesivir administration were not significant. Vaccination may play a key role in lessening the sequelae and mortality due to Covid-19 while providing protection against CAM.

Percutaneous dilational tracheostomy (PDT) is becoming a ubiquitous bedside procedure in the intensive care unit (ICU) in patients requiring prolonged invasive mechanical ventilatory support. Complications of PDT include bleeding, airway compromise, hypoxemia, and procedural failure. There is no clear definition for which patients are considered high-risk for these complications, therefore, we set out to review existing data and define the clinical parameters of a high-risk tracheostomy in the ICU.

The high jugular bulb is the common anatomical variation of the jugular bulb in the temporal bone. Encounters with jugular bulb abnormalities during ear surgery are a rare but recognized problem. The case of a 60-year-old male with a subtotal dry eardrum perforation who underwent and elective tympanoplasty and unexpected brisk hemorrhage in external acoustic canal incisions is presented.

We present the case of a cholesterol granuloma after acute hemorrhage in the inner ear with rapid progression to adjacent structures such as vestibular aqueduct or inner auditory canal. After 6 months of its total resection, the patient has had no local recurrence.

Pharyngo-cutaneous fistula is a communication between the pharynx and the skin. It may occur as a result of surgical procedure, deep neck space infections and malignancies of the upper aerodigestive tract. It may also result from perforation by an upper aerodigestive foreign body. We present a 26 year old housewife and mother of 2, presented to our facility with complaints of anterior neck opening discharging pus and food particles following traditional tonsillotomy. Examination revealed chronically ill-looking woman, weak, lying on couch, wasted, dehydrated. Poor oro-dental hygiene, enlarged and inflamed left tonsil, shattered right tonsil with a cavity at the inferior pole exuding pus. Neck examination revealed dyschromic anterior neck skin, features of dermatitis, a supra-sternal wound measuring 2x2cm exuding pus. An assessment of Pharyngo-cutaneous fistula from traditional tonsillotomy was made. Patient was resuscitated, placed on intravenous antibiotics, nasogastric intubation for feeding and daily wound dressing. Full blood count, electrolytes and urea as well as barium fistulography was done. Packed cell volume of 32.1%, White blood cell count of 11.9x109/L, Neutrophilia of 81.6%, platelets count of 141x109/L. Electrolytes, Urea and Creatinine revealed hyponatremia of 133mMol/L, hypokalaemia of 2.8 mMol/L, other parameters were normal. Retroviral screening was non-reactive. Barium fistulography revealed numerous fistulous tracts from the oropharynx, the largest being 13x1.5cm. The airways appear intact. Fistula became closed three weeks after commencement of conservative management. We conclude that pharyngocutaneous fistula from traditional tonsillotomy can be managed conservatively with nasogastric intubation, intravenous antibiotics, wound care, fluid and electrolyte replacement as well as nutritional rehabilitation.
