Material and Method: One hundred twentyfive RA patients and 105 healthy controls were enrolled in the study. Patients were assessed and PLR, NLR, and DAS28, RAQoL values were calculated. Disease activity was calculated by DAS28 and quality of life was examined by RAQoL, HAQ (Health Assessment Questionnaire). Results: NLR (2,4 ± 1,9) and PLR (155,2 ± 82,3) value was significantly higher (p < 0.05) in RA. Positive correlation was observed between the two groups between DAS 28 with age, morning stiffness, VAS pain assessment, RF, ESR, CRP,NLR , PLR and swollen and tender joint number (p < 0.05) Significant negative correlation was observed between DAS 28 and hemoglobin, RBC, lymphocyte, monocyte % value (p < 0.05). There was no significant correlation between DAS28 and RAQoL, HAQ (p>0,05). Discussion: This study presents that NLR and PLR are associated with disease activity in patients with RA. NLR and PLR values can be assessed as an additional inflammatory marker in patients with RA.

Use of qSOFA criteria in prediction of poor prognosis in acute pancreatitis qSOFA标准在预测急性胰腺炎不良预后中的应用

Material and Method: The data of 98 patients with acute pancreatitis diagnosis which was verified by the computerized abdominal tomography in the university hospital emergency department between 2013 and 2018 were used in our study. that was designed as monocenteric retrospective study. The SOFA and qSOFA criteria of the patients were calculated during the application to the emergency department. Results: Fifty-three percent (n=52) of the patients were female. Their age averages were 57.5±18.57/year. Forty-eight (%49.0) patients were detected to have any of the poor prognostic criteria. It was also detected that the qSOFA score of 17 (517.3) patients were 1 and above and the qSOFA score of 81 (582.7) patients was 0 during the application to the emergency department. The SOFA (p<0.001) and qSOFA (p=0.004) values were significantly different for the patients having the good prognosis and poor prognosis. When we take the Cut-off as > 0 for the qSOFA criteria, we determined the AUC as 0.641, Sensitivity as %39.58, Specificity as %86, and CI% as 0.538 to 0.735, and p=0.001, respectively. Discussion: The SOFA and qSOFA criteria used in order to predict the organ failure in the sepsis can be a scoring system that will be able to be used in predicting the poor prognosis in acute pancreatitis in the emergency department.

The results of arthroscopic treatment for talus osteochondral lesions 距骨软骨病变的关节镜治疗效果

Material and Method: The patients with talus osteochondritis dissecans weretreated by arthroscopic debridement; and bone marrow stimulation was performed with Kwire. Twenty-sixof them formed the primary and 15 of them formed the revision group. Clinical assessment included the Ogilvie-Harris score and American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot score. The defects were classified according to the system of Berndt and Harty. The lesion side, symptom time, trauma history, age-related complication, Ogilvie-Harris and AOFAS scores, follow-up period, and well-being condition of the groups were compared. Results: A total of 41 patients, 25 males and 16 females, with a mean age of 34.3 years (range 19-62), were included in the present study. Follow-up ranged from 23-73 month (mean 34,3). The individual analysis results showed that the pre-operative AOFAS scores were significantly different between the primary and the revision groups, while post-operative scores were not significantly different between the primary and therevision groups. For the whole group, the AOFAS skore was excellent in 9 patients, good in 25, fair in 6 and poor in one. In the primary group the AOFAS scores were good in 14 (54%), fair in four (15) and excellent in eight (31) patients, while in the revision group they were good in eleven (73%), fair in two (13%) and poor in one (7%) patient. The combined good and excellent scores were 85% for the primary group and 80% for the revision group. The mean AOFAS AHS score at the 12th month was 88.2 in the primary group and 82.8 in the revision group. Discussion: Arthroscopic curettage and bone marrow stimulating process is a viable method and recommended for both primary and revision treatment of an osteochonditis dissecans of the talus.

Our study aimed to identify the HCV genotype distribution in patients infected with HCV attending our hospital in Ordu Province in the Central Black Sea region. Material and Method: The study included a total of 196 patients whose samples were sent to our laboratory for HCV viral load measurements from January 2016 to May 2018 with HCV-RNA positivity identified. Genotype definition was completed with the real-time PCR method. Results: It was observed that 4 patients (2%) had genotype 1, 7 patients (3.6%) had genotype 1a, 179 patients (91.4%) had genotype 1b, 5 patients (2.5%) had genotype 3 and 1 patient (0.5%) had genotype 4. Discussion: The most common genotype observed among patients attending our hospital was genotype 1. Genotypes 3 and 4 were rarely observed in comparison to Turkish data, similar to other provinces in regions receiving little migration. As medication resistance and treatment failure is high for genotype 1, the risk of encountering treatment failure continues to be a serious problem in our region.
