Portrait of Ms. Diaz: Empirical study of patient journey mapping instruction for medical professional students 迪亚兹女士肖像:医学专业学生病人旅程绘图教学的实证研究

An interdisciplinary team of educators from medicine, design, and informatics piloted an online journey map (JM) exercise targeting 48 medical students and physicians assistants students. The JM exercise was designed to teach about patient empathy skills, person-centred care, and the socio-ecologic determinants of health. Prior to the exercise, the students were given a sample patient archetype introducing Ms. Diaz, a person with diabetes visiting a virtual clinic. Students worked in small groups to create a JM from Ms. Diaz’ perspective about, and experiences with, a telemedicine clinic. Our preliminary qualitative analysis of the JMs from the exercise showed that learners were able to create JMs that included all key sections including process phases, user perceptions, pain points, and design opportunities. Almost half of the responses focused upon socio-cultural and socio-technical issues as opposed to strictly clinical concerns. We believe this pilot shows the potential for journey maps to be used in health professional education to empathize with patients, identify societal problems in healthcare delivery, and design responsive solutions. Furthermore, the virtual classroom format highlights the scalability and extensibility of this strategy to a broad range of educational goals.

Initial steps to measurement and improvement of family-centered communication during the pediatric patient journey of bone marrow transplantation 在儿科患者骨髓移植过程中测量和改善以家庭为中心的沟通的初步步骤

Often, pediatric patients' caregivers feel like they are not being heard or consulted by the healthcare system they have entrusted for their loved ones’ care. These difficulties are well known to the healthcare system, and significant research has been conducted to understand how to provide what’s come to be known as patient- and family-centered care (PFCC). PFCC is grounded in mutually beneficial partnerships among health care providers, patients, and families. In 2019 we started a quality improvement initiative, partnering with families to increase our family-centeredness, initially focusing on communication. We report our quality improvement initiative's initial steps to understand communication between patients and caregivers in the inpatient setting. We report variables identified as barriers to PFCC, and our initial interventions, including small tests of change, to implement and improve PFCC in the inpatient setting. We hope that our experience will inspire others to undertake similar initiatives at their institutions.

The current state of knowledge on mobile health interventions for opioid related harm: Integrating scoping review findings with the patient journey 阿片类药物相关伤害的流动卫生干预措施的现状:将范围审查结果与患者旅程相结合

Opioid-related harm has become a major public health crisis around the world. There is a paucity of literature that examines the state of mHealth technologies in relation to the prevention and management of opioid-related harm. The purpose of this research is to examine the current state of knowledge with respect to mHealth technologies focused on opioid harm reduction and to identify gaps and technological opportunities. This research was conducted in two phases. The first phase involved the completion of a scoping review in six peer-reviewed research databases and grey literature searches in two search engines. The second phase involved the development of a Patient Journey Map to describe the findings of the scoping review in order to identify mHealth gaps and opportunities in relation to the recovery-oriented cascade of care. For the scoping review, nine articles met the inclusion criteria. These articles focused on accessibility, utilization, acceptability, feasibility and patient outcomes of mHealth interventions. These studies showed mHealth interventions are highly accessible, utilized and acceptable to opioid users, feasible to implement and can improve appointment adherence and patient outcomes. The Patient Journey Map demonstrates future mHealth interventions should focus on the prevention, diagnosis and post-recovery phases of the patient journey.

Patient journey mapping represents a new way of reasoning about continuity of care, reducing wait times and improving patient safety. Patient journey mapping allows users such as health professionals, patients and policy makers to identify technologies that can be used to support patient care. Patient journey maps allow one to visualize a patient’s journey and at the same time understand, where gaps in patient care exist. In this paper we discuss a novel approach to patient journey mapping for supporting reasoning and decision making about how technological tools could be integrated into a patient’s health journey. The approach allows for reasoning surrounding technologies in the patent’s digital ecosystem.

Patient journey mapping: Current practices, challenges and future opportunities in healthcare 患者旅程图:医疗保健领域的当前实践、挑战和未来机遇

Patient Journey Maps are an emerging concept that visually map each interactive touchpoint that the patient experiences as they navigate the care continuum. The purpose of this article is to: 1) Identify the ways that patient journey mapping has been used to identify efficiencies and inefficiencies in the healthcare process from a patient perspective, 2) Identify the type of approaches that have been documented to visually identify the patient journey, 3) Identify how information tools can be taken into account to improve gaps identified by patient journey mapping; and 4) Detail what patient journey visualization and mapping tools currently exist (and are used) in research and healthcare practice. A scoping review literature exploration, following the Arksey and O’Malley Framework (2005) was conducted in the PubMed database, with a focus on English publications only, using the search terms “patient journey map.” Two researchers iteratively assessed the articles based on inclusion and exclusion criteria; 30 articles were included in the study. A thematic analysis was conducted and the findings were tabulated in the data extraction table. The patient journey map has considerable promise but continues to be an underutilized resource in industry - further research and standardization is required to increase adoption in the healthcare setting.

Journey mapping methods have a number of practical uses. One of the most promising applications in the area of healthcare is to apply patient journey mapping to identify a patient’s pathway through their healthcare journey. Nowhere is this more important than in the area of cancer care. With lengthy wait times in many countries and the complexity of care paths that cancer patients travel, there is ample opportunity to identify both gaps in care as well as opportunities to improve care processes. In this article the authors discuss a case study of a patient journey involving multiple care organizations, several health professionals and care in both Canada and the United States. By applying patient journey mapping a simplified version of such complexity can be presented in a visual and succinct way, allowing health professionals and managers of healthcare organizations to identify where inefficiencies in care and patient safety issues occur. Furthermore, this mapping can form the basis for optimizing care processes and holds considerable promise for patient-centred healthcare. While extensive studies have been conducted to understand the factors that influence individual intentions to share knowledge, a review of the literature shows that these studies did not consider the effect of the individual sentiment of the knowledge sharer in the act of sharing knowledge. Individuals coming from different tribes might have sentiments against each other, which could influence their communication and social interaction and thus affect knowledge sharing. This study collected data from lecturers in ten polytechnic institutions in Ghana using a questionnaire and analyzed the data using the structural equation modelling (SEM) technique. The findings show that attitude, subjective norm, equal status, cooperativeness, identification, top management, and immediate superior have a significant influence on individuals’ intention to share knowledge with people from other tribes. Contrastingly, common goal, institutional support, and friendliness do not influence individuals’ attitude to share knowledge with people from other tribes. https://doi.org/10.34105/j.kmel.2020.12.017

Adaptation and validation of the measure of organizational citizenship behavior in collaborative learning 合作学习中组织公民行为测量的适应性和验证

Organizational citizenship behavior refers to “individual behavior that is discretionary, not directly or explicitly recognized by the formal reward system, and that in the aggregate promotes the effective functioning of the organization (Organ, 1988, p. 4)”. Previous research suggests organizational citizenship behavior can be a beneficial concept in a collaborative learning setting because it may affect learning outcomes, student satisfaction, and social loafing in organizations and teams. However, a measure of organizational citizenship behavior in the collaborative learning setting does not exist. This research aimed to adapt and validate the measurement of organizational citizenship behavior in a collaborative learning setting. 511 college students in Korea participated in this study. First, we conducted exploratory analysis, and as a result, four dimensions were extracted: altruism, conscientiousness, sportsmanship, and courtesy. Subsequently, we conducted confirmatory factor analysis to examine the stability of multidimensionality, and the result indicated the four-factor structure has a reasonable fit statistically. In addition, we examined convergent, discriminant and criterion validities. With this validated measurement, future studies can examine how organizational citizenship behavior in the collaborative learning is related to other learning constructs such as student engagement and academic achievement. https://doi.org/10.34105/j.kmel.2020.12.015

The purpose of this study is to build a model for Digital Content Management (DCM) in agricultural university libraries in Bangladesh. The model focuses on identifying the necessary tools and techniques required for DCM and finding out the problems and prospects of DCM for enhancing resource sharing among the agricultural universities in Bangladesh. To investigate these issues, surveys were used to collect data from 245 participants, who were students and teachers from seven public agricultural universities in Bangladesh. The findings reveal some hindrances to the establishment of DCM, such as lack of constant power supply, limited bandwidth speed, some users have lacking fundamental IT knowledge and shortage of digital resources, in addition to some suggestions for improving DCM in these libraries. Based on the survey results and related literature, the DCM model for agricultural university libraries of Bangladesh has been proposed and discussed. https://doi.org/10.34105/j.kmel.2020.12.019

Investigating the mediating role of organizational trust in the relationship between perceived organizational support and Knowledge sharing 组织信任在感知组织支持与知识共享关系中的中介作用研究

Knowledge sharing is vital for any organization, and it has an essential impact on developing ‎‎skills, increasing value, and maintaining a competitive advantage in the organization. In the knowledge management (KM) literature, ‎‎perceived organizational support and organizational trust have been pointed out as ‎‎primary factors for knowledge sharing. The present study investigated the role of ‎‎perceived organizational support in promoting nurses’ knowledge sharing, considering ‎‎organizational trust as a mediator‎. The cross-sectional study design was applied to examine the ‏hypothesized relationships.‏ ‎Data were ‎‎collected using three questionnaires from three hundred thirty ‎‎nurses working in five public hospitals in Shiraz city. The Partial Least Squares Structural ‎‎Equation Modeling (PLS-SEM) approach was used to analyze data. The results revealed a significant and positive association between perceived organizational support, organizational trust, and knowledge sharing. The findings also demonstrated that organizational trust plays a mediating role between perceived organizational support and knowledge sharing.
