Sexual disinhibition in dementia is a distressing and embarrassing behaviour. Currently, there are no guideline‐directed management aims to address this behaviour in patients with dementia. This case report highlights a combined non‐pharmacological and pharmacological approach for managing dementia in a patient who showed marked sexual disinhibition.

Clozapine is licensed for treatment‐resistant schizophrenia. Clozapine dosage is normally adjusted according to each patient's clinical response and the side‐effect profile, but caution is advocated when using clozapine, due to its association with potentially life‐threatening side‐effects. One such adverse reaction is the risk of seizures. There is much debate as to how to prevent clozapine‐induced seizures: some authors recommend primary and some, secondary prophylaxis with antiseizure drugs. This article highlights an interesting case study and explores various factors that must be considered before a decision is made to prescribe anticonvulsant medication.

The Distress Thermometer (DT) has been validated across various medical conditions, but no studies have investigated its performance in people with Parkinson's disease (PD). In this study, the authors aimed to determine if the DT is valid and reliable in a sample of participants with PD recruited from a movement disorder clinic. Use of a tool such as the DT allows staff to identify and address the most distressing symptoms and refer patients on for specialist input when needed.

Filicide‐suicide is the killing of a child older than 12 months, followed by parental complete or incomplete suicide. In most cases, the motive of maternal filicide‐suicide is altruistic. This article reports a case of twin filicide by a depressed mother and also highlights the risk of filicide in depressed multiple birth older mothers.

The COVID‐19 pandemic has numerous implications for the mental health of the world's population; the perinatal population are particularly at risk of mental ill health during this time. This review presents current evidence surrounding the impact of the pandemic on perinatal mental health and lessons for clinical practice both during and beyond the current pandemic.

People with intellectual disability (ID) are much more vulnerable to psychiatric illnesses than the general population and as such the pandemic has most likely affected their mental health. The mental health of patients with ID has been affected due to factors such as reduction in staffing levels due to illness, day services and respite care being closed, restrictions to visits and to going out to public places. This case series explores the mental health implications that contracting COVID‐19 had on five patients with ID attending an intellectual disability psychiatry service and reflects upon key messages for clinical practice.

Patients with Huntington's disease are a risk group for developing problematic gambling, which is characterised by subjects’ inability to stop gambling despite financial, personal or professional problems. Predisposition to pathological gambling and other addictions involves disturbances in the same corticostriatal circuits that are affected in Huntington's disease, and displays similar disinhibition‐related symptoms. The authors present a case of long‐standing pathological gambling, impulsivity and organic mood disorder related to Huntington's disease.

The COVID‐19 pandemic presented unprecedented challenges that required a well‐coordinated response across central and local government, health services, and non‐government sectors. This study compared characteristics, presentation and treatment of care home patients referred to the care home pathway team during three lockdowns between March 2020 and March 2021 in the Guildford and Waverley Home Pathway. Despite availability of mental health services, the findings indicate a reduction in access to mental health services as well as physical health investigations for elderly residents in care homes during lock down 1 compared with the subsequent lockdowns.
