Finding Installed Capacity For Practical Training Of A Designated Medical Specialty At A University Hospital 寻找大学医院指定医学专业实践培训的装机容量

Objective: Propose a methodology to determine the number of medical students who can rotate, for the practice of medicine, in a university hospital, so that the quality of training processes and in-patient care are assured. Materials and Methods: A three-step procedure is presented, in order to find the number of students that the institution can accept simultaneously. Results: The method is based on an integer linear model and it was implemented to assess installed capacity of General Surgery service at Hospital Universitario Clínica San Rafael, increasing in two students (33 %) the training capacity. Conclusions: The proposed methodology not only guaranties the quality of training processes and in-patient care, but also generates other intangible results such as having a more agile way of planning, reducing the planification time. The methodology is easily extended to other services within hospitals.

Estimación de la incertidumbre en la calibración de esfigmomanómetros según OIML R16-1 desde una perspectiva de metrología legal 从法律计量的角度估计OIML R16-1血压计校准的不确定度

Objetivo: Estimar la incertidumbre en la calibración de esfigmomanómetros bajo la OIML R16-1 desde una perspectiva de metrología legal.  Materiales y métodos: Se seleccionaron aleatoriamente cuatro (4) esfigmomanómetros de diferentes marcas con resolución de 2 mmHg para realizar la calibración, la cual consiste en realizar comparaciones directas entre el esfigmomanómetro y un manómetro patrón digital con resolución de 0,01 mmHg, generando presión por medio de una pinza neumática. La incertidumbre se estima usando la Guía para la Expresión de la Incertidumbre de Medida (GUM) y la recomendación internacional OIML R16-1, mediante dos series de mediciones, cada una con secuencia ascendente y descendente, para obtener en cada caso 4 mediciones con su error correspondiente. Resultados y discusión: Se obtiene la representación de la incertidumbre en sus expresiones tipo A, tipo B y expandida, por medio de un modelado físico y matemático no estocástico. De las series de mediciones obtenidas del esfigmomanómetro en los procesos de calibración, se analizan los aportes obtenidos de incertidumbre, donde se infiere que la incertidumbre predominante, en la mayoría de los casos, es por resolución del equipo, seguido del comportamiento del equipo en la prueba de repetibilidad e histéresis. Conclusiones: La estructura propuesta para la estimación de la incertidumbre sigue los lineamientos internacionales y puede usarse también en procesos de aprobación de modelo para un impacto final en la seguridad del paciente.

Study of Concentration-depth Profiles of the Titanium and Nitrogen Ions by SRIM/TRIM Simulation SRIM/TRIM模拟研究钛、氮离子浓度-深度分布

In this research, the concentration-depth profiles reached by titanium and nitrogen particles, on the surface of AISI/SAE 1020 carbon steel substrates, by using of ion implantation technique, are studied. The ions are surface deposited by means of high voltage pulsed discharges and electric arc discharge under high vacuum conditions. The concentration and position distribution of the metallic and non-metallic species are obtained by simulation of the interaction of ions with the matter, stopping and ranges of ions in the matter, by the computer program transport of ions in matter. The implantation dose is calculated from the discharge data and the previously established study parameters in this work. From the simulation results, the depth profiles demonstrated that titanium and nitrogen ions may reach up to 300 Å and 600 Å and concentrations of 1.478 x 1016 ions⁄cm2 and 2.127 x 1016 ions⁄cm2, respectively. The formation of titanium microdroplets upon the surface of the substrates is identified from the micrographs obtained by the scanning electron microscopy technique; furthermore, the presence of titanium and nitrogen implanted on the surface of the substrate is verified through the elemental composition analysis by the energy dispersive spectroscopy, validating the effect of ion implantation on ferrous alloys.

Prototype for Knee-load Relief through Ischiatic Support 坐骨肌支撑减轻膝关节负荷的原型

Objective: The objective of this work was to develop a passive exoskeleton prototype for the relief of knee-load employing ischiatic body weight support. Methods and materials: A functional prototype was developed and tested with three volunteers to analyze its potential effectiveness and effects on gait kinematics. The performance of the prototype was assessed using motion capture and pressure mapping systems, and a testing bench for the study of ischiatic body weight. Results and discussion: The results of the tests indicate that the prototype allows reducing the load supported by the knees and does not have a significant effect on the kinematics of the hip and knee joints. The process allowed the designers to identify possibilities of improvement mainly on reducing the restrictions imposed by the prototype to the motion of the ankles, especially on the midstance of the support phase. Conclusions: The passive exoskeleton prototype developed for ischiatic body weight support allows setting different percentages of knee-load relief. The prototype does not have a significant effect on the kinematics of the hip and knee joints. Nevertheless, improvements must be performed to reduce the restrictions to the motion of the ankles.

System for Immersive Medical and Engineering Training Based on Serious Games 基于严肃游戏的沉浸式医学与工程训练系统

Objective: The goal of this study was to present a system (hardware and software) focused on training by exploring and interacting with realistic models on virtual worlds throughout the use of serious games. Methods: Two applications are proposed. The first application uses realistic Building Information Modeling (BIM) files to develop simulations and focuses on machines’ operational safety training in engineering facilities and laboratories. The second is a tool that improves skills to interpret Medical Imaging (MI). Both virtualization games create interactive models that can be explored using a first-person view serious game on a Cave Automatic Virtual Environment (CAVE). Results: A perceptual study was performed asking focus groups to evaluate their experiences with the implemented serious games. According to the users’ perception, an immersive serious game for training is an engaging and interesting activity, taking into account that the virtual environment experience is very realistic. In general, a serious game displayed in a virtual reality environment can be an effective tool for learning. Conclusions: Although this kind of virtual reality systems could be considered novel for professional training, they need to be constantly updated in order to maintain students involved and focused on the learning process.

Impact of substrates on the electrical properties of thin chromium films [Online First] 衬底对铬薄膜电性能的影响[在线浏览]

Objective: We studied the impact of substrates on the electrical properties of thin chromium films. Substrates may serve many purposes, such as to define orientation, to conduct electrical current in vertical devices, as a gate in transistors, etc. The thickness range of the chromium films grown on both substrates was (3.5-70) nm. Methods and materials: We used Fuchs-Sondheimer(FS) and Mayadas-Shatzkes(MS) theories to analyze electrical resistivity data for chromium(Cr) films grown on both substrates simultaneously by thermal evaporation in vacuum, under identical deposition conditions. Results and discussion: The infinitely thick film resistivity (ρ0), conduction electron mean free path(l), specularity parameter(p), scattering power of the grain boundary(α') and grain boundary reflection coefficient(R') were found to depend upon the nature of the substrate and the binding force between them and evaporated chromium atoms. The growth and microstructure of the chromium films were examined using atomic force microscopy (AFM) and scanning electron microscopy (SEM). Conclusions: Our experimental data exactly fits with the MS theory in the entire thickness range grown for the chromium films deposited on both the substrates. Examination of film structure by SEM indicated that the films consist of grains of relatively pure chromium of different sizes, and depends upon deposition conditions and parameters, which are important factors that dictate the structural properties of the films.

Results and discussion: The infinitely thick film resistivity (ρ0), conduction electron mean free path(l), specularity parameter(p), scattering power of the grain boundary(α') and grain boundary reflection coefficient(R') were found to depend upon the nature of the substrate and the binding force between them and evaporated chromium atoms. The growth and microstructure of the chromium films were examined using atomic force microscopy (AFM) and scanning electron microscopy (SEM). Conclusions: Our experimental data exactly fits with the MS theory in the entire thickness range grown for the chromium films deposited on both the substrates. Novel computational approach to solve convolutional integral equations: method of sampling for one dimension [Online First] 求解卷积积分方程的新计算方法:一维采样方法[在线优先]

Objective: This paper proposes a new methodology to solve one-dimensional cases of integral equations with difference kernels using Fourier analysis. Methodology: In this study, it was proven that any Fredholm equation of the first kind can be expressed as an extended convolutional problem; consequently, a new approach to solve that problem, using the nonideal instantaneous sampling theory and Fourier analysis, can be developed. Results and Discussion: The proposal was extensively evaluated and compared with the method of moments by considering two benchmarks. The first was a narrowband problem related to a second-order differential equation with specific boundaries. The second was a standard wideband problem related to wire antenna radiation in electrodynamics, known as the Pocklington equation. In both cases, we derived new interpretations and different approaches to solve the problems efficiently. Conclusions: The new proposal generalized the method of moments via new interpretations, strategies and design rules. We found that the techniques based on the method of moments are point-matching procedures independent of the weighting functions; the basis functions can be designed as generalized interpolation functions with more information provided by the original domain; the weighting functions literally represent a sampled linear filter; the unknown continuous function can be approximated without using the classical variational approach; and several new strategies based on the Fourier transform can be used to reduce the computational cost

An approach for the identification of particulate matter in the clouds of Bogotá using satellite imagery analysis [Online First] 利用卫星图像分析识别波哥大云中的颗粒物的方法[在线第一]

Objective: An analysis of the air quality of Bogotá by identifying clouds during the period from 2013-2017 and verifying patterns of behavior between cloud formation and the concentration of particulate matter is presented. Materials and methods: The study sample includes data provided by the Bogotá Air Quality Monitoring Network (RMCAB), taking into account the concentration of particulate matter, temperature, precipitation, wind direction and wind speed. The data are compared with Landsat 8 satellite images and different combinations of spectral bands through the use of the Geographic Information System (GIS) ArcGis. Results and discussion: A high model correlation is reflected in a percentage greater than 90%, presenting a greater coincidence with a periodicity of two years during the dry period; it is possible to observe that the concentration of pollutants follows the trend of the wind vector lines, and the concentration has a direct correlation with cloud formation, which is influenced by temperature, wind speed and wind direction. Conclusions: This paper provides an alternative for the measurement of particulate matter and contributes to the collection of information on this research topic.

Model for the prediction of noise generated by fixed sources [Online First] 固定噪声源噪声的预测模型[在线优先]

Objective: This article proposes a prediction model applicable to the propagation of noise generated by fixed sources as the result of an analysis of phenomena related to the generation and propagation of sound levels and the subsequent correlation between the levels estimated with the data recorded in the field. Materials and methods: An experimental program was designed that included the measurement of sound pressure levels with a sound level meter to different weather conditions and distances from the noise emission source to compare them with the levels estimated by ISO 9613 Part 2. A statistical analysis of the data recorded in field was performed to observe the dependence with the meteorological variables recorded during the measurements. Results and discussion:  Standard error for the proposed prediction method is 11.4 dB, and absolute average error is 9.1 dB. The correlation coefficient of the proposed model is 0.87. A statistically significant relationship exists between the variables with a 95.0% confidence level. Conclusion: A propagation model was obtained that presented a better fit than the method of ISO 9613 Part 2 and a higher correlation coefficient.

Hemoglobin screening using cloud based mobile photography applications [Online First] 利用基于云的移动摄影应用筛选血红蛋白[在线第一]

Objective: This paper aims to present Selfienemia, a novel non-invasive hemoglobin estimation mobile application working under controlled illumination conditions. Materials and methods: Mobile application was supported by a parameter optimization and curve fitting backend in the cloud to provide dynamic and device independent results based on a-priori observations and the World Health Organization’s (WHO) color scale.  Thirty-four and sixty-four individuals were involved during both the mobile app training and posterior blind test respectively. Results: Selfienemia (Mobile App) was found to be suitable as a non-invasive hemoglobin estimation tool. Estimation levels showed high correlation against the gold standard invasive test. Skin pigmentation and color components were found to have specific relationships labeling individuals within one Hb concentration group. Conclusion: Selfienemia may provide an effective way to screen hemoglobin in medical shortage situations. However, it still can’t replace traditional invasive testing. Future work must include testing additional fitting techniques for illumination aware scenarios as well as quantification of skin pigmentation effects on estimation results.
