Experimental And Clinical Evidence of the Sulfide Balneotherapy Influence Efficacy on the Trophological and Regenerative Status: a Fundamental, Randomized Controlled Trial 硫化物疗法对营养和再生状态影响的实验和临床证据:一项基本的随机对照试验

INTRODUCTION. The therapeutic strategies for managing patients with comorbid conditions, such as arterial hypertension (AH) and prediabetes, are widely adopting non-drug methods along with the recommended medications.AIM. To evaluate the effect on fat distribution and humoral status of sulfide balneotherapy in the conditions of the experiment on white rats and clinical study of AH and prediabetes patients.MATERIAL AND METHODS. The study was of a clinical and experimental nature and consisted of 2 phases – experimental and clinical. The experimental study was carried out on obese, non-pedigreed white rats. Animals were exposed to sulfide baths and a combination of sulfide applications and baths. In the clinical phase of the study, patients with AH and prediabetes were included. All patients included in the present study received standardised drug therapy according to clinical guidelines, and patients in the observation group additionally received combined sulphide balneotherapy (Suksun lake sulphide applications and baths) for 14 days. The study program included assessment of weight, waist circumference (WC), body mass index (BMI), assessment of serum concentrations of vascular endothelial growth factor (VEGF); tissue growth factor (TGF b), insulin and leptin, as well as determination of epicardial fat thickness (EFT).RESULTS AND DISCUSSION. The experimental conditions revealed lipolysis in adipocytes (omentum), as well as an increase in the number of hemocapillaries in all areas of the heart and the growth of CD34 expression. The results of the clinical stage of the study suggest redistribution of fat depots against the background of sulfide balneotherapy. There were no statistically significant differences in weight and BMI in the dynamics, but there was a significant decrease in OT and TEF in the group of patients who received balneotherapy, who also had a significant decrease in HEFR concentration, leptin and an increase in TEFR beta 1.CONCLUSION. The presence of lipolytic, regenerative and adipokine-regulating effects of sulfide balneotherapy was demonstrated in the experiment and clinical study.

Cephalgic Syndrome Correction in Pediatric Patients at the Sanatorium Treatment Stage: a Simple Randomized Prospective Study 儿童患者在疗养院治疗阶段的头痛综合征纠正:一项简单的随机前瞻性研究

INTRODUCTION. Diagnosis and treatment of headache in pediatric patients is of priority medical and social importance, including when correcting it at the sanatorium and health resort stage. Difficulties and problems are associated with the complexity of taking an anamnesis in children, incorrect diagnosis, comorbidity, the need for timely correction of risk factors, and the appointment of the effective pathogenetically proved treatment.AIM. To determine the structure and features of the cephalgic syndrome in pediatric patients undergoing treatment in Evpatoria sanatoriums and to determine the effectiveness of psychological correction in the treatment of patients with frequent tension-type headache by using the method of music therapy method.MATERIAL AND METHODS. As a result of a complex examination of pediatric patients, 150 patients with headache complaints were selected out of 513 examined by the method of simple random sampling. Together with a psychologist, a subgroup of 30 patients with frequent tension headaches was selected from the total number of patients after psychological testing for whom music therapy was included in the treatment plan.RESULTS AND DISCUSSION. In the structure of the cephalgic syndrome the prevalence of tension headache was revealed, while 20% of patients had frequent episodic tension headache against the background of increased anxiety and fatigability, which significantly affects quality of life and social functioning. The patients who underwent correction of tension headaches by music therapy showed improvement of extraversion indices, reduction of neuroticism, decrease of anxiety, fatigability and irritability indices, decrease of frequencyand intensity of headaches. None of the patients were diagnosed with headache according to ICHD-III in the referral diagnosis for sanatorium treatment, but were diagnosed with vegetative-vascular dystonia and other comorbidities.CONCLUSION. The obtained data of a complex examination made it possible to substantiate a pathogenetically differentiated rehabilitation course in sanatorium treatment and led to an increase in the efficiency and quality of treatment for each patient. Using the method of music therapy, emotional and psychological correction was carried out, which occupies an important place in the relief of headache in pediatric patients.

Complex Use of B-Luron Supplement in Knee Osteoarthrosis: a Prospective Randomized Study B-Luron补充剂在膝关节骨关节病中的复杂应用:一项前瞻性随机研究

INTRODUCTION. Knee osteoarthrosis (KO) among the adult population often leads to permanent disability, a sharp decrease in the quality of life, and chronic use of analgesics. At the same time, according to the clinical guidelines of the Russian Rheumatology Association for osteoarthrosis treatment, hyaluronic acid medications are included in the list recommended for KO, and they are particularly effective at stages I-II of the disease.AIM. Evaluation of the efficiency of the dietary supplement B-Luron for KO patients within 12 weeks therapy.MATERIAL AND METHODS. A total of 70 patients with stage I-II deforming osteoarthrosis (DOA) were followed up, consecutively included in the study from December 14, 2021 to January 31, 2022, for an average of 90 days (3 months).RESULTS AND DISCUSSION. The efficiency of B-Luron for I-II DOA stages was shown: a statistically significant decrease in the level of pain in knee joints, an improvement in the function of the joint were found compared to the initial state. The analysis of objective data showed a positive trend in ultrasound parameters and MRI features of joint inflammation and a decrease in the need for analgesics within 12 weeks.CONCLUSION. In a prospective randomized clinical trial with 70 knee DOA patients as a part of the combination therapy (12 weeks use) with non-steroidal anti-inflammatory drugs, B-Luron showed its efficiency for stages I-II of the disease; there was a positive trend in ultrasound parameters and MRI signs of inflammation in the joints as well as a decrease in the need for non-steroidal anti-inflammatory drug use.

The Use of Terrain Cure in the Treatment and Rehabilitation of Patients with Cardiovascular Diseases: a Review 地形疗法在心血管疾病患者治疗与康复中的应用综述

INTRODUCTION. The review is devoted to the study of one of the modern complex methods of treatment and rehabilitation of patients with cardiovascular diseases – terrain cure which includes elements of therapeutic physical training, climatotherapy and provides for dosed physical activity in the form of hiking in rough terrain, climbing in mountainous areas on specific, marked routes under the supervision of a doctor. It is a method of exercise therapy that improves endurance, cardiovascular, respiratory and nervous system function, metabolism, as well as the muscles of the limbs and trunk.AIM. To highlight the mechanisms of the complex action of terrain cure, combining elements of climatotherapy and metered physical activity for the most optimal use in patients with pathology of the cardiovascular system, including those with excess body weight.MATERIAL AND METHODS. The analysis of publications in the electronic databases PubMed and the Russian Scientific Citation Index by keywords: terrain cure , metered physical activity, climatotherapy, rehabilitation, cardiovascular diseases. The date of the last search is August 9, 2022.CONCLUSION. The world and domestic scientific and practical experience of the use of terrain cure testifies to its high efficiency and an increase in the adaptive and compensatory potential of the patient's body, the safety of using this method in both preventive and therapeutic and rehabilitation programs, due to the main mechanisms of its rehabilitative action. Taking into account its wide availability, cost-effectiveness and additional therapeutic effects when in contact with the terrain, it seems advisable to expand the introduction of the terrain cure and use it for preventive purposes not only in sanatorium–resort conditions, but also in the urban environment – parks and other recreation areas of citizens.

Current Management Strategy for Osteoarthritis Patients: a Review 当前骨关节炎患者的治疗策略综述

A strategic approach is crucial to the management of patients with osteoarthritis (OA). It should be based on the current understanding of the pathogenesis of OA as an inflammatory disease. A review of current clinical guidelines for the management of patients with OA shows significant differences in the evaluation of pharmacological approach, especially the place of symptomatic slow-acting drugs for osteoarthritis, (SYSADOA) and a certain consensus in relation to non-pharmacological methods (primarily exercise, patient education, body weight control, various physiotherapy methods, orthotics and massage/manual therapy). It should also be taken into account the international “treat to target” recommendations, the main idea of which is careful regular monitoring of the patient’s condition and adaptation of treatment tactics depending on the response to treatment. Based on the analysis of literature data and their own clinical experience, the authors developed an algorithm for the strategy of complex therapy of OA, including the following steps: 1) pain control (2-4 weeks), 2) inflammation control (4-6 weeks); 3) control over cartilage degradation (6 weeks – 12 months). For each stage, a specific combination of systemic pharmacotherapy (non-steroidal anti-inflammatory drugs, SYSADOA), intra-articular administration of glucocorticoids and hyaluronic acid, exercises and magnetotherapy (pulsed electromagnetic field) was proposed. A staged comprehensive strategy for the treatment of patients with OA should help to achieve control over the symptoms, while minimizing the duration of NSAIDs and avoiding polypharmacy, and further achievement of inhibition of structural progression.

Motoric Automatisms of the Maxillofacial Region: a Lecture 颌面区域的运动自动性:讲座

INTRODUCTION. Effective kinesitherapy of peripheral and central paralysis of the mimic, masticatory, lingual and pharyngeal muscles requires in-depth study of the features of the cooperative work of these muscles in the norm and in pathology.AIM. To acquaint rehabilitation specialists with motor automatisms of the brachiofacial area and show the principles of their practical use.MATERIAL AND METHODS. We studied more than 70 literature sources on involuntary activity of the maxillofacial region and adjacent regions muscles as well as we summarized our own experience in rehabilitation of patients with peripheral facial neuropathies and with bulbar and pseudobulbar syndromesRESULTS AND DISCUSSION. The study showed that the face, neck, and arm are involved in various mesencephalic-bulbar automatisms, both normal (ontogenetic and psychosomatic synkinesia, postural-ocular and postural-mandibular reflexes) and pathological (trunk eye-facial synkinesia, oral automatic reflexes), much more closely than other body parts. All of them, with the exception of facial hyperkinesias, may serve as a basis for optimizing kinesitherapy methods for movement disorders of the brachiofacial region.CONCLUSION. The face, neck, and hand are closely involved in a variety of stem and subcortical automatisms. The better the physical therapist is familiar with the features of the existing or potentially available to the patient reflex activity in the part of the body being rehabilitated, the more effective the rehabilitation treatment will be.The face, neck and arm are closely involved in a variety of stem and subcortical automorphisms. Drinking Mineral Water and Low-Intensity Electromagnetic Radiation Combinational Effect on Rat Testes in Metabolic Syndrome: а Randomized Controlled Study 饮用矿泉水和低强度电磁辐射对代谢综合征大鼠睾丸影响的随机对照研究

INTRODUCTION. To correct disorders in the male reproductive system organs in the metabolic syndrome, it seems reasonable to use a combined effect of natural and artificial therapeutic physical factors – drinking mineral water and low-intensity electromagnetic interference of ultrahigh frequency – on an experimental model of this disease. These factors have antioxidant, cytoprotective and regenerative effects and can have a wide range of effects on various adaptation and protective regulatory mechanisms.AIM. To reveal the development of metabolic and structural adaptation-protective reactions in rat testes under the combined effect of drinking sulphate mineral water and low-intensity electromagnetic radiation microwave in experimental simulation of metabolic syndrome.MATERIAL AND METHODS. The experiments were performed on 26 nonlinear male rats weighing 180-200g. The metabolic syndrome model was reproduced for 60 days using a high-caloric diet. The rats were divided into 3 groups: 1st (experimental)-rats received drinking sulfate mineral water in combination with microwave electromagnetic radiation against the background of a high-calorie diet; 2nd (control) – rats received only a high-calorie diet; 3rd (intact) – rats without any influences. Research methods: biochemical, lightoptical, morphometric, electron microscopy. Significance of differences was assessed by Student's t-test.RESULTS AND DISCUSSION. It was found that the combinational effect of drinking sulfate mineral water and low-intensity electromagnetic radiation microwave contributed to the enhancement of a number of adaptive and protective reactions in the testes of rats against the background of a high-caloric diet. They were mainly manifested in the activation of antioxidant protection and protein synthesis, improvement of spermatogenic cells differentiation processes and increase in their number, enhancement of cellular and intracellular regeneration.CONCLUSION. The data obtained can be used in the development of new approaches to the methods of adaptation-protective and compensatory-restorative processes enhancement in the organs of the male reproductive system in metabolic syndrome.

Elderly People Physical Rehabilitation Personalization: a Prospective Comparative Study of 198 Patients with Sarcopenia 老年人肢体康复个性化:对198例肌肉减少症患者的前瞻性比较研究

AIM. To develop a methodology for the physical rehabilitation of elderly men with sarcopenia taking into account individual characteristics and somatotype.MATERIAL AND METHODS. A comprehensive anatomical-anthropological and instrumental (bioimpedance analysis) examination of 198 men aged 61 to 75 years (68±3.27 years) suffering from sarcopenia was carried out. The patients were divided into two groups depending on the method of physical rehabilitation. The duration of the rehabilitation programs was 12 weeks. The obtained results were compared with the indices of medical and biological state of 37 elderly men without sarcopenia signs (Group III). Somatotyping of all study participants was carried out according to the classification proposed by V.M. Chernorutsky.RESULTS AND DISCUSSION. Normosthenic patients predominated (group I – 46%, group II – 47%, group III – 67%), astenics in groups I and II amounted to 27.5%, in group III – 25%. The number of hypersthenic in group III was 8%, that was 17% and 19% less than in groups I and II, respectively. After 12 weeks no significant differences between groups II and III in the main indices had been revealed. The body mass index of men of group I was 1.04 and 1.07 times lower than in groups II and III, respectively. The difference in carpal strength between groups I and III was 4.9±1.34 kg (15.41%; P < 0.05), while the difference in that index between groups II and III (1.88 kg or 5.9%) were reliably insignificant (P > 0.05).The complete absence of signs of disease was found in 29 patients (32.5%) in group II and in 16 patients (14.7%) in group I, thedifference between the groups being 17.9%.CONCLUSION. Sarcopenia is characteristic of hypersthenic males. Somatotyping is an accessible rapid diagnostic tool that allows us to identify groups of patients prone to sarcopenia.

Rehabilitation Programs Effectiveness Model: a Retrospective Comparative Study of Patients with Post-Acute COVID-19 Syndrome 康复项目效果模型:急性后COVID-19综合征患者的回顾性比较研究

AIM. To evaluate the expected effectiveness of post-COVID rehabilitation strategies.MATERIAL AND METHODS. The study was conducted August, 2021 – March, 2022 in the republican and federal rehabilitation centers: Krasnodar Territory (n=25), the Republic of Buryatia (n=25), and Moscow (n= 25). The data were obtained by a retrospective analysis of medical records and by interviewing the study participants who had an indication or had been treated in a rehabilitation centre for post-acute COVID-19 syndrome (U 09.9). The average length of time after the onset of the acute period of the disease was 6.5 ± 2.5 months. A prognostic model was constructed to estimate the expected effectiveness of the rehabilitation programme, as measured by a reduction in the severity of dyspnea. Nonlinear maximum likelihood logit regression was used to build the model.RESULTS AND DISCUSSION. According to the presented model, the best expected rehabilitation effectiveness was found in the National Medical Research Center (Moscow) (β=-1.788685, p=0.009964604), a slightly lower expected rehabilitation effectiveness wasobserved in the resort with a curative climate (Krasnodar Territory) (β=0.9913501, p=0.182944), the lowest expected effectiveness was registered in the regional rehabilitation center (the Republic of Buryatia) (β=1.054594 p=0.2642723). The need for an integratedapproach in the treatment of patients with post-acute COVID-19 syndrome was pointed out by many domestic and foreign researchers, however, the choice and content of treatment strategies is debatable. Our study provides a preliminary answer to this question.CONCLUSION. The developed model of the expected effectiveness of the rehabilitation of patients suffering from shortness of breath after undergoing COVID-19 has a specificity of 54.54%, sensitivity of 91.3% and an overall accuracy of 84.21%. Zero-Gravity Robotic-Assisted Locomotion Simulator in Rehabilitation: a Prospective Randomized Clinical Study of 30 Spinal Trauma Sequelae Patients 零重力机器人辅助运动模拟器在康复中的应用:30例脊柱创伤后遗症患者的前瞻性随机临床研究

INTRODUCTION. According to the WHO, between 250,000 and 500,000 people are affected every year with spinal cord injury (SCI) around the world. The number of disabled people as a result of spinal cord injury in Russia is estimated to be over 250,000 and is increasing due to injury rate growth and survival rate improvement of patients in the acute and long-term periods of traumatic spinal cord injury (TSCI).AIM. To justify the efficacious and safe use of a zero-gravity robotic simulator for a locomotor therapy in the complex rehabilitation of patients with SCI and compare this therapeutic approach with conventional motor rehabilitation programs using other robotic mechanotherapy techniques.MATERIAL AND METHODS. Neurological and functional disorders were analyzed in 30 patients with SCI on the basis of clinical examination and electromyography (EMG) findings. All the patients were classified into an intervention group and a control group by a sequential randomization. The rehabilitation programs for the intervention and control groups were alike, except for robotic mechanotherapy.RESULTS. A significant positive dynamic change in motor functions according to the ASIA Impairment scale was noted in the intervention group, in which 2 patients moved up to a higher level. The EMG data showed a significant difference between the groups in favor of the intervention one. A significant difference in postural function improvement was found between the intervention and control groups in a seated position. The rehabilitation of patients from the intervention group engaged the axial muscles, promoting an increase in strength and better control of the trunk muscles. A significant spasticity decrease and changes in the functional status of the patients were observed in both groups. The patients’ ability to perform normal daily activities was registered to improve.CONCLUSION. The study demonstrated that robotic musculoskeletal training in zero-gravity conditions is safe and effective in complex rehabilitation of patients with SCI and improves motor skills, self-care and postural function. A significant correlation was found between neurological and functional changes, indicative of a restorative concept of the new therapeutic modality. Further studies with increased capacity are reasonable.
