Abses Otak 脑脓肿

Cerebral abscess may rise from hematogenous spread, contiguous spread, or direct trauma. Some risk factors includepulmonary abcess or AV fistulas, congenital cyanotic heart disease, immune compromise, chonic sinusitis / otitis,dental problems. Symptoms are similar to any other mass lessions but tend to progress rapidly. Streptococcus is mostcommon organism that recovered, while up to 25% of cases sterile. Staging of cerebral abcess is very important todifferentiate therapeutic management based on clinical findings and imaging. Mortality and morbidity rate has beenreduced due to improvement in antibiotics, surgery and ability to diagnose.

Sepsis is life-threatening condition caused by bacteria. Sepsis patient usually treated in ICU with antibiotic as treatment.Irrationally antibiotic usage can lead to antibiotics resistance which increased morbidity and mortality This studyaimed to know the characteristic of sepsis patients, specimen identification, bacteria analysis, and antibiotic resistanceof sepsis patient in Intensive Care Unit (ICU). The source of data were from medical record including age, gender,culture specimen, bacteria culture test and result of antibiotic resistance test. Result showed that27 of samples, therewere 51,85% male and 48,15% female patient with most common age group was >65 years old (25,93%). Total amountof specimen that had pathogenic bacteria was 37, (67,57% sputum cultures and 18,92% blood cultures). 37 of samplesfound to be gram negative bacteria and highest number was A.baumanii (37,85%). Antibiotic resistance test showedthat sepsis-causing bacteria has highest resistance to ampicillin (100%) and most sensitive to amikacin (72,98%) andtigecyclin (51,36%).

Identifikasi Protozoa Usus Oportunistik dan Faktor Risikonya Pada Anak Panti Asuhan Kota Pekanbaru 机会主义原生动物的鉴定,以及北干巴鲁市儿童孤儿院的风险因素

Opportunistic intestinal protozoa are protozoa that can infect humans at a time when the body’s defense system isdeclining (immunocompromising). Opportunistic protozoan infections are infections by intestinal protozoa that werenot previously considered important, but now can cause disease in humans. Opportunistic intestinal protozoan infectionsbesides being found in immunocompromised patients are also reported to infect children. Some of the intestinalprotozoan species that have been identified to cause infection are Cryptosporidium sp, Isospora belii, Cyclospora sp,and Blastocystis hominis. This study was an analytical study with a cross sectional design conducted in June 2018until January 2019. In this study, modified Ziehl-Neelsen staining was used and the results were 25.6% of orphanagesinfected with opportunistic intestinal protozoa, which consisted of Cryptosporidium sp. as much as 14.1%, Blastocystishominis as much as 4.6%, infection with a mixture of Cryptosporidium sp and Blastocystis hominis as much as 4.6%,Isospora belii as much as 2.3%, and no infection by Cyclospora cayetanensis. There was no association betweenopportunistic intestinal protozoan infections with poor handwashing habits, bowel habits, food hygiene, drinkingwater treatment, and bad animal raising habits.

Akupunktur sebagai Terapi De Quervain’s Tenosynovitis

De Quervain’s tenosynovitis is a disease with pain and edema in the styloid process due to thickening of the sheathsthat encase the tendons of abductor pollicis longus (APL) and extensor pollicis brevis (EPB). Management of DeQuervain’s tenosynovitis include pharmacotherapy combined with conservative therapy and if it fails then surgicalintervention is required. In case of De Quervain’s tenosynovitis, acupuncture for relieving pain and stiffness. Reportedthe case of a woman aged 52 year, complaints of pain and stiffness in the left thumb since two months before went tothe Poliklinik Akupunktur dr. Cipto Mangunkusumo Nasional Hospital. On physical examination the left wrist areafound tenderness and spasm in the area of APL and EPB, VAS 4, there was limitation in the first carpometacarpal jointfunctional and Finkelstein’s test was positive. Acupuncture therapy done at the point LI5, LU7, LU9 and Ashi. Therewas pain reduction and functional improvement of the carpometacarpal joints. Acupuncture gives good results forpain and stiffness in patients with de Quervain’s.

Gambaran Jumlah Neutrofil Darah Tepi Pasien Penyakit Paru Obstruktif Kronik (PPOK) di Ruang Rawat Inap RSUD Arifin Achmad Provinsi Riau Tahun 2017

Chronic Obstructive Pulmonary Disease (COPD) is a common, preventable and treatable diesease that is characterizedby progressive and persistent airflow limitation in respiratory tract and associated with an inflammatory responsecaused by exposure to noxious particles or gases. Several study showed that neutrophils are more activated and playsan important role in phatogenesis of COPD patients. The aim of this study is to know the description of peripheralblood neutrophil of COPD patients. This study was using descriptive retrospective design with total sampling technique.The sources of data were medical records of COPD patients in January – Desember 2017 with number of sample is 137patients. The results of this study is COPD patients with high neutrophils were found in 94 (68,61%) patients. Thisstudy shows that COPD patients at lung wards Arifin Achmad Regional General Hospital Riau Province in 2017obtained neutrophilia.

Tingkat Daya Tahan Jantung Paru Pada Pusat Pendidikan dan Latihan Pelajar Atlet Sepak Bola (PPLP) Pekanbaru Menggunakan Metode Lari 12 Menit Pekanbaru采用12分钟短跑方法训练和教育中心的肺耐力

Cardiorespiratory fitness is defined as the maximum capacity of the heart and lungs to inhale and spread oxygen orabbreviated as VO2 Max. The higher the VO2 max, the higher the body’s resistance when exercising, so that an athleteis very important to have a high level of VO2 max. This research was a descriptive study with a sample of all Center forEducation and Training soccer athletes measured using the 12-minute run method. Based on the research, the highestage was 15 years with a frequency of 9 (37.5%), with the most practice time was those who practiced for <1 year witha frequency of 15 (62.5%), most of the respondents have normal nutritional status with frequency 17 ( 70.8%), and thehighest value of cardiorespiratory fitness (VO2 Max) is sufficient with a frequency of 17 athletes (70.8%).

Pengaruh Kombinasi Latihan Beban dengan Metode Pyramid set dan Konsumsi Susu Tinggi Protein Terhadap Peningkatan Massa Otot 负载训练与金字塔式成型方法和高蛋白牛奶对肌肉质量增加的影响

Weight training with pyramid set method and high protein milk are the effective methods to increase muscle mass.There are not many studies that combined these 2 methods to increase the muscle mass. The purpose of this study wasto analyze the effect of combination of pyramid weight training sets and consumption of high protein milk to increasethe muscle mass. This type of study is a quasi-experimental one group with a pre and post test design. The studyperiod was 8 weeks with 32 meetings and high protein milk in of 32 grams of whey protein concentrate before and afterexercise.The results of this study also prove that there is a significant effect of intervention on participants musclemass with p = 0,000. It can be concluded that the combination of pyramid weight training sets and high protein milkconsumption is effective to increasing muscle mass.

Diagnosis Cepat Kandidemia Neonatus dengan Pemeriksaan Spesimen Darah Pulasan Giemsa

Candidemia is one of the major problems in neonates with low birth weight (LBW). Neonatal candidemia has a high rateof morbidity and mortality. Early initiation of antifungal therapy could be decreasing the mortality, but this managementoften hampered due to late diagnosis. The gold standard for diagnosing candidemia is blood culture, but it takes a longtime about 5 days. A rapid alternative method is needed to decrease candidemia morbidity and mortality. The methodchosen in this study is Giemsa stain of blood smear which result could be read within one hour. The study wasconducted in the Department of Parasitology, Faculty of Medicine Universitas Indonesia with a total blood samples of170 from 2009-2012. Blood samples are devided in two, first is made thick blood smear than stain with giemsa, than readby microscope by 400×and 1000× magnification. The second is cultured on Sabauraud agar media and incubated inroom temperature for ten days. Thirty four patients (20%) were positive for Candida, 28 (82.4%) positive samples withboth giemsa staining and culture, while 6 (17,6%) positive culture samples but negative giemsa staining. The values ofthe sensitivity and specificity of the Giemsa stained blood smear examination were 82%, and 100%. This result showsthat Giemsa staining has a good diagnostic value for detecting neonatal candidemia.

Over the last few years, there has been a decrease in amount of effective antibiotics, coincide with the lack of newantibiotics relevant for the treatment of Gram negative multi-drug resistant organisms infections. This problem can beovercome by the assessment and re-introduction of old antibiotics such as fosfomycin. Fosfomycin is a broad spectrumantibiotic that is bactericidal. This study aimed to find out the sensitivity pattern of clinical isolate multi-drug resistantGram negative bacteria to fosfomycin with descriptive cross sectional method. The sample used in this study wasclinical isolates collection of Microbiology Laboratory Faculty of Medicine, University of Riau from Hospitals inPekanbaru. The result showedsensitivity to fosfomycin Escherichia coli producing ESBL (88%), Klebsiella pneumoniaeproducing ESBL (95%), Acinotobacter baumannii resistant meropenem (27%), Pseudomonas aeruginosa resistantmeropenem (36%) and K.pneumoniae resistant meropenem (70%). Fosfomycin can be used as alternative therapy forinfections caused by Gram-negative Multi-drug resistant organisms.

Gambaran Radiografi Toraks Pasien HIV Dengan Tuberkulosis Paru Berdasarkan Nilai CD4 Di Rumah Sakit Umum Arifin Achmad Provinsi Riau

Diagnosis of tuberculosis in HIV-Tuberculosis coinfected patients is difficult. Thorax x-ray is a primary tool to evaluatetuberculosis in HIV. The purpose of this cross-sectional study was to assess and compare various radiologicalpatterns of pulmonary tuberculosis in HIV patients with CD4 counts. This study conducted in Arifin Achmad HospitalRiau province from December 2016-December 2018 with 55 HIV-tuberculosis coinfected patients. WHO guidelineswere used for diagnosis of HIV and tuberculosis. 100% of the patients had sputum positive pulmonary tuberculosis,with higher incidence (83,6%) among CD4 less than 200 as compared to CD4 more than 200 (16,4%). Infiltration (32,7%)followed by normal (18,2%), and fibrosis (11%) seen with CD4 less than 200. Infiltration (9,2%) followed by normal(3,6%) and fibrosis (1,8%) with CD4 above 200. Mid and lower zone (50%) more commonly seen in CD4 < 200. Despitein CD4 >200 upper zones more commonly seen. Most lesions were found in mid zone of lung. There were significantdifferences in radiological patterns of pulmonary tuberculosis in HIV according to CD4 counts.
