The Effect of Immediate Dentin Sealing on the Marginal Adaptation of LithiumDisilicate Overlay Restorations using different types of luting agents 即刻牙本质封闭对二硅酸锂覆盖体边缘适应性的影响

Objective: This study evaluated the effect of immediate dentin sealing on the marginal adaptation of lithium disilicate overlays with three different types of resin-luting agents: preheated composite, dual-cure adhesive resin, and flowable composite. Materials and Methods: Forty-eight maxillary first premolars of similar size were prepared with a butt joint preparation design. The teeth were separated into two primary groups, each with twenty-four teeth: Group DDS: Delay dentin sealing (non-IDS) teeth were not treated. Group IDS: dentin sealing was applied immediately after teeth preparation. Each group was subsequently separated into three separate subgroups. Subgroups (DDS+Phc, IDS+Phc): cemented with preheated composite (Enamel plus HRi, Micerium, Italy), Subgroups (DDS+Dcrs, IDS+Dcrs): cemented with dual-cured resin cement (RelyX Ultimate, 3M ESPE, Germany) and Subgroups (DDS+Fc, IDS+Fc): Cemented with flowable composite (Filtek supreme flowable, 3M ESPE, USA). Using a digital microscope with a magnification of 230x, the marginal gap was measured before and after cementation at four different locations from each surface of the tooth, and the mean of measurements was calculated and analyzed statistically using the independent t-test, one-way ANOVA test, Bonferroni correction at a significance level of 0.05. Results: The samples that were immediately sealed with dentin bonding agent showed lower marginal gaps than delayed dentin sealing, both pre-and post-cementation for all subgroups, with a statistically significant difference (p < 0.01). The marginal gap was significantly lower in the IDS+Fc (48.888 ± 5.5 micro m) followed by the IDS+Dcrs group (53.612 ±5.8 micro m) and IDS+Phc (79.19 9±6.9 micro m) respectively, while the largest marginal gaps were observed in the DDS+Phc group (86.505 ± 5.4 micro m). Conclusion: Generally, the teeth with IDS showed better marginal adaptation than teeth without IDS. The marginal gap was smaller with flowable composite and dual-cure resin cement than with preheated composite. Prosthetic maintenance assessment for implant fixed complete dentures and implant overdentures: a randomized clinical trial 种植固定全口义齿和种植覆盖义齿的维护评估:一项随机临床试验

Objective: The rehabilitation choice for the edentulous patients usually lies between the fixed and removable prosthetic options. The treatment decisions are affected by many factors where complications and maintenance needs are both considered crucial factors, in addition to the cost effectiveness of the chosen treatment. Material and Methods: This study was applied on 44 edentulous patients, where 22 patient for each group were enrolled in the outpatient clinic of prosthodontics, Cairo University as per a set of eligibility criteria. Four inter-foraminal implants were installed for all eligible participants. Three months later, healing abutments were used for soft tissue preparation prior to the fabrication of the final prosthesis. A prosthetic treatment option was then randomly allocated to obtain two equal groups via computer generated randomization program; Group. A received telescopic implant overdentures, and Group. B received screw retained dentures. All Complications (Screw loosening or fracture, tooth or denture base fracture and mucositis) were reported after overdenture insertion along the follow up period (1, 6, 9 and 12 months respectively). Results: The frequency of the screw loosening for hybrid overdentures where (59.1%) compared to (27.3%) of telescopic prosthesis at 12 months follow up period (p=0.035), mucositis reporting at 6m interval had shown the highest frequency in both groups (Group A (54.5%), Group B (81.8%), (p=0.045), all other reported complications that lack statistical significance either within the same group or between both groups at different time intervals. Conclusion: Both treatment modalities; telescopic implant overdenture and hybrid fixed screw-retained are reliable for restoring the completely edentulous arches, the decision whether to make a fixed or removable implant denture shall be guided with the patient preference together with the dentist assessment in relation to the patient’s state of general and oral health.

The ability of different formulations of artificial saliva to protect dentin from erosive wear 不同配方的人工唾液保护牙本质免受侵蚀磨损的能力

Objective: Evaluate the protective effect of artificial saliva formulations associated or not with mucin on dentin. Materials and Methods: Bovine dentin specimens were randomly allocated to 10 groups (n = 20) according to the artificial saliva tested and the presence or absence of mucin: Amaechi et al. (1998); Klimek et al. (1982); Vieira et al. (2005) and Eisenburger et al. (2001) and deionized water (control). Samples were submitted to an erosive cycle consisting of two immersions of 120 min in the saliva, followed by 1 min in hydrochloric acid solution, and new storage in saliva for 120 min. Surface loss (μm) was measured before and after the cycle. Data were analyzed using 2-way ANOVA and Tukey’s test (p < 0.05). Results: A significant difference was observed for the saliva formulation but not for the presence of mucin. The deionized water provided the highest surface loss and the Eisenburger’s saliva formulation the lowest. The groups testing the Amaechi, Klimek, and Vieira saliva did not present significant differences. Conclusion: Eisenburger’s saliva formulation provides a higher protective effect against dentin erosion. The presence of mucin did not increase the erosion-preventive effect of artificial saliva formulations.

Materials and Methods: Bovine dentin specimens were randomly allocated to 10 groups (n = 20) according to the artificial saliva tested and the presence or absence of mucin: Amaechi et al. (1998); Klimek et al. (1982); Vieira et al. (2005) and Eisenburger et al. (2001) and deionized water (control). Samples were submitted to an erosive cycle consisting of two immersions of 120 min in the saliva, followed by 1 min in hydrochloric acid solution, and new storage in saliva for 120 min. Surface loss (μm) was measured before and after the cycle. Data were analyzed using 2-way ANOVA and Tukey’s test (p < 0.05). Results: A significant difference was observed for the saliva formulation but not for the presence of mucin. The deionized water provided the highest surface loss and the Eisenburger’s saliva formulation the lowest. The groups testing the Amaechi, Klimek, and Vieira saliva did not present significant differences. Conclusion: Eisenburger’s saliva formulation provides a higher protective effect against dentin erosion. Effect of 10% Proanthocyanidin gel on demineralized organic matrix degradation: ELISA method 10%原花青素凝胶对脱矿有机基质降解的影响:ELISA法

Objective: This study evaluated Proanthocyanidin protective effect on dentin subjected to erosion and its inhibition on degradation of the demineralized organic matrix (DOM). Material and Methods: The tested groups were: G1 - 10% Proanthocyanidin gel (test group), G2 - 1.23% NaF (positive control 1), G3 - 0.012% Chlorhexidine (positive control 2) and G4 – Placebo (negative control with no active compound) and two methodologies were performed: contact profilometry and ICTP ELISA method. To quantify dentin wear, profilometry was performed. Data were submitted to Analysis of Variance followed by Fisher’s LSD Test. To assess the collagen degradation, ICTP ELISA method was performed. Data were submitted to the Kruskal-Wallis followed by the Dunn´s test. Simple linear regression and Pearson Correlation test were also performed (p<0.05). Results: The profilometry showed significantly lower wear of G1 when compared to other groups and G2, G3 and G4, which did not present significant difference among them. In the ICTP ELISA analysis, G1 and G4 did not show significant differences and the same happened between G2 and G3. However, G1 and G4 had lower values of collagen degradation compared to groups G2 and G3. Data showed that degraded DOM is a significant predictor to explain the values obtained through the ICTP ELISA. Conclusions: The results allow to verify that 10% proanthocyanidin provided less tooth wear and decreased degradation of the DOM, suggesting a good ability to prevent dentin erosion. The regression analysis also suggests that contact profilometry is a good strategy to quantify dentin wear. KEYWORDS Dentin; Tooth erosion; Proanthocyanidins; Dental wear; Preventive health.
