Othello syndrome and dopamine dysregulation syndrome 奥赛罗综合征和多巴胺调节失调综合征

Othello syndrome is a paranoid delusional jealousy characterised by false absolute certainty of the infidelity of a partner, leading to preoccupation with a partner's sexual unfaithfulness based on unfounded evidence. Here, we present a case of Othello syndrome comorbid to dopamine dsyregulation syndrome in a patient suffering from juvenile Parkinson's disease. The case investigates the relationship between compulsive levodopa use and delusional jealousy, the serious consequences of these conditions and the difficulty in patient management.

Observing the relationship between COVID‐19 vaccination and the neurological side‐effect of worsening seizure frequency, this case series offers insight into the impact of vaccine administration on seizure frequency in three individuals with intellectual disabilities and epilepsy. With questions still remaining with regard to vaccine safety, this article provides an example of the unique challenges faced by this patient population. It is hoped that the publication will stimulate further investigation into related issues and will also help to further inform vaccination strategies.

Lithium is licensed for the treatment and prophylaxis of mania, bipolar disorder, recurrent depression and intentional self‐harming behaviour. Despite its effectiveness, lithium use is complicated by its narrow therapeutic range. In 2009, the National Patient Safety Agency issued a safety alert highlighting the issues and requested NHS organisations ensure reliable systems and monitoring were in place. This article explores how well the guidelines are followed in terms of lithium prescribing and monitoring across primary and secondary care.

Transforming care planning: linking risk assessment and risk management 改革护理规划:将风险评估与风险管理联系起来

It is well established that a good risk assessment is essential for safe care planning in psychiatric inpatient units. However, the current practice, at best, is inconsistent and can fluctuate between being unsafe and being over‐restrictive. Here the authors describe a fresh approach to the process of risk‐informed care planning and how this improved the safety of their patients’ care while making it less restrictive.

There is a limited but important role for pharmacological interventions (psychotropics) in the management of challenging behaviours in patients with intellectual disability. Due to limited number of psychotropics licensed for the treatment or management of challenging behaviours, this case series will further expand the horizon of research to explore the role of pregabalin in the treatment or management of complex patients with intellectual disability presenting with severe challenging behaviours.

The authors describe an unusual case of a patient diagnosed with this condition in addition to paranoid schizophrenia, in a forensic low secure unit. The focus of the article is on the distinctiveness of the presentation, the heterogeneity of symptoms and difficulties encountered in the diagnostic formulation process.

Amyloid angiopathy‐related inflammation: a case report 淀粉样血管病相关炎症:病例报告

Cerebral amyloid angiopathy‐related inflammation (CAARI) represents a rare, more aggressive, yet clinically and radiologically distinct manifestation of cerebral amyloid angiopathy. Here, the authors describe a case of a patient in their 80s with dementia who presented with a loss of consciousness event likely related to a seizure. The MRI of the brain showed amyloid angiopathy with surrounding inflammation. This case illuminates CAARI as a distinct pathology, how to approach it as a differential diagnosis, and crucially, how to treat it.

Excessive polypharmacy in the elderly carries a high burden of drug‐related morbidity and contributes negatively to the environmental impact of health care systems. The aim of the project was to evaluate the outcomes of implementing the STOPPFrail tool within a mental health service for older people (MHSOP) inpatient setting in a population of patients with dementia. The authors demonstrate that the STOPPFrail guidelines can be implemented to help to overcome barriers to deprescribing.

Mood disorder is a rare complication of acoustic neuroma resection, and therefore unlikely to be listed as a possible risk on a preoperative consent form. However, the occurrence of this has been recognised since as far back as 1914. This case study reports the new presentation of mania in a 65‐year‐old woman, seven weeks after surgical removal of an acoustic neuroma. The case study explores whether the acoustic neuroma and its surgical management could be directly causative of her psychiatric disorder, or whether it simply represents a psychological trigger in a susceptible patient.
