Clozapine and aripiprazole‐induced neutropenia 氯氮平和阿立哌唑诱导的中性粒细胞减少症

Neutropenia is a well‐known adverse side‐effect of clozapine that can also be seen with other atypical antipsychotics. In this article the authors discuss a patient who experienced neutropenia with clozapine and who subsequently redeveloped neutropenia with aripiprazole after a short trial. The case illustrates that close monitoring and caution is advisable when trialing patients on any further antipsychotics after a neutropenic event with clozapine.

Psychiatric comorbidities in epilepsy patients: results of a Tunisian study 癫痫患者的精神合并症:突尼斯研究的结果

Psychiatric comorbidities are underestimated in epilepsy. Here the authors assess the prevalence of psychiatric comorbidities in a cohort of patients with epilepsy in an outpatient neurology unit in Habib Bourguiba Hospital, Tunisia, and identify the sociodemographic and clinical factors associated with the presence of these psychiatric disorders in this patient sample. The results highlight that particular attention should be paid to such patients as part of a global biopsychosocial approach.

Psychiatric comorbidities are common in epilepsy. Common conditions such as depression and anxiety, and neurodevelopmental conditions ADHD and autism are also more prevalent in people with epilepsy. This article discusses a girl with a background of controlled epilepsy who was referred to the paediatric psychiatry clinic. It demonstrates a clear need for joint working between neurologists and psychiatric services to optimise the quality of life of such individuals.

Despite the panoply of clinical assessment and neuroimaging modalities at the disposal of clinicians today, the diagnosis of neurodegenerative disorders may still prove challenging, particularly in the early stages, because of the subtlety of early changes and/or their overlap with (so‐called) psychiatric disorders. A case illustrating these difficulties is presented, in which long‐term follow‐up proved to be the most important investigation to establish the correct diagnosis.

Blood‐based biomarkers for identifying Alzheimer's disease (AD) pathology have been of considerable interest over the past decade and can offer a significant advantage over the currently available and validated cerebrospinal fluid and positron emission tomography biomarkers in terms of cost, invasiveness and accessibility. This review explores the role of three blood‐based biomarkers; plasma amyloid β, tau and neurofilament light chain, and their combination, in the identification of AD pathology and how they may be utilised in clinical practice.

Cluster headache is a potentially debilitating condition that can lead to restrictions in occupational and day‐to‐day function. Here, the authors outline a case of a significant reduction in headache frequency in a patient who had an incomplete response to NICE‐recommended treatments, and was treated for attention deficit hyperactivity disorder with stimulant medications. This article builds on a small number of case reports indicating a potential role for stimulant medications such as methylphenidate in the management of cluster headache.

Caring responsibilities for children and family members with complex health care needs often go unnoticed among health care staff. Statistically, a significant portion of these responsibilities are met by women, widening the gender pay gap. This article highlights the impact of caring responsibilities on physicians and the need for more inclusive work and leave policies to support recruitment and retention of this workforce, valuing both professional and lived experience in care giving.

Here, the authors discuss a patient with mild intellectual disability and Down's syndrome who appeared to experience deterioration in her well controlled thyroid function twice, secondary to higher dosages of risperidone for treatment of her delusional disorder. The case therefore suggests that thyroid function monitoring may be of value in individuals predisposed to thyroid dysfunction who are commenced on antipsychotic therapy, especially risperidone.
