Impairment of lipid metabolism is also a little-known problem. We aimed to investigate hormone levels and lipid metabolism in children with acute brucellosis. Material and Method: Forty-six children with acute brucellosis, who had never been treated for the disease, and 39 healthy children were prospectively investigated. They were compared in terms of liver functions, serum lipid profiles, and hormone levels. Results: The patients had higher levels of AST, ALT, and GGT (p=0.04, p=0.01, and p=0.01, respectively). The patients also had higher cholesterol and triglyceride, and lower LDL and HDL levels than healthy children. The difference between triglyceride levels was statistically significant (p=0.04). Serum hormone levels were similar between groups. With antibiotic treatment and lifestyle modifications, all abnormal laboratory parameters returned to normal. Discussion: Children with acute brucellosis should be monitored for dyslipidemia, which can be relieved with antibiotics and lifestyle changes without using lipid lowering agents. Investigation of hormone levels in these patients is necessary only if signs and symptoms suggest an endocrine abnormality.

Elderly prisoners are more likely to get ill in comparison with their non-prisoner peers because of inadequate access to health care and medicines, and they often have referrals to emergency departments. This study evaluates the elderly inmate emergency department admissions in Turkey. Material and Method: A retrospective cross-sectional descriptive study was conducted in the emergency department of a tertiary healthcare facility with approximately 210,000 admissions annually. Inmate patients ≥65 years of age were included. The 140 total patients were categorized as Group 1 (65-84 years) or Group 2 (>85 years). Emergency referrals were grouped as surgical, medical, psychiatric disorders, and injury. Results: Correlation between categorical variables was analysed with Pearson chi-square test, and the effect of each factor on variables dependent on multiple factors was tested using logistic regression analysis. A total of 140 elderly inmates were eligible for the study. There was the highest number of patients in the medical group (n=98, 70%), and the least in the injury and psychiatric groups (n=4, 2.8%). Cardiovascular system disorders were the leading referral cause. Males had a greater hospitalization rate (p=0.033) and females had no injuries. Recurrence was higher in Group 2 (p=0.007). 92 patients (67.6%) were discharged from emergency department. The mortality rate was 2% (n=17). Discussion: The number of elderly inmates and thus the corresponding rates of acute care in the inmate population are gradually increasing. Already-overburdened EDs are usually the first referral units for all health problems of this vulnerable population in Turkey. Therefore, prisons should provide critical healthcare on-site for their medically vulnerable elderly population.

The aim of this study was to assess whether CTAR correlates with the left ventricular ejection fraction (EF) better than CTR in healthy individuals and patients with congestive heart failure. Material and Method: The study included 156 healthy individuals and 98 heart failure patients undergoing echocardiography and digital chest radiography. The CTR was calculated in the traditional manner, and the CTAR was taken as the ratio between the pixel counts of the cardiac area and whole thoracic area. Results: The traditional CTR showed an inverse correlation with ejection fraction in both healthy individuals (r = -0.13) and heart failure patients (r = -0.19). The CTAR showed an improved correlation (r = -0.32 for healthy individuals and r = -0.37 for heart failure patients). If it is assumed that an ejection fraction of 55% or more indicates normal cardiac function, a value of 50% or less for CTAR can be taken as normal. Discussion: Digital chest radiography is widely used as a popular technique for diagnosing various thoracic and cardiac diseases. Cardiomegaly is associated with an adverse outcome in patients with heart disease. Therefore, evaluation of heart size is an important clinical variable, and changes in heart size may be used to monitor and grade cardiac disease severity. Conclusion: Our findings suggest that the CTAR correlates better with cardiac function as assessed by the ejection fraction than the traditional CTR.

Discussion: Digital chest radiography is widely used as a popular technique for diagnosing various thoracic and cardiac diseases. Cardiomegaly is associated with an adverse outcome in patients with heart disease. Therefore, evaluation of heart size is an important clinical variable, and changes in heart size may be used to monitor and grade cardiac disease severity. Additionally, they are amongst the most common inherited disorders in Turkey and can be preventable with the help of premarital screening programs. The aim of the present study was to analyse the hemoglobinopathy test results in individuals who applied to the hemoglobinopathy diagnosis centre in Bilecik/Turkey. Material and Method: In our study, hemoglobinopathy test results from January 2006 to April 2017 in Bilecik Hemoglobinopathy Diagnostic Centre were evaluated retrospectively. Blood samples from individuals who applied for pre-marriage counselling and general screening were collected and hemoglobinopathies (Hb A0, A1C, A2, F, C, D, E, and S) were screened by using High Performance Liquid Chromatography (HPLC). Results: A total of 23,676 people were included in the present study. Beta-thalassemia trait was detected in 1,317 (5.5%) and, HbA2 at a level over 3.5% was detected in 1,177 (5.0%). Additionally, elevated HbF levels were over 2% of individuals (324/23,676). The positivity of hemoglobin D and HbS traits were 0.06% (15 patients) and 0.01% (two patients) respectively. Discussion: In the present study the positive rate of HbA2 was 4.6%. When comparedThis finding was higher than the overall average rate in Turkey. The rate is similar to those in studies from Istanbul and the Mediterranean region, but it is higher than in studies from Bursa and Kocaeli. Our results indicated that premarital hemoglobinopathy screening should be continued for all partners planning to get married and for each individual in Bilecik who may potentially have a hemoglobinopathy trait.

The aim of our study is to determine the bacterial growth and antibiotic resistance profiles of bacterias isolated from endotracheal aspirate cultures (ETA) obtained from ICU’s of our hospital for the last one year. Material and Method: Between October 2017 and September 2018, ETA samples from adult intensive care units were examined retrospectively. In addition to conventional methods, identification and antibiotic susceptibilities were studied in accordance with the recommendations of the European Committee on Antimicrobial Susceptibility Testing (EUCAST) using fully automated VITEC (Biomerieux, France). Results: Of the 205 ETA specimens from adult ICU’s, 113 growths were detected in 103 patients. In ETA, 37 (32,7%) Acinetobacter baumannii, 29 (25,6%) Pseudomonas aeruginosa, 13 (11.5%) Staphylococcus aureus, 12 (%10,6) Klebsiella pneumoniae, 6 (5,3%) Escherichia coli, 4 (3,5%) Enterobacter cloacae and 2 (1,7%) Stenotrophomonas malthophiliae was isolated. Meropenem and imipenem resistance of A. baumannii was 89.1% for both, whereas it was 48,2% and 51,7% for P. auroginosa, respectively. Colistin and tigesiklin resistance was not detected for all isolates. Oxacillin resistant S.aureus strains were determined as 46.1% (6/13), while linezolid, teicoplanin, and vancomycin resistance were not detected. Discussion: Increased carbapenem resistance observed in antimicrobial susceptibility tests, for the most frequently bacterias isolated from ETA samples in ICU of our hospital, has shown the importance of the antimicrobial susceptibility testing.
