Impact of home environment on motor development of infants in South India: A cross-sectional study  家庭环境对南印度婴儿运动发展的影响:一项横断面研究

Introduction: Early childhood development is a fundamentally important phase and is influenced by stimulus and nurturing. Environment influences, particularly the home environment, is considered the critical factor for optimal growth and development.  Objectives: To determine the impact of home environment on motor development in South Indian infants. Method: 164 infants aged 1-11 months residing in Belagavi City, India, were recruited in a community-based cross-sectional study based on the inclusion and exclusion criteria. Structured questionnaire was used to assess the home environment followed by motor development assessment using Peabody developmental Motor Scale-2 (PDMS-2).  Results: Gestational age was positively associated, while duration of breast feeding, hospitalization and weight of the infant (-1.17, p= 0.03) had negative association with the fine and total motor quotient of PDMS-2. Father’s education, occupation and income had significant positive effect on total and fine motor quotients. Outside space to play had positive association with fine motor quotient (2.28, p= 0.0154). Non availability of play material like pop up toys and child never exposed to play with floating toys, cups and socks also showed lower total quotient scores.  Conclusions: Gestational age, father’s education, occupation and income had a positive effect on the motor development of South Indian infants participating in the study.

Serum interleukin-17 as a biomarker in paediatric bronchial asthma: A case control study  血清白细胞介素-17作为儿童支气管哮喘的生物标志物:一项病例对照研究

Introduction: Bronchial asthma (BA) is a chronic childhood disease causing significant morbidity and mortality. A novel and suitable biomarker is needed for early diagnosis of BA.  Objectives: To establish the association of serum interleukin-17 (IL-17) levels in children with BA and to determine the diagnostic performance of IL-17 in predicting severity of BA.  Method: This was a case control study conducted at the Institute of Child Health and Research Centre, Government Rajaji Hospital & Madurai Medical College, Madurai, India from August 2020 to July 2021. Cases were selected according to the Global Initiative for Asthma (GINA) guidelines and controls were healthy siblings or age and sex matched controls The associations of IL-17 with BA were statistically analysed using Epi info v7 and SPSS 20. Analysis was done using one way ANOVA and t-test. ROC curve was used to find the diagnostic cut-off point. p-value <0.05 was considered statistically significant.  Results: Mean age of the participants was 8.74±2.21 years; 55.8% were males. Mean IL-17 level was significantly higher among cases (2.5±2.7) as compared to controls (1.31±0.96) (p=0.0043). There was a significant increase in mean IL-17 levels with increase in severity (p=0.00). The area under curve for IL 17 levels in diagnosing severe persistent asthma was 0.870 (95% CI=0.708-1.000). The best diagnostic cut-off point was 3.26pg/ml with a sensitivity of 90% and a specificity of 90%. The best diagnostic cut-off point of IL 17 was 1.12pg/ml in predicting asthma with a sensitivity of 60% and a specificity of 50%.  Conclusions: IL 17 level can used as a biomarker for identifying patients with severe persistent asthma.

Assessment of perception of oral healthcare professionals in management of medical emergencies in children: A cross-sectional study 口腔保健专业人员在儿童医疗紧急情况管理中的认知评估:一项横断面研究

Background: Medical emergency is an acute condition which can occur when least expected. It is more common to occur in a dental setup as this environment can be stressful for a patient. Children are more prone to emergencies as their dental procedures usually takes place under sedation. Therefore, it is mandatory for a dentist to have adequate knowledge, proper attitude and good training in handling any medical emergency seen in children during their dental treatment.   Objectives: To assess the perception of oral healthcare professionals in management of medical emergencies in children. Method: A cross-sectional survey comprising 270 participants was conducted on oral healthcare professionals including interns, postgraduate students, practising dentists and faculty dentists. An 18-item validated questionnaire containing four domains of professional demographic data, knowledge, attitude and practice were distributed among the participants. Descriptive analysis was used followed by Chi Square test to check for association between age, gender, educational qualification and years of experience.Results: The level of knowledge, attitude and practices was lower in graduate dentists than postgraduate dentists. The correlation between knowledge, attitude and practices was found to be highly statistically significant (p=0.001).Conclusions: The study concluded that while knowledge and attitude levels were high in the population, there is a gap in practices of the dentists to handle a medical emergency.

Physical growth and intellectual function of children with congenital hypothyroidism: An observational study 先天性甲状腺功能减退症患儿的身体发育和智力功能:一项观察性研究

Introduction: Congenital hypothyroidism (CH) is an important preventable cause of mental retardation. Objectives: To assess growth parameters and the developmental outcome of children with permanent CH on follow up. Method: This was a hospital-based observational study done in the paediatric endocrinology clinic of a tertiary care hospital in South India. The study population included all children with permanent CH aged 5-18 years attending the clinic from March 2017 to March 2018. Weight, height and head circumference of children were measured and the body mass index (BMI) was calculated. Ultrasonography (USG) of the thyroid was done for all patients and structural abnormalities of the thyroid (aplasia, hypoplasia or ectopic gland) were taken as dysgenesis. If USG had normal thyroid it was taken as dyshormonogenesis. Intelligence quotient (IQ) assessment was done by a clinical psychologist. Results: There were 35 children with CH enrolled in the study. Of this, 17 (49%) were diagnosed on or before 6 months of age and 18 (51%) after 6 months of age; 24 (69%) had thyroid dysgenesis and 11 (31%) had dyshormonogenesis; 4 had severe stunting and 5 were obese. Among the 17 children diagnosed on or before 6 months of age, 12 (70.6%) had normal intelligence and 5 (29.4%) had IQ <85 whereas among the 18 children diagnosed after 6 months of age 6 (33%) had normal intelligence and 12 (66%) had IQ <85, which was statistically significant (p=0.028). There were no significant differences between early and late diagnosis groups in other parameters like developmental delay, stunting and BMI. Among 24 children with dysgenesis of thyroid gland, 14 (58%) had IQ <85 whereas among 11 children with dyshormonogenesis, 3 (37.5%) had IQ <85. This was not statistically significant (p=0.08). Conclusions: In this study, a statistically significantly larger proportion of children diagnosed on or before 6 months of age had normal intelligence compared to children diagnosed after 6 months of age. The study population included all children with permanent CH aged 5-18 years attending the clinic from March 2017 to March 2018.
