The article presents the results of a study of the effectiveness of osteopathic treatment of young women aged 18-45 with vertebral artery syndrome. The study involved more than 100 women. Patients with signs of psychiatric pathology and organic brain diseases were excluded. Laser Doppler flowmetry of blood microcirculation was chosen as a method of the evidence-based control. The main group was exposed to osteopathic and conservative treatment while the comparison group (control group) was treated by the generally accepted methods. It was found out from to the study results that osteopathic treatment significantly improved the quality of life including reduction in complaints of fatigue, tinnitus, dizziness and absence of complaints of headache and pain in the cervical region. No changes were registered in the control group during the same examination periods.

A clinical case of cure of spinal canal stenosis and reduction in the size of a herniated intervertebral canal by manual therapy methods and involvement of the patient in therapeutic activities through physical exercises is considered through the prism of neurophysiological knowledge about reality, the principles of national Russian and the preceding Soviet medicine. There is an analysis of the ideas, which are typically used in clinical practice by neurologists and neurosurgeons for analysis, synthesis, and justification of treatment prescription according to their profile and the limited use by them of knowledge of fundamental physiology about the real structure of the human body and its functions in a specific clinical case. Attention is focused on the scientific nature of the methods of manual therapy, applied for diagnosis and treatment, of the national Russian and Soviet schools of medicine, including the Czechoslovak school of manual medicine, which differs significantly from traditional medical chiropractic with different metaphysical justification in the chiropractic schools of American osteopathy and chiropractic, applied kinesiology, and also in Indian Ayurveda, Japanese shiatsu, reiki, mutual aid techniques of martial arts of Indochina, traditional Chinese medicine. The clinical analysis uses scientific knowledge of fundamental physiology, neurophysiology, systemic psychophysiology, cognitive science about the activity of functional systems, which is significant for the patient’s recovery, including ensembles of neurons of central generators of the ordered activity, sensitivity regulation systems, medial and lateral movement control systems, and neuromuscular complexes of motor units. The normalization of their activity by manual therapy methods leads the patient to clinical recovery and an active life of high quality.

The aim of the study is to assess the possibility of studying the depth and area of high-temperature skin lesions in burns by manual methods. Materials and methods: 99 patients with II-III degree thermal skin burns with a lesion area of up to 10% of the body surface were examined at admission to hospital as well as in dynamics. Clinical (palpation diagnostics), instrumental (laser Doppler fluorometry), analytical, and statistical methods of research were used. The study result is the substantiation of the manual (palpation) diagnosis method for determining the true depth and area of the burn wound, the condition of the underlying tissues, the involvement of which in the burn process in the projection of the burn wound affects the course and prognosis of II-III degree thermal burns in patients and their quality of life in the future. The novelty, informativeness, and efficiency of the manual method of diagnosis are shown.

Musculoskeletal (non-specific) low back pain is a significant public health problem that ranks first in the number of years lost due to persistent ill health. The sacroiliac joint is the direct source of pain in 15-30%. Various techniques of manual therapy are used to treat such cases. Currently, there are just few studies comparing the clinical efficacy of different manual therapy techniques, and their results are contradictory. One of the main reasons is the impossibility of conducting a classical double-blind placebo-controlled randomized trial to evaluate the efficacy of MT techniques due to the difficulty of “blinding” completely the patient and the researcher. Since there are no guidelines on how to conduct an appropriate simulation therapy to guarantee the reliability of the methodology and results of clinical trials, we propose our standardized study protocol to investigate the comparative efficacy of high-velocity low-amplitude thrust mobilization and low-velocity low-amplitude rhythmic mobilization of the sacroiliac joint with active placebo.

The article presents a clinical case, follow-up of it in a patient with vertebral artery syndrome (VAS) and the applied methods of osteopathic treatment when a specialized Doppler flowmetry device is used for diagnosis. The issues of diagnosis, obtaining reliable information and its systematization are discussed. The principles and methods of diagnosis by means of the Doppler flowmetry device for the purposes of the VAS treatment are given. Conclusions and proposals for the institutionalization of this method of diagnosis and treatment are laid down.

The article discusses anatomical and physiological features of the pelvis, somatic dysfunctions of the pelvis, which are observed in various diseases and have a significant impact on the functioning of various physiological systems of the body. The results of osteopathic correction of pathogenetically significant dysfunctions in the body, which lead to the emergence and development of many diseases, in combination with osteopathic correction of the detected somatic dysfunctions of the pelvis in 695 patients are presented. The effectiveness of this combination has been shown. The importance of the pelvis for human health and vital activity is substantiated.

Myofascial pain syndrome is one of the most common manifestations of musculoskeletal pain, and it is manifested not only by the pain syndrome but also by localized highly sensitive areas of pain sensitivity changes. Clinical manifestations of isolated trigger zones have been described in the literature. However, in practice, one has to deal with the myofascial pain syndrome caused by multiple trigger zones in the muscles of one or several neighboring regions. In this case, pain patterns overlap and change the clinical picture and, in some cases, lead to misdiagnosis. In order to illustrate such a course of the myofascial pain syndrome, the article presents a clinical case. As a matter of discussion, it is suggested to designate myofascial pain syndrome with multiple active trigger zones in several muscles with interference ofthe referred pain zones as “complex myofascial pain syndrome”.

The aim of the study was to identify the sacroiliac joint dysfunction in patients with various manifestations of vertebrogenic pain in the low back and pelvis and to determine the regimen ofmanual therapy techniques. A comprehensive assessment of examination and treatment of patients with the sacroiliac joint syndrome was carried out. The positive role of the combined treatment including drug therapy and manual therapy was noted. A decrease in the pain syndrome intensity was found in 79.5% of cases, in all groups of patients, already at the beginning of treatment.

At present, the question remains open as to the quality of the methods applied for the clinical study of autonomic functions which makes it possible to get an idea of them in normal and pathological conditions. The lecture discusses autonomic reflexes that are most reliable in clinical practice, as well as reflexes that are directly related to osteopathic techniques. When conducting reflexes, it is necessary to take into account the strength and type of response, the change in the reflex during repeated studies, the effect of various influences on the reflex manifestation, and the ease or difficulty of establishing a conditioned connection to the reflex. Since the mechanism of most osteopathic techniques is reflex, these features are fully applicable to diagnostic and therapeutic osteopathic techniques. Therefore, some techniques, which are modified autonomic reflexes, are both diagnostic and therapeutic.
