The Effect of Cognitive Behavioural Therapy-Based Psychoeducation on Medication Adherence and Aggression in Individuals Diagnosed With Schizophrenia: An Experimental Study. 基于认知行为疗法的心理教育对精神分裂症患者服药依从性和攻击行为的影响:一项实验研究

Introduction: It is known that treatment compliance is low and aggression is higher in individuals diagnosed with schizophrenia compared to the normal population. Cognitive behavioural therapy (CBT) is known to reduce relapse and hospitalisation and increase well-being in individuals diagnosed with schizophrenia. However, there are almost no studies on increasing treatment compliance and decreasing aggression.

Aims: This study was conducted to determine how CBT-based psychoeducation affects medication adherence and aggression in individuals diagnosed with schizophrenia.

Methods: The study was conducted as a quasi-experimental model with the pre-test-post-test control group with 73 schizophrenic patients (33 experimental, 40 control) between June 2022 and July 2023. Data were collected using the Descriptive Characteristics Form, Morisky Medication Adherence Scale (MMAS) and Buss-Perry Aggression Questionnaire (BPSQ). The schizophrenic patients in the experimental group were given eight sessions of CBT-based psychoeducation, while the schizophrenic patients in the control group were not given any training. The data were analysed using mean, standard deviation, chi-squared test, dependent samples t-test and independent samples t-test.

Results: It was determined that the aggression level of the experimental group before the training was 80.51 ± 19.38, and after the CBT-based psychoeducation, it was 73.12 ± 15.28. It was determined that the aggression level of the control group before the training was 84.22 ± 12.13, and after the post-test, it was 85.60 ± 11.72. It was determined that the medication adherence level of the experimental group before the training was 2.75 ± 1.25, and after the CBT-based psychoeducation, it was 3.57 ± 0.67. It was determined that the medication adherence level of the control group before the training was 2.05 ± 1.33, and after the post-test, it was 2.17 ± 1.36. The psychoeducation based on CBT caused a statistically significant difference (p = 0.001) in medication adherence and aggression levels in the experimental group.

Discussion: It was determined that CBT applied to individuals diagnosed with schizophrenia was effective on increasing their medication adherence and reducing their aggression. It is recommended for psychiatric nurses to include CBT-based psychoeducation in their nursing practices in order to increase medication adherence and reduce aggression in individuals with schizophrenia.

Implications for practice: The findings emphasise that CBT-based psychoeducation increases treatment adherence and significantly decreases the level of aggression in individuals diagnosed with schizophrenia. Psychiatric nurses should include CBT-based psychoeducation in their treatments.

Patient Participation in Multidisciplinary Team Meetings in Residential Mental Health Services: An Explorative Study of Patients' Perception. 住院心理健康服务中多学科小组会议的患者参与:患者感知的探索性研究。

Introduction: The lack of evidence of mental health patients' perception on patient participation in multidisciplinary team meetings (MTMs) is a potential threat to the person-centeredness of care.

Aim: To explore the perceptions of mental health patients regarding patient participation in MTMs and to identify factors associated with these perceptions.

Method: In a cross-sectional study, 127 former and 109 currently admitted mental health patients completed the Patient Participation during Team Meetings Questionnaire (PaPaT-Q). The STROBE-checklist was used for reporting the study.

Results: Overall, patients' perceptions were positive. Former patients expressed greater willingness to participate in MTMs, considered participation as more important, and felt more competent. These patients also exhibited a stronger preference for an autonomous role in medical decision making when participating in MTMs. The perception varied across factors such as gender, educational level, nature of psychological complaints and prior experience(s) with participation in MTMs.

Discussion: These findings may help mental healthcare professionals to become more aware to factors associated with mental health patients' perception of patient participation in MTMs.

Implications: The results can be used by mental healthcare professionals to motivate mental health patients in an even more tailor-made basis to participate in MTMs when admitted in a hospital.

Nurse-Led Physical Screening of Patients With Substance Use Disorders: A Cross-Sectional Study. 由护士主导的药物使用障碍患者体格筛查:横断面研究

Introduction: Somatic comorbidity is the main cause of reduced life expectancy in patients with substance use disorders (SUDs).

Aim: This study aims to investigate somatic health challenges in patients with SUDs using the USS. Patient self-management of somatic health and quality of life were also assessed.

Method: In our study, a stratified random sample of 136 clients who received treatment at a specialist addiction facility was drawn to investigate their physical health and lifestyle behaviours using the Utrecht Somatic Screening 2.0 (USS 2.0). The sample included three subgroups, that is, patients receiving outpatient treatment through Flexible Assertive Community Treatment (FACT), Heroin-Assisted Treatment (HAT) and Opioid Replacement Therapy (ORT). Pain, self-management and quality of life were also measured. Descriptive statistics were used to analyse the data.

Results: The findings indicate that many patients experience a large number of somatic health problems, including pain and exhaustion. The most prevalent conditions in this study are cardiovascular disease (25%) and COPD (20%), underweight and poor oral health. Tobacco smoking is prevalent among 88% of patients. Fifty percent of the patients expressed a need for support in managing their physical health.

Implications for practice: Somatic health care and lifestyle promotion should be tailored to the specific characteristics of patients.

Conclusion: Patients with SUDs suffer from poor physical health and show unhealthy lifestyle behaviours, which demand the promotion of tailored somatic health.

<p>This paper follows the paper exploring the place of qualitative research in mental health nursing practice. It moves from the methodologies to the method of conducting qualitative research. It will explore three key components of the research process: developing a research question, data collection, and data analysis. It also considers some of the limitations of qualitative research.</p><p>All qualitative research processes pivot on the research question. The question will determine both the methodology and the method of conducting the research. It encapsulates the object of the research inquiry and facilitates the research process. Ideally, its focus would be the patient's experience of some aspect of mental health nursing practice. A good place to start is to identify an area of mental health nursing interest or consider those taken-for-granted aspects of practice e.g., the patient's experience of admission, treatment, a specific intervention, the relationship with nurses, and the psychiatric process. The next step is to explore the literature on the topic and what studies have been undertaken.</p><p>The research needs to contribute to new knowledge, so it is important to identify gaps in the literature. For example, your interest may be the use of observations in an acute inpatient setting as a strategy to manage risk. You would need to explore the nursing literature and the broader literature on observation and risk management and the mental health nursing literature. Most research raises further questions, and it is these unanswered questions that require further investigation. There will be relevant studies that have focused on observation and risk management, but your goal is to identify those aspects of the findings that raise further questions. Studies may have examined patients' experiences of observations from different perspectives, and your question may develop from those findings e.g., one study found that patients' sleep was disturbed by nursing observations at night (Veale et al. <span>2020</span>). You may choose to develop a subsequent question around what patients think may be more helpful or the patients' experience of sleep deprivation and its impact on recovery.</p><p>Another strategy for developing a research question is to focus your exploration of the literature on systematic and integrative reviews and qualitative meta-syntheses. Observations may be regarded as a form of restrictive practice so you would need to look at the literature reviews on this topic. For example, an integrative review of patient preferences related to restrictive practices (Lindekilde et al. <span>2024</span>) reported that factors such as environment, communication and duration were found to influence patients' preferences. Secondary Traumatic Stress and Coping Experiences in Psychiatric Nurses Caring for Trauma Victims: A Phenomenological Study. 照顾创伤受害者的精神科护士的二次创伤压力和应对经验:现象学研究。

Aim: In this study, it was aimed to evaluate the secondary traumatic stress and coping experiences of psychiatric nurses who care for people who have experienced psychological trauma with a phenomenological approach.

Materials and methods: In this study, semi-structured in-depth interviews were conducted with 18 psychiatric nurses residing in Turkey using the phenomenological research method. The criterion sampling method was used to reach the sample group. Interviews continued until data saturation was achieved, and all interviews were audio-recorded and then transcribed. The data were analysed using thematic analysis. The study was conducted and reported using the COREQ checklist.

Findings: In the analysis of the data, three themes (difficulties in caring for people who have experienced trauma, the effects of caring for people who have experienced trauma on the individual and coping, reflections of secondary trauma on the profession) and seven sub-themes (mental, physical, social, psychosocial difficulties and coping, quality of care, professional and organisational aspects) were identified.

Conclusion: As a result of the study, it was determined that secondary traumatic stress was observed in psychiatric nurses caring for individuals who experienced psychological trauma. It was determined that caring for people who have experienced psychological trauma can negatively affect nurses psychosocially, and individuals have difficulty coping. In line with the results of the study, it is thought that necessary intervention programs should be created for nurses experiencing secondary traumatic stress.

Introduction: The COVID-19 pandemic profoundly affected nurses globally. As frontline workers, nurses faced increased responsibilities amid challenges such as isolation, infection risks, family obligations and disrupted social support systems. Coping with these challenges was associated with adverse mental health outcomes.

Aim/question: This perspective paper examines a collaboration between two schools of nursing in the United States and India to address these mental health challenges.

Methods: Faculty from both institutions identified key mental health topics, leading to five webinars that delved into topics such as acute and long-term impacts of COVID-19 on nurse mental health, coping mechanisms, caregiving responsibilities and children's needs during the pandemic.

Results: Impressively, over 11,000 nurses from 60 countries engaged, fostering a global platform for sharing evidence-based knowledge, experiences and strategies.

<p>Qualitative research methods complement mental health nursing practice—with a focus on the lived experience. These methods do not work from pre-determined categories applied to the person's experience but rather seek to find and interpret the categories and themes from within the collected data Qualitative research fits with the skills embedded in mental health nursing practice, qualitative research because it is well-suited to the investigation of the complexities of human responses and experiences that are integral to everyday practice. Mental health nurses who practice a recovery-focused model of care have a unique perspective on the complexities of mental health distress and the patient's needs for connectedness, hope, identity, meaning and empowerment that promotes recovery (Leamy et al. <span>2011</span>).</p><p>Qualitative research methods are oriented towards understanding meanings and experiences. They are therefore potentially very useful in the mental health context because they can provide new insights and knowledge in poorly understood and complex areas, such as understanding subjective experiences of mental disorders and their treatments (Fossey et al. <span>2002</span>). The subjective experience is needed to inform the interpersonal, socio-cultural and political context of mental distress, treatment and service delivery. While quantitative and statistical research methods enable deduction and prediction irrespective of context, qualitative research enables contextualised understandings of subjective experiences.</p><p>Quantitative research is focused on the experimental examination of an intervention in terms of quantity, amount, intensity or frequency and is supposedly objective but the choice of what is examined and how is always a subjective process. Qualitative researchers study things in their natural setting, attempting to make sense of or interpret phenomena in terms of the meanings people bring to them (Denzin and Lincoln <span>2011</span>). It does not obscure the subjective nature of the research process but acknowledges that there can be multiple interpretations of the data. An audit trail identifies how the interpretations were developed.</p><p>While qualitative research makes no claims to objectivity it does follow rigorous processes based on theoretical paradigms and epistemologies. These need to be clearly stated within the paper because they determine the characteristics of the study aim and its methods of data collection and analysis. The most commonly used methods are phenomenology, grounded theory, discourse analysis and interpretative thematic analysis. These methods are supported by explicit theoretical paradigms underpinned by ontological and epistemological assumptions of the nature of reality and the nature of experience.</p><p>The following section provides an overview of some of the core methodologies. Qualitative researchers study things in their natural setting, attempting to make sense of or interpret phenomena in terms of the meanings people bring to them (Denzin and Lincoln &lt;span&gt;2011&lt;/span&gt;). It does not obscure the subjective nature of the research process but acknowledges that there can be multiple interpretations of the data. An audit trail identifies how the interpretations were developed.&lt;/p&gt;&lt;p&gt;While qualitative research makes no claims to objectivity it does follow rigorous processes based on theoretical paradigms and epistemologies. These need to be clearly stated within the paper because they determine the characteristics of the study aim and its methods of data collection and analysis. The most commonly used methods are phenomenology, grounded theory, discourse analysis and interpretative thematic analysis. These methods are supported by explicit theoretical paradigms underpinned by ontological and epistemological assumptions of the nature of reality and the nature of experience.&lt;/p&gt;&lt;p&gt;The following section provides an overview of some of the core methodologies.
