[Description of an intervention to prevent pressure-induced skin lesions caused by personal protective equipment (PPE) during the care of patients with COVID-19]. [一项预防COVID-19患者护理期间个人防护装备(PPE)造成压力性皮肤损伤的干预措施的描述]。

Objective: To describe a strategy designed to avoid pressure-related skin lesions related to the use of personal protective equipment (PPE).

Methods: This was a prospective descriptive observational study.

Study population: healthcare professionals at the Parc Salut Mar (Barcelona) who used PPE for protection against COVID-19 between March 30 and June 15, 2020.

Intervention: Each professional was provided with an individual kit to prevent PPE-induced lesions, friction, or moisture. A multidisciplinary surveillance team was deployed for the early detection of these adverse events.

Results: Individual kits were provided to 1044 professionals, 32 of whom were scheduled for in-person medical assessments. Six participants (0.57%) developed Stage III or IV pressure ulcers, 24 (2.3%) had dermatitis, folliculitis, eczema and two participants did not show for their appointments. The overall rate of pressure ulcers was 0.57%; for dermatitis it was 2.3%.

Conclusions: The use of preventive measures appears to be effective in preventing facial pressure ulcers induced by EPIs among healthcare professionals. Our resultsalso underscore the importance of expanding preventive measures to include not only pressure ulcers but also dermatitis.

[Effects derived from occupational exposure to hazardous substances in pregnant working women: systematic review]. [职业性接触有害物质对怀孕女工的影响:系统评价]。

Objective: To review the effects derived from occupational exposure on pregnant working women exposed to hazardous substances.

Methods: Critical analysis of the works retrieved by systematic review in MEDLINE (PubMed), EMBASE, Cochrane Library, Scopus, Web of Science, LILACS and MEDES, up to January 2021. The Search Equation was developed by means of the descriptors «Pregnancy», «Hazardous Substances» and «Occupational Exposure», as well as using related Entry Terms and the filters «Humans» and «Adult: 19+ years». The quality of the articles was evaluated via the STROBE questionnaire and the level of evidence and the grade of recommendation via the SIGN criteria.

Results: Out of the 420 references retrieved (366 digitally and 54 manually) and after applying the inclusion and exclusion criteria, 35 articles were selected. The STROBE evaluation obtained a median of 15.32, and the SIGN criteria, a 2+ level of evidence and a C grade of recommendation. Article obsolescence was high (Burton-Kebler half-life: 22.0; Price index: 0.00%). In 25 out of 35 articles, the exposure is to organic solvents. In 22 articles, congenital anomalies were notified.

Conclusions: The reviewed articles exhibited high obsolescence and a degree of evidence and recommendation that did not allow to fully ensure the validity and reliability of the observations made. The results manifested the association between the exposure to dangerous substances and the occurrence of different damages regarding the health of a pregnant worker and her offspring. The most common exposure was to organic solvents and the most observed effect was congenital alterations.

En la notificación nº 366 “Enfermedad de Coronavirus” publicada por el Ministerio de Sanidad en fecha 03/05/2021, con los datos individualizados notificados por las CCAA a la Red Nacional de Vigilancia Epidemiológica (al sistema SiViEs), se indica que están notificados en España 3 540 430 casos de COVID-19 de los cuales 78 293 son fallecidos. Del total de casos notificados, corresponden al personal sanitario y sociosanitario unos 80 065 (2.26%) casos de COVID-19 notificados con diagnóstico posterior al 10 de mayo de 2020, siendo significativamente mayor el porcentaje entre las mujeres (77.95% o 62 416 casos) que entre los hombres (21.95% o 17 582 casos). Además, a la cifra de 80.065 casos se deben sumar un total de 35.548 casos declarados desde el inicio de la pandemia hasta el 7/05/2020(3), obteniendo una cifra total de 115 613 casos notificados de COVID-19 en personal sanitario.

Introduction: Frontline healthcare workers have a high risk of exposure to SARS-CoV-2 coronavirus, which causes COVID-19. The use of appropriate personal protective equipment (PPE) is essential to prevent this occupational disease. Surgical masks and filtering face piece (FFP) respirators are important parts of this PPE. European standard EN 149 establishes three protection levels for FFP respirators (FFP1, FFP2, FFP3), depending on the particle infiltration degree through their materials, and these, in turn, are based on their filtration effectiveness. The aim of this laboratory test is to determine and quantify the filtration and fit rate of different FFP respirators, singly and in combination with surgical masks, by performing a series of fit tests and consequently, to check whether this combination improves protection levels for healthcare workers who care for COVID-19 patients.

Material and methods: Several FFP respirators and surgical masks, singly and in combination, were fit tested with a PortaCount Pro + 8038, which fulfills OSHA standards, in a series of tests performed on healthcare workers in seven different breathing situations when taking care on COVID-19 patients, in order to determine and quantify their fit to the workers' face.

Results: Wearing a surgical mask together with a highly efficient FFP respirator provided increased respiratory protection. Interestingly, one of these highly efficient FFP models, combined with a surgical mask, achieved a protection factor over 200 (whereas 100 is the minimum required protection factor).

Conclusions: Surgical masks, when used together with a FFP2 respirator, could significant ly improve the degree of fit of all self-filtering face piece by providing greater filtration efficiency and greater user protection from exposure to aerosols.

Objectives: To analyze the differences in work-related eye injury trends among the different Autonomous Communities in Spain.

Methods: This was a descriptive, retrospective and longitudinal study based on a Social Security-affiliated mutual insurance company work injury and illness database for the period 2008-2018 that included all Spanish Autonomous Communities. Accidents that caused an injury to any ocular structure were analysed, and both the incidence of work-related eye-injuries per 100 000 insured workers and the relative risk of suffering an eye injury were compared.

Results: We observed a general decrease in work-related eye injuries in all of the communities, although with two distinct time periods. The annual percentage change decrease occurred in all communities until 2013. From 2013 to 2018, however, this decreasing trend was only observed in Asturias, Castilla-La Mancha, Canary Islands, Madrid, Murcia and Navarra. Castilla-La Mancha was the Autonomous Community with the highest incidence (886.56/100.000), as well as the highest relative risk of suffering an eye injury (RR 2.66; IC 95% 2.58 - 2.75). At the other end, the lowest incidence (82.25/100.000) and RR (RR 0.247; IC 95 % 0.22 - 0.28) were observed in Catalonia.

Conclusions: We observed a statistically significance decrease in the incidence and relative risk of work-related eye injuries in all Autonomous Communities over the study period, although this trend changed from 2013 onward.

[Occupational health nursing: a holistic perspective]. [职业健康护理:整体视角]。

Occupational health nursing has a unique opportunity to identify the needs of the working population within the scope of its competences. In the different possible future of work scenarios, factors related to globalization, demographic changes, the massive use of tech - nology, the digitization of a significant number of processes, the multidisciplinary nature of the approach to worker care and some approaches close to public health, are driving a new approach to occupational health and safety, while eliciting an ethical debate. In this social, professional and occupational reality, interventions by this nursing subspecialty will need to be based on comprehensive, safe, collaborative, coordinated, quality care, focused on people and based on the best available scientific evidence.
