Development of the Caring Competence Scale for Family Caregivers of Persons With Mental Disorders 精神障碍家庭照顾者照顾能力量表的编制。

Introduction

Although family care is crucial for the recovery of persons with mental disorders, no tool is available to measure the caring competence of family caregivers.

Aims

This methodological study aimed to develop a scale that reflects the caring competence of family caregivers of persons with mental disorders and to verify its validity and reliability.

Methods

Fifty initial items were derived on the basis of a previous study that analysed the concept of caring competence among family caregivers of persons with mental disorders. The initial items were revised based on content validity, and 48 preliminary items were selected. Data were collected from 361 families of persons with mental disorders. Data were analysed using factor analysis.

Results

The caring competence scale for family caregivers of persons with mental disorders consisted of five sub-factors and 22 items. The five sub-factors were compassionate relationship formation (six items), skilful disease management (four items), respecting the patient (four items), self-care (four items), and hope (four items). The developed scale had high reliability, with a Cronbach's alpha of 0.93. Verifying the criterion validity of the developed scale revealed that the correlation between the criterion scale (caregiving self-efficacy scale) and the developed scale was statistically significant, with a correlation coefficient of 0.63.

Discussion

This study presents empirical evidence for the reliability and validity of the caring competence scale for family caregivers of persons with mental disorders.

Implications for Practice

This scale provides reference for mental health nurses to evaluate the caring competence of family caregivers of persons with mental disorders.

<div> <section> <h3> Introduction</h3> <p>Psychiatric nurses are actively involved in involuntary hospitalisations and treatments. However, the scientific literature lacks insights into strategies used to navigate roles and institutional expectations in this regard.</p> </section> <section> <h3> Aim</h3> <p>To achieve a deeper understanding of practices that support the exercise of individual rights in psychiatry during involuntary hospitalisations and treatments.</p> </section> <section> <h3> Method</h3> <p>A situational analysis was conducted. Nineteen (<i>n</i> = 19) mental health workers participated in a semi-structured interview and completed a socio-demographic questionnaire. Case law review (<i>n</i> = 126) and non-participant observations (<i>n</i> = 70 h) were also conducted.</p> </section> <section> <h3> Results</h3> <p>The moral division of labour is a central element in mental health interventions, organising four types of practices: socio-legal assistance, support for exercising rights, intersectoral coordination and ignorance and trivialisation.</p> </section> <section> <h3> Discussion</h3> <p>Our findings reveal that nursing care is predominantly medicalised, confining nurses to a subordinate role. They also indicate marginal practices focused on socio-legal assistance and human rights support. Intersectoral coordination, though crucial, remains an invisible aspect of nursing practice.</p> </section> <section> <h3> Implications for Practice</h3> <p>There remains a significant lack of awareness about human rights issues in psychiatry. This research underscores the importance of considering professional hierarchies, organisational expectations and legal awareness to address role conflicts in psychiatric care.</p> </section> <section> <h3> Relevance Statement</h3> <p>This research highlights the need to study nurses' agency, offering insights into how they interpret and apply psychiatric laws. Such research could deepen knowledge on human rights issues in psychiatry or, at minimum, promote greater recognition of these concerns. Social policy implications are also notable, underscoring the need for integrated socio-legal assistance during involuntary treatments to support patients navigating complex systems and exercising their rights.

Background

Concussions are a common injury, with many experiencing mental health related symptoms after injury. Despite the persistence of these symptoms, it is not well detailed the impacts of anxiety and depressive-like symptoms.

Aim

This lived experience narrative recounts the authors lived experience with multiple concussion injuries and mental health symptoms.

Methods

The author used a narrative approach to detail the multiple experiences with concussion. The author reflected on these experiences, drawing on their experience as a registered nurse.

Findings

The narrative reveals persistent concussion symptoms are multifactorial. It illustrates how the interdisciplinary mental health care team can benefit concussion patients experiencing anxiety and depressive-like symptoms.

Discussion

The author reflects on their journey and calls for a shift to a more person-centred approach in concussion care. They also highlight areas for improvement in mental health services. Lastly, future directions for mental health nursing are provided.

Conclusion

This narrative highlights the anxiety and depressive symptoms experienced after concussion. By incorporating mental health nurses within concussion care, mental health issues can be addressed in a timely and approriate manner.

Introduction

In Canada, two distinct nursing professions—Registered Nurses (RNs) and Registered Psychiatric Nurses (RPNs)—serve complementary roles, with RPNs primarily practising in Western provinces.

Background

The development of RPNs as a unique professional group is rooted in the establishment of key institutions. In British Columbia (BC) this was notably Riverview Hospital and the BC School of Psychiatric Nursing. These and similar institutions across Western Canada contributed significantly to shaping the standards and education pathways for RPNs.

Aim

This paper examines the evolution of RPNs as a unique designation in Western Canada, focusing on their historical development, including the foundational role of Riverview Hospital and the BC School of Psychiatric Nursing until its closure in 2012. In contrast to RNs, whose education incorporates mental health as one of several focuses, RPNs receive specialized training centred on psychiatric and mental health care, shaping distinct professional identities and scopes of practice.

Method

A historical lens is used to identify the organizational and educational reforms that shaped the identity and integration of RPNs within the broader nursing landscape. An exploration of the historical and contemporary dynamics that shaped RPN role and identity is offered, comparing it with RNs' roles in mental health services and examining the impact of educational and organisational changes. Introduction

Forensic healthcare workers in mental health settings face significant challenges when working with sex offenders. Balancing staff wellbeing with effective offender rehabilitation remains critical.

Aim

This study examined how attitudes, perceptions and workplace factors influence the wellbeing of frontline forensic healthcare workers caring for sex offenders.

Method

A cross-sectional design, adhering to the STROBE checklist, was used. Ninety-one participants from UK privatised mental health settings completed an online survey. Wellbeing was assessed using measures of compassion satisfaction, burnout and secondary traumatic stress, with predictors including attitudes, perceptions and workplace factors.

Results

Positive attitudes, such as understanding offender intent, were associated with improved wellbeing, while excessive trust reduced compassion satisfaction. Perceptions like risk awareness acted as psychological buffers. Workplace factors, including environmental safety and quality supervision, enhanced wellbeing, whereas co-worker support unexpectedly reduced compassion satisfaction.

Discussion

The findings highlight the importance of rational attitudes in enhancing wellbeing. While punitive perceptions offered emotional buffering, fostering balanced approaches is essential for improving therapeutic environments.

Limitations and Implications

The study focused on privatised UK settings, limiting generalisability to NHS contexts. Future research should explore broader populations.

Recommendations

Employers should enhance environmental safety, supervision and training to foster rational attitudes and address peer dynamics.

Background

Anxiety is a common psychological disorder, which may affect daily life, including work, social and personal relationships, leading to a poor quality of life and social costs. Visual Art Therapy (VAT) is an increasingly recognised non-pharmacological intervention aimed at improving mental health through creative activities such as painting and sculpture.

Objective

To examine the effectiveness of VAT in reducing anxiety symptoms in adults.

Design

A systematic review and meta-analysis, including only randomised controlled trials (RCTs).

Methods

Ten databases were systematically searched from inception to July 26, 2024. Standardised mean difference (SMD) was calculated using random effects models. Sensitivity, subgroup and meta-regression analyses were performed to explore potential sources of heterogeneity.

Results

The pooled results of 35 studies showed VAT had a significant effect on improving anxiety symptoms (SMD = −1.31; 95% CI, −1.80 to −0.95; p < 0.001; 35 RCTs, 3167 participants; very low-quality evidence), compared to control group. Subgroup analysis revealed that the effect size was influenced by several factors, including the mean age, duration of the intervention, gender distribution, the presence of mental health problems in adults and the types of interventions used.

Conclusion

VAT may be a potentially effective alternative approach for managing anxiety symptoms in adults, particularly in the context of mental health nursing. Designing and providing this programme for adults with anxiety symptoms could offer a valuable, non-pharmacological intervention that mental health nurses can integrate into their practice.

Introduction

The reduction of coercion requires a stable therapeutic relationship. It is generally assumed that coercive measures have a negative effect on the therapeutic relationship, but little is known about the specific impact.

Question

What is the impact of coercive measures in acute psychiatric care on the therapeutic relationship between nurses and patients?

Method

An integrative review and a thematic analysis were undertaken.

Results

Theme 1, labelled ‘Destructive effects’, encompasses three subthemes: ‘Loss of trust’, ‘Power imbalance’ and ‘Engagement reduction’ and highlights the negative impact on the therapeutic relationship. Theme 2, entitled ‘Nursing dilemma’, with the subtheme ‘Dehumanisation’ discusses the inherent conflicts faced by mental health nurses. Theme 3, ‘Reinforcement’, suggests potential enhancements in therapeutic relationships.

Discussion

Key characteristics of the therapeutic relationship, such as providing support, meeting at eye level, empathy and trust, can suffer damage from coercive measures and diminish a fundamental aspect of psychiatric nursing. Moreover, the absence of a therapeutic relationship can foster behaviours that prompt additional coercive measures, creating a negative cycle with adverse effects for all involved.

Implications for Practice

Nurses need to be aware of the effects of coercive measures on therapeutic relationships and use coercive measures as a last resort.

Introduction

The assessment of patient-reported outcomes of psychiatric and/or mental health nursing remains understudied, limiting understanding of what matters to patients.

Aim

To identify, appraise and synthesise available evidence on patient-reported outcomes, influencing factors, nursing care/interventions and measurement scales of mental health nursing in hospitals.

Method

Ovid MEDLINE, CINAHL, EMBASE, APA PsychARTICLES, Scopus and Web of Science were searched from conception to April 2024 (update March 2025). Two reviewers independently screened and selected the studies based on a predefined PICO framework in a two-stage process. Quality assessment was conducted using the Johanna Briggs Institute (JBI) guidelines.

Results

Analysis of 22 studies identified 25 patient-reported outcomes, measured by 30 unique scales, clustered into three categories: ‘symptoms & complaints-based patient-reported outcomes’, ‘psycho-emotional patient-reported outcomes’, and ‘functional patient-reported outcomes’. Ten influencing factors emerged from two studies. Nursing care/interventions compromised four clusters: nurse-delivered group programme (n = 8), one-to-one nursing therapy or interaction (n = 8), group and individual nursing sessions (n = 1) and interpersonal relationship (n = 5).

Conclusion

Patient-reported outcomes serve as robust validators for advancing psychiatric and/or mental health nursing within hospital settings. Findings emphasise the importance of well-conceptualised nurse-sensitive patient outcomes using valid and reliable scales to recognise the full psychotherapeutic potential of mental health nursing.

Two reviewers independently screened and selected the studies based on a predefined PICO framework in a two-stage process. Quality assessment was conducted using the Johanna Briggs Institute (JBI) guidelines.</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Results</h3>\u0000 \u0000 <p>Analysis of 22 studies identified 25 patient-reported outcomes, measured by 30 unique scales, clustered into three categories: ‘symptoms &amp; complaints-based patient-reported outcomes’, ‘psycho-emotional patient-reported outcomes’, and ‘functional patient-reported outcomes’. Ten influencing factors emerged from two studies. Nursing care/interventions compromised four clusters: nurse-delivered group programme (<i>n</i> = 8), one-to-one nursing therapy or interaction (<i>n</i> = 8), group and individual nursing sessions (<i>n</i> = 1) and interpersonal relationship (<i>n</i> = 5).</p>\u0000 </section>\u0000 \u0000 <section>\u0000 \u0000 <h3> Conclusion</h3>\u0000 \u0000 <p>Patient-reported outcomes serve as robust validators for advancing psychiatric and/or mental health nursing within hospital settings. Perspectives of Nurses and Adolescents About Pro Re Nata Treatment in Adolescent Inpatient Psychiatric Units: A Qualitative Study 青少年精神科住院病人护士与青少年对亲性治疗的看法:一项质性研究。

Introduction

‘Pro re nata’ (PRN) treatment, commonly prescribed in adolescent psychiatric inpatient units, is administered on an ad hoc basis to address specific symptoms. It is prescribed in advance and delivered by nurses when the situation requires it.

Question

This study aims to explore the lived experience of adolescents and nurses during the use of PRN treatment in adolescent psychiatric inpatient settings and to cross these perspectives.

Method

This qualitative study used a specific qualitative research method called Inductive Process to analyse the Structure of lived Experience (IPSE), a five-stage descriptive phenomenological approach based on an inductive process. IPSE was specifically designed for health research by methodologists with extensive experience in qualitative health research. The study followed the SRQR guidelines for qualitative research. Data was collected through semi-structured interviews.

Results

The study included 14 adolescents and 12 nurses. Data analysis revealed a structure of experience with three axes: individual, relational and group.

Discussion

PRN treatment in inpatient settings contributes to the development of agency and empowerment for both adolescents and nurses.

Limitations

This study was conducted in France, where PRN is referred to as ‘si besoin’ (if necessary), and was carried out within the unique context of an inpatient setting.

Recommendations

The efficacy of PRN treatment should be evaluated not only for its therapeutic effects but also in consideration of the institutional and developmental factors surrounding its use.
