Abstract The influence of ideas central to contemporary forensic psychiatry on the design of England's regional secure units, as well as some other influences on the final designs, are described. While the design processes were, for the most part, successful, some elements of the design-thinking may have been overwrought, involving some degree of ‘magical thinking’ in which the symbolization of desired professional outcomes is confused with the direct causation of these outcomes. While this had little effect on the overall efficacy of the designs, it seems to have led to certain security features being inadequately designed.

Cognitive Analytic Therapy for Offenders 罪犯认知分析疗法

Abstract Cognitive analytic therapy (CAT) has emerged as an innovative form of integrative psychotherapy, which has yielded promising outcomes for several clinical populations. Recently, CAT reformulations and interventions have been applied in the management of forensic patients (Pollock and Kear-Colwell, 1994; Pollock, 1995, 1996, 1997a). The present paper describes two illustrative case-studies to demonstrate the use of CAT as a theoretical basis and treatment framework for interventions with offenders and to highlight the attributes of this form of psychotherapy. Specific clinical issues are emphasized, including: (1) the use of CAT language and concepts to provide an understanding of the offender's representation of his or her relation to the victim; (2) the linking of sequential patterns (termed reciprocal role procedures, dilemmas, traps and snags) to understand the offender's motivations for committing the offence; and (3) the usefulness of CAT interventions, which help to focus and direct clinica...

One of the many proverbs thrown at me when I was kid was ‘he who pays the piper calls the tune’. In modern marketing parlance that might be translated into ‘the customer is king’. A study of the private rural piped water business recently launched by the World Bank’s Water and Sanitation Program has shown this to be an important factor in efforts to improve the access of poor people to safe drinking water. (An earlier blog has discussed the results of the companion study on rural sanitation.)

Abstract This paper is concerned with a case-note study of 44 women on the psychiatric wing of HMP Holloway who had been referred to NHS psychiatric services by prison medical officers. Half the women were difficult to place, being refused a bed at least once. The other half, the comparison group, obtained beds without difficulty. The ‘difficult to place’ women were significantly more likely than the comparison group to have suffered physical and/or sexual abuse in childhood; to have committed a variety of offences; to have committed crimes of serious violence and arson; to have spent time in a secure psychiatric facility in the past; to be considered a danger to themselves and a management problem in Holloway; to have a personality disorder diagnosis; and, eventually, to receive a prison sentence or community disposal rather than a hospital order. Abstract The last decade has seen a sustained interest by both psychologists and psychiatrists in sexual offending. To date, research into the mentally ill sex offender has been limited, with few empirical studies. Home Office statistics demonstrate a rise in numbers of both restricted and unrestricted patients admitted to hospital by sexual offence. These patients are usually detained under the category of mental illness within the meaning of the Mental Health Act 1983. Forensic psychiatrists are expected to possess an expertise in the assessment and treatment of this population and to address concerns about recidivism. The relevance of theories of sexual offending to the mentally ill is explored and consideration given to any role mental illness may play in the offence.
