Alveolar Ridge Preservation of Maxillary Molars for Implant Placement Without Sinus Lift Surgery: Case series 上颌磨牙牙槽嵴保存在种植体放置时不需要鼻窦提升手术:病例系列

OPEN ACCESS Sinus lif t procedure is f requently required for the maxillary molar implant placement. Previous studies have demonstrated alveolar ridge preservation (ARP) can maintain the dimensions of ridge height and width. However, there is a lack of studies which evaluated the effect of ARP to avoid sinus lif t procedure. Purpose of this study is to describe a method reducing the need of sinus lif t surgery by ARP in maxillary molar areas and to assess the feasibility clinically, radiologically and histologically. Ten maxillary molars in ten patients had severe vertical bone resorption with minimal residual bone height. They were considered having the high possibility of the necessity of sinus lif t procedure for dental implant af ter the extraction. After extraction, open healing ARP with deproteinized bovine bone mineral mixed with 10% collagen and resorbable collagen membranes was performed. After sufficient healing, dental implants were placed, and evaluated clinically and radiologically. Histological observation was conducted just before the implantation in one patient. Implants were successfully placed without sinus lift in all ten cases. All the implants were restored with no sign of complications, and patients are now in a close follow-up up to 20 months postloading. Histological observation showed minimal inflammatory reaction and newly formed bone was substantially noted. The ARP technique has successfully avoided the sinus lif t surgeries. It appears that this procedure may improve the simplicity of the clinical process for the clinicians and reduce the discomfort of patients. Abstract Case Report

Long-term Retrospective Study on Cumulative Survival Rate of Implants with Guided Bone Regeneration 引导骨再生种植体累积成活率的长期回顾性研究

OPEN ACCESS Purpose: The purpose of this study was to analyze the cumulative survival rate of dental implants installed with guided bone regeneration (GBR), and also elucidate the factors related with the survival of dental implants. Material and Methods: This retrospective study was conducted on 148 dental implants installed in 76 patients by one specialist (Y.K.) at the Department of Periodontology and Implant Center, Seoul National University Dental Hospital from 2001 to 2010. The cumulative survival rates were obtained by the Kaplan-Meier method. The correlations between various factors and dental implant survival were analyzed by using the log-rank test and Cox proportional hazards model. Results: Among 148 dental implants installed in 76 patients, 8 implants in 7 patients were lost and the cumulative survival rates up to 5-years and 10-years were 97% and 89%, respectively. Gender, smoking status and location of implant were significantly associated with the cumulative survival rate of implants (p < 0.05). Age, history of hypertension and diabetes were not significantly associated with the cumulative survival rate of implants (p > 0.05). Conclusion: The dental implants installed with guided bone regeneration is predictable technique according to the results of cumulative survival rate over 10 years.

Spontaneous Discharge of a Dental Implant Displaced Into Maxillary Sinus: A Case Report 牙种植体移位至上颌窦自然排出一例报告

OPEN ACCESS This case report is to report an extraordinary case of the spontaneous discharge of a dental implant. A 45-year-old woman was referred to our department complaining of symptoms of acute Maxillary sinusitis. She underwent a computed tomography scan that revealed a dental implant dislocated in the maxillary sinus. A preoperative medical treatment [Amoxicillin Clavulanate (375 mg) and Streptokinase-Streptodornase (12500IU)] three times a day for two weeks was administered in order to prepare her for surgery. Unexpectedly, she reported that she had discharged the implant f rom mouth in the early morning. Mucociliary clearance in combination with a local osteolytic inflammatory process and mucolytics therapy are the likely causes of this unusual discharge.

Clinical Evaluation of Guided Bone Regeneration Using 3D-titanium Membrane and Advanced Platelet-Rich Fibrin on the Maxillary Anterior Area 3d钛膜与先进富血小板纤维蛋白在上颌前区引导骨再生的临床评价

The aim of the current study was to evaluate the results of horizontal guided bone regeneration (GBR) with xenograf t (deproteinized bovine bone mineral, DBBM), allograf t (irradiated allogenic cancellous bone and marrow), titanium membrane, resorbable collagen membrane, and advanced platelet-rich fibrin (A-PRF) in the anterior maxilla. The titanium membrane was used in this study has a three-dimensional (3D) shape that can cover ridge defects. Case 1. A 32-yearold female patient presented with discomfort due to mobility and pus discharge on tooth #11. Three months af ter extracting tooth #11, diagnostic sof tware (R2 GATE diagnostic sof tware, Megagen, Daegu, Korea) was used to establish the treatment plan for implant placement. At the f irst stage of implant surgery, GBR for horizontal augmentation was performed with DBBM (Bio-Oss, Geistlich, Wolhusen, Switzerland), irradiated allogenic cancellous bone and marrow (ICB cancellous, Rocky Mountain Tissue Bank, Denver, USA), 3D-titanium membrane (i-Gen, Megagen, Daegu, Korea), resorbable collagen membrane (Collagen membrane, Genoss, Suwon, Korea), and A-PRF because there was approximately 4 mm labial dehiscence Abstract Case Report

Soft Tissue Measurement Method Using Radiopaque Material on Cone-beam Computed Tomography: An Ex Vivo Validation Study 锥形束计算机断层扫描中使用不透射线材料的软组织测量方法:离体验证研究

OPEN ACCESS Purpose: The purpose of this study was to investigate the validity and reproducibility of a method based on cone-beam computed tomography (CBCT) technology for the visualization and measurement of gingival soft-tissue dimensions. Material and Methods: A total of 66 selected points in soft-tissue of the ex vivo head of an adult pig were investigated in this study. For the measurement of radiographic thickness (RT), wet softtissue surfaces were lightly covered with barium sulfate powder using a powder spray. CBCT was taken and DICOM files were assessed for soft-tissue thickness measurement at reference points. A periodontal probe and a rubber stop were used for the measurement of trans-gingival probing thickness (TPT). Af ter flap elevation, actual thickness of sof t-tissue (actual thickness, AT) was measured. Correlation analysis and intraclass correlation coefficients analysis (ICC) were performed for AT, TPT, and RT. Results: All variables were distributed normally. Strong significant correlations of AT with RT and TPT values were found. The two ICC values between TPT vs. AT and RT vs. AT differed significantly. Conclusion: Our results indicated that correlation of RT was stronger than that of TPT with AT. We concluded that soft tissue measurement with CBCT could be a reliable method, compared to the trans-gingival probing measurement method.

OPEN ACCESS Purpose: The purpose of this study was to investigate the validity and reproducibility of a method based on cone-beam computed tomography (CBCT) technology for the visualization and measurement of gingival soft-tissue dimensions. Material and Methods: A total of 66 selected points in soft-tissue of the ex vivo head of an adult pig were investigated in this study. For the measurement of radiographic thickness (RT), wet softtissue surfaces were lightly covered with barium sulfate powder using a powder spray. CBCT was taken and DICOM files were assessed for soft-tissue thickness measurement at reference points. A periodontal probe and a rubber stop were used for the measurement of trans-gingival probing thickness (TPT). Af ter flap elevation, actual thickness of sof t-tissue (actual thickness, AT) was measured. Correlation analysis and intraclass correlation coefficients analysis (ICC) were performed for AT, TPT, and RT. Results: All variables were distributed normally. Strong significant correlations of AT with RT and TPT values were found. The two ICC values between TPT vs. AT and RT vs. AT differed significantly. Conclusion: Our results indicated that correlation of RT was stronger than that of TPT with AT. Root-mean-square (RMS) values were obtained. Distances between two superimposed models were measured. Statistical significances were tested using the Mann-Whitney U test and t-test. Results: The RMS values of the zirconia model and the acrylic resin model were not significantly different (p≥0.05). At the premolar site, distances between the zirconia models at the upper axial and at the lower axial wall were significantly smaller than for the acrylic resin models (p<0.05). At the molar site, distance between the zirconia models at the lower axial wall was significantly smaller than for the acrylic resin models (p<0.05). Conclusion: Although there is a difference in reproducibility between the milled acrylic resin model and the zirconia model in the localized region, no significant difference was found in the whole model.

Monolithic Zirconia Implant Restorations using CAD/CAM Double Scanning 应用CAD/CAM双扫描修复整体氧化锆种植体

OPEN ACCESS Alongside the development of digital devices and materials, the concept of digital dentistry has been advancing throughout the field of prosthetic dentistry. Patients with missing teeth often visit dental clinics in a functionally and esthetically disrupted state. Provisional restorations play an important role in setting the patient-specific information, such as the position and alignment of teeth for esthetics, pronunciation, lip support, vertical dimension and occlusal plane. In conventional methods, there are many difficulties in reflecting provisional prosthesis to final restorations. However, with the double scanning method, which is a process of superimposing and replicating scanned models of provisional prosthesis on a definitive model, it is easier to reflect provisional restorations to final ones. It also has the advantages of simplifying the manufacturing process, and making it possible to reproduce the same prosthesis when it has been fractured or lost. In this case report, two patients went through a provisional restoration phase to evaluate function and esthetics. The final monolithic zirconia implant restorations were fabricated using the double scanning method. These cases are presented to show that with CAD/CAM technology and double scanning method, it is possible to fabricate an esthetic and functional final restoration that reflects the provisional restoration.

Implant Placement of Severe Alveolar Bone Resorption Site with Oroantral Fistula: A Case Report 重度牙槽骨吸收伴口窦瘘的种植体植入术1例

OPEN ACCESS Introduction: Patients with oroantral fistula usually have severe alveolar bone defects. Alveolar bone augmentation procedure may be needed when placing implant in atrophic bone. Currently, various alveolar bone augmentation techniques are recognized as promising treatments. The purpose of this study is to evaluate the results of simultaneously performed sinus augmentation, guided bone regeneration (GBR) and block bone graft procedures in patient who have the severe alveolar bone loss with oroantral fistula. Case: A 58-year-old male patient was referred for implant removal at #15, 16 intruded in the right maxillary sinus. At first, implant fixture was removed and, #14 was extracted. After 7 months, combination technique including sinus augmentation & block bone graft & GBR was carried out. Sinus augmentation was performed by the lateral approach. The bone block harvested from the mandibular ramus was performed. The bone block is placed on the alveolar bone and fixed with a screw to the alveolar bone with GBR. At 7 months af ter bone augmentation, Implants were placement at the #14, 16, 17 edentulous site. After implantation 6 months, implant second operation was done. And the prosthetic procedure was completed. Conclusion: Healing occurs without complications and at 9 months follow-up check, normal clinical and radiological findings were observed.

Possibility and Limitation of Applying Occlusal Adjustment on Implant Prosthesis similar to Natural Teeth 牙合调节应用于类似天然牙种植体修复的可能性与局限性

OPEN ACCESS Restoration with fixed prostheses using implants is a generalized treatment method in partially or fully edentulous patients. In the past, where only the function of the implant prostheses was emphasized, successful osseointegration was just enough to be considered clinically satisfactory. Recently, however, as the patient’s demands have increased, the recovery of function and esthetics similar to the lost natural teeth has become an important issue for implant prostheses restoration. There may be controversy as to whether occlusion of natural teeth can be applied to implants with different morphological, histological, and dynamical characteristics f rom those of natural teeth. Therefore, this article will investigate the possibility, prerequisites, limitations and solutions of implant occlusion formation similar to natural teeth.
