Implementing the DSM-5 parent/guardian-rated level 1 cross-cutting symptom measure child 6−17 in a community mental health clinic 在社区心理健康诊所实施 DSM-5 家长/监护人评定的 6-17 岁儿童一级交叉症状测量法

Problem

Despite their relevance to assessment and clinical decision-making in behavioral health, measurement tools are still rarely used in clinical settings. With an increase in undiagnosed and untreated mental health problems in children and adolescents, a southeastern US Community Health System was the ideal setting for the implementation of a behavioral health assessment tool.

Methods

This quality improvement project was conducted to address the question: “Among mental health clinicians at the Community Health System, will the use of the DSM-5 parent/guardian-rated level 1 cross-cutting symptom measure 6−17 (CCSM) improve diagnosis and treatment choices of children with mental health conditions, over a 4-week period?” A convenience sample was recruited from among behavioral health counselors working at various out-patient clinics and school-based counseling sites in the Community Health System. After nurse-led training on use of the CCSM, including a PowerPoint intervention and individual coaching sessions, the five clinical participants were encouraged to use the CCSM screening tool on their patients presenting for mental health assessment. Knowledge of the tool was assessed using a pre- and post-training survey. Completed screening tools were collected from participants over the 4-week duration of the project.

Findings

A chart audit comparing the project period with the previous year showed an increase in diagnosis and treatment in 6−17 year-old patients at the outpatient clinic.

Conclusions

This project demonstrated the usefulness of the CCSM for behavioral health assessment in the child/adolescent population at the Community Health System and supported use of the intervention in other clinics.

A qualitative analysis of child and family complaints related to child mental health services 对与儿童心理健康服务相关的儿童和家庭投诉的定性分析

Problem

Little is known about issues of patient-reported problems, in particular within psychiatric services for children with mental ill health. Child and family complaints related to child mental health services can be analyzed and discussed in light of the universal human right to health and healthcare. Therefore, the aim of this study was to analyze child and family complaints related to child mental health services.

Methods

A retrospective, descriptive design was used. Child and family complaints were analyzed with a qualitative thematic analysis.

Findings

The findings were described in three themes describing lack of access to care, inadequate communication between services and children with mental ill health and their families, and lack of clarity of who is responsible for care, leading to neglect of children's needs. These issues place a huge responsibility on parents or relatives.

Conclusion

Better communication within mental health services, and better collaboration with other services such as school and social services, could limit children's suffering. Healthcare services with named professionals who specialize in child mental health and provide continuity in care, are required. General human rights principles should guide planning and care of children.

Problem

Adolescence is a period of biological, cognitive, psychosocial, and emotional development. Negative experiences of Coronavirus (COVID-19) infection are associated with more changes which impacts on quality of life (QoL). However, parent-proxy and child self-reports may vary, and we have lack of knowledge about these discrepancies. The aim of this study was to identifying the impact of mother−daughter health education on female adolescents' QoL during the COVID-19 pandemic.

Methods

This quasi-experimental study was conducted at two time points; before (T1) and 3 months after health education thorough blended learning (T2) from January to May of 2020. Then 196 participants were divided into intervention (N = 96) and control (N = 100) groups and Health Related QoL was assessed using the PedsQL™.

Results

The results showed significant increase in scores of total QoL and all QoL domains in adolescents by self-reports of adolescents and mothers proxy reports (except emotional performance) compared to the control group at T2 (p < 0.05). Furthermore, mother's rated increase in social performance significantly higher in both groups at T2.

Conclusions

With high social anxiety due to COVID-19, adolescents may be vulnerable to various risks. Improving mothers understanding of the needs of their adolescents are an important issue; given that it is possible increase their QoL by health education, especially during COVID-19 pandemic. It is recommended to organized health education in schools to increase the knowledge of mother–daughter thorough blended learning.

Problem

Building rapport and trust between healthcare professional and patient/parents would improve satisfaction of patient/parents. The purpose of this study was to develop “The Pediatric Provider Communication Skills Assessment Scale” in this study.

Methods

A trial form, with linguistic and psychometric validation and consisting of 44 statements, was administered to a sample of 325 individuals. Data were collected between January 20 and October 22, 2021. The validity of the scale was concluded after examining its construct validity and internal validity. “Exploratory factor analysis” was used for determining construct validity, and “comparison of lower-upper groups” for internal validity. “Cronbach's α reliability coefficient” and “split half test consistency coefficient” were calculated for testing the reliability of the scale.

Findings

The Pediatric Provider Communication Skills Assessment Scale that we developed consists of one dimension and 20 items, and the variance it can explain was calculated as 62.3%. The Cronbach α reliability coefficient of the scale was calculated as 0.90, indicating high reliability.

Conclusions

According to the findings, “The Pediatric Provider Communication Skills Assessment Scale” is a scale with features that can measure in a valid and reliable way, with high variance and a low number of items. The study aims to develop the “The Pediatric Provider Communication Skills Assessment Scale,” and present it to the literature as a validated and reliable new objective measurement tool. This study will enable us to understand more about the complex communication processes involved in pediatric care and how these may influence care delivery.

Problem

One of the most prevalent psychiatric conditions in children is attention deficit hyperactivity disorder (ADHD). Regular psycho-education for people with chronic illnesses and their families is beneficial for prevention, treatment, and rehabilitation. This study aimed to determine the effect of psycho-education given to mothers of children with ADHD on mother-child interaction and family functionality.

Methods

The sample of the study consisted of 98 mothers and children (50 participants for the intervention group and 48 participants for the control group). Mothers in the intervention group were given psycho-education. Pre and post-tests were completed by mothers.

Findings

Post-test mean scores in terms of warmth/affection, hostility/aggression, indifference/neglect, and control subscales of the Parental Acceptance-Red/Control Scale were statistically significantly lower among the intervention group. Post-test mean scores in terms of communication, showing interest, and the behavior control subscales of the family assessment device were statistically significantly lower in the intervention group.

Conclusions

For families dealing with a child with ADHD, psychoeducation increased mother-child interaction and family functioning Therefore, psychoeducation should be widespread in the treatment of ADHD. When ADHD causes problems with mother-child contact, maternal psychoeducation may be helpful.

Topic

Globally, the COVID-19 pandemic had impacted the health care delivery including inpatient psychiatric facilities. Within psychiatric settings, life of inpatients was profoundly altered.

Purpose

This paper aimed to understand if pandemic-related changes within an inpatient Eating Disorder Unit in a specialized psychiatric hospital in Ontario, Canada impacted incidence of aggression and use of coercive methods among adolescents.

Source Used

An exploratory study design was used to examine incidence of aggression, self-harm, code whites, staff assist, restraints and seclusion, and nasogastric feeding (NGF) among adolescents with eating disorders before and after the modified service delivery within the inpatient unit. Descriptive analyses were conducted.

Results

Analyses revealed a complete reduction in episodes of self-harm, aggression, staff assists, use of restraint and seclusion as well as an 80.14% reduction on average use of NGF.

Conclusion

Authors speculate that the change in environment and program delivery method, peer influence, and shift in power relations between patient and staff may have resulted in improved experiences. This report provides insights to adopt a recovery-oriented service delivery for adolescents with eating disorders in inpatient settings.

Objective

Epidemiological studies on the prevalence of elevated depression symptoms among female adolescents in Saudi Arabia report a wide variation, ranging from 13.9% to 80.2%. However, different methods of assessment and sampling have been used. The aim of the current study is to estimate the prevalence of elevated depression symptoms amongst female adolescents in Saudi Arabia using a gold-standard self-report measure, the Mood and Feelings Questionnaire (MFQ).

Design and Measures

A cross-sectional study was conducted with 515 female students aged 13−18 years, recruited from public schools. Participants completed the Arabic versions of the MFQ, Rosenberg Self-Esteem Scale and Multidimensional Scale of Perceived Social Support.

Results

Mean MFQ score for this sample was 26.35 and almost half of participants (48.2%) had scores above the cut-off. Severity of depression varied with age, with those aged 13 showing reduced symptoms, and was negatively correlated with self-esteem and perceived social support. There were no associations with other demographic factors.

Conclusion

Elevated levels of depression symptoms were common in this sample. This highlights the need to improve public mental health in this community and to improve methods of identifying and treating depression in female adolescents.
