Pediatric psychiatric inpatients' perspectives of aggression management: Discernment in the doorway 儿科精神病住院患者对攻击管理的看法:门口的辨别。

Problem

Aggressive behavior is common on psychiatric inpatient units. Seclusion and restraint interventions to manage patients' aggressive behavior may have the consequence of being traumatizing for patients. Pediatric psychiatric patients' perspective on the use of seclusion and restraint interventions is not present in the literature.

Methods

This hermeneutic nursing research study asked the question, “How might we understand children's experiences of seclusion and restraints on an inpatient psychiatric unit?” Four past pediatric psychiatric inpatients shared their hospitalization experiences that occurred within the previous year when they were 10 years old. The texts of the research interviews were compared to Attachment Theory for a deeper understanding of the meaning of the message.

Findings

Participants commonly described experiences with seclusion and restraints as feeling trapped and alone in a dark room. They recommended the nurses step into the room with them to help them heal. Interpretively, the rooms on inpatient units could be considered as actual and metaphorical spaces of possible harm or healing.

Conclusion

The participant's voices expand understanding of nurse's use of discernment at the doorway of a patient room to ensure the most therapeutic care is provided to the patient in these spaces through a secure nurse–patient relationship.

Psychoeducation group facilitation training for nurses: A quality improvement project 护士心理教育小组促进培训:质量改进项目。

Background

According to the 2020−2021 National Survey of Children's Health, 23.3% of children and adolescents met criteria for one or more mental, emotional, developmental, or behavioral problems. However, the prevalence of depression, anxiety, and post traumatic stress disorder have risen dramatically since the COVID-19 pandemic, leading to an increase in suicidal ideations, completed suicides, and pediatric psychiatric hospitalizations. There is a critical need for high quality mental healthcare treatments, including psychoeducation. Inpatient psychiatric nurses are well positioned to lead psychoeducation groups, but few receive training.

Local Problem

Nurses at a designated inpatient pediatric psychiatric unit expressed discomfort in facilitating psychoeducation groups.

Methods

This is a one-group prepost and follow-up quality improvement study designed to improve the quality of psychoeducation group facilitation skills provided by nurses at a child and adolescent inpatient psychiatric unit. A convenience sample of nurses (N = 16) participated. Baseline (T1), post-test (T2), and follow-up (T3) data were collected regarding nurses' self-efficacy, evidence based-practice attitudes, and knowledge of psychoeducation group facilitation.

Interventions

Nurses received 45 min of psychoeducation group facilitation education and 15 min of skills simulation.

Results

The nurses' knowledge, self-efficacy, and attitudes toward evidence-based practice when facilitating psychoeducation groups increased significantly from the baseline to the post-test, which was sustained from the post-test to the follow-up period.

Conclusion

Study findings indicate the importance of supporting nurses in their role of psychoeducation group facilitation through education and program structure. Further research is needed to assess longer term sustainability and efficacy in leading psychoeducation groups.

Late-night eating and inactivity: Links to depression and suicide risk in adolescents living in Turkey 深夜进食和缺乏活动:生活在土耳其的青少年抑郁和自杀风险的关联。

Objective

Depression, anxiety, and eating disorders are all common during adolescence. This study aims to investigate the potential relationship between eating at night, physical activity, and depression/suicide risk in adolescents.

Methods

This study followed a cross-sectional descriptive design and involved collecting data from adolescents aged 13–18 years who attend four different high schools. The data collection was done through an online survey using Google Forms. Sociodemographic form, eating at night questionnaire, International Physical Activity Questionnaire (Short Form), Reynolds Adolescent Depression Scale, and Suicide Probability Scale were used for data collection. The data collected were analyzed electronically using descriptive statistics and correlation tests to examine relationships between different parameters.

Results

The study revealed significant associations between academic achievement, daily sleep duration, and daily walking distance with depression and suicide probability (p < 0.05) among the participants. Adolescents who consumed food after 10 pm exhibited higher depression and suicide probability scores than their counterparts (p < 0.05). Additionally, an analysis of physical activity levels demonstrated that highly active adolescents had lower depression and suicide probability scores.

Conclusions

In conclusion, this study suggests that there is a significant relationship between eating at night, physical activity, and depression/suicide risk in adolescents. Specifically, academic success, daily sleep duration, daily walking distance, and eating habits were found to be associated with depression and suicide probability. Encouraging physical activity and healthy eating habits, particularly avoiding eating late at night, may be beneficial in reducing the risk of depression and suicide among adolescents.

Determination of the relationship between depression and suicide in young adolescents 确定青少年抑郁与自杀之间的关系。

Objective

This study was designed to explore the relationships between depression, and the likelihood of suicide among young adolescents by considering various factors.

Design and Samples

It was conducted in a descriptive-cross-sectional format, with fieldwork spanning from November 16, 2022, to June 30, 2023. The study assessed the likelihood of depression, and suicide in young adolescents attending a university in Eastern Turkey.

Measurements

Sociodemographic features survey form, Beck's Depression Inventory and Suicide Probability Scale were used to collect data.

Results

The results from this investigation indicated that women, individuals with chronic illnesses, those with a history of psychiatric conditions, those expressing a constant desire to die, those with suicidal thoughts or plans, and those with a family member who had attempted suicide exhibited significantly higher mean scores for depression and suicide. There was a positive correlation between high depression scores and suicidal tendencies. However, no conclusive evidence of a link between depression, and suicide was established.

Conclusions

In this study, certain demographic and psychological factors were identified as correlating with elevated levels of depression and suicidal tendencies among young adolescents. It is imperative that individuals identified as being at risk be promptly referred to appropriate units for comprehensive interventions and support.

Demographic, lifestyle, social, and psychological factors associated with worry-related sleep problems among school-going adolescents in Timor-Leste 东帝汶在校青少年中与担忧相关的睡眠问题有关的人口、生活方式、社会和心理因素

Objective

Adolescent sleep problems are a worldwide public health issue. The present study examines factors associated with worry-related sleep problems among school-going adolescents. The country of interest is Timor-Leste, a low-income country, where studies pertaining to adolescent sleep problems are lacking.

Design and Measures

Data were analysed from the Global School-Based Student Health Survey Timor-Leste (n = 3455). An ordered probit model was used to assess the effects of demographic, lifestyle, social, and psychological factors on different levels of worry-related sleep problems (i.e., no, mild and severe sleep problems).

Results

School-going adolescents were more likely to face mild or severe worry-related sleep problems if they were older, passive smokers, alcohol drinkers and moderately active. School-going adolescents who sometimes or always went hungry were more likely to experience worry-related sleep problems than those who did not. Involvement in physical fights, being bullied, and loneliness were positively associated with the probability of having modest or severe worry-related sleep problems.

Conclusion

Age, exposure to second-hand smoke, alcohol consumption, physical activity, going hungry, physical fights, being bullied and loneliness are the important determining factors of adolescent worry-related sleep problems. Policymakers should pay special attention to these factors when formulating intervention measures.

Effects of a structured educational program on nursing students' level of knowledge and awareness toward child neglect and abuse: A quasi-experimental evaluation 结构化教育计划对护理专业学生对忽视和虐待儿童问题的了解和认识水平的影响:准实验评估

Background

Nurses have a key role in the recognition, reporting, and channelization of child abuse and neglect. Nurses should have sufficient knowledge about the presentation of child abuse and neglect. They also need to understand their role in reporting any instance where they detect cases of abuse and neglect.

Purpose

The aim of this study is to determine the effect of a structured Child Neglect and Abuse Educational Program (CNaA-EP) on the level of knowledge and awareness of nursing students about child neglect and abuse.

Methods

This study conducted with nursing students used a pretest−post-test quasi-experimental design. The third-year nursing students were divided into two groups: experimental (n = 25) and control groups (n = 30). The intervention group received CNaA-EP. In both groups, pretests were performed using the “Child Abuse and Neglect Awareness Scale,” and “Child Abuse and Neglect Knowledge Test” The posttest was conducted 1 week after the 14-week CNaA-EP course was completed. Numbers, percentages, chi-square test, t-test, Kolmogorov–Smirnov test, Fisher's exact test were conducted using SPSS 22.00 software.

Results

The education group's scores of child abuse/neglect awareness (t = −2.768 p < 0.05) and child abuse/neglect knowledge (t = −12.416 p < 0.01) were significantly higher than those of the control group. After the education program, a significant difference was found between the education and control groups on the core measures.

Conclusion

A child neglect and abuse education program was an effective method of educating nursing students on knowledge and awareness of child abuse and neglect. For this reason, education programs should be planned and implemented to increase awareness about abuse and neglect problems that threaten children's health and development.

Problem

Self-stigma in parents of children with mental illness is an area easily overlooked by mental health providers. Many studies have shown that self-stigma in parents may result in social interaction avoidance, lower self-esteem, increased psychological pressure, and so on. However, a comprehensive picture of how parents of children with mental illness in China experience self-stigma is lacking.

Methods

Individual semistructured face-to-face interviews were conducted with 20 parents from China during their child's psychiatric hospitalization. We followed the consolidated criteria for reporting qualitative research (COREQ) checklist.

Findings

Four main themes were identified that captured the experience of self-stigma in parents of children with mental illness: (1) losing face and fear, (2) internalizing affiliate stigma, (3) feeling of self-blame and self-doubt, and (4) keeping oneself away from society.

Conclusions

These findings explain how Chinese parents of children with mental illness experience self-stigma. To better effect the treatment and rehabilitation of children with mental illness, it is necessary to recognize this self-stigma in parents and take action to improve their mental health.
