Hepatitis E virus (HEV) is the leading cause of acute viral hepatitis worldwide, recognized as a significant global public health concern. Recent advancements in understanding the natural history of HEV infection have shed light on its epidemiology and clinical implications. The primary mode of HEV transmission is fecal-oral, occurring through contaminated water or food. Parenteral transmission, particularly through blood transfusions, was initially overlooked but has been increasingly recognized in both developing and industrialized countries. Acute HEV infection typically manifests as self-limiting jaundice, particularly in immunocompetent individuals. However, recent data suggest that acute infection can progress to a chronic form in various immunosuppressive conditions, including solid organ transplantation, hematological malignancies, and human immunodeficiency virus (HIV) infection. Chronic HEV can lead to cirrhosis, which may progress rapidly in some cases. Extrahepatic manifestations, particularly neurological complications, have also been reported. HEV remains underdiagnosed globally due to a lack of awareness among healthcare providers in many regions. Advances in serological and molecular assays have facilitated reliable diagnosis, both in immunocompetent and immunocompromised patients. Ribavirin monotherapy has proven effective in treating chronic HEV infection in immunosuppressed individuals and is currently widely recommended. However, its efficacy in acute HEV remains inconclusive. In 2011, an effective and well-tolerated HEV vaccine was developed and approved in China. This vaccine holds promise for high-risk populations, particularly individuals with cirrhosis and travelers to endemic regions.

INTRODUCTION: This study aimed to evaluate and raise the therapeutic hypothesis of the use Photobiomodulation (PBMT) as a complementary therapy in patients with sleep disorder (SD) in a broad approach involving other pathologies superimposed on this pathology. METHODS: This is a clinical case report of a patient with Parkinson's disease (PD) complaining of sleep disorder. The proposed treatment is the use of photobiomodulation (PBMT) with intraoral application using an innovative dosimetry. SD was assessed using an adapted analogue scale. RESULTS: In the evaluation performed after the last session, there was an improvement in the sleep disorder compared to the pre-treatment evaluation. However, one month after the end of the treatment, it was observed that the quality of sleep remained practically the same. CONCLUSION: The results of the present study suggest that photobiomodulation could become a new therapeutic perspective for the treatment of sleep disorders (SD). However, controlled, and randomized clinical studies must be carried out to strengthen and confirm the evidence observed in this case report.

The prevailing global social and geopolitical situation is perceived as unfavorable by most people. According to the authors, many worrying threats are linked to the mental state of the planet's inhabitants. Thus, they believe that the emergence of authoritarian regimes, the constant instigation of wars, and the poor living conditions in many communities require explanations grounded in accurate psychological concepts. The authors refer to their previous work that connects authoritarian regimes with conservative personality traits, particularly those found in far-right, populist groups. These regimes often promote a patriarchal social structure and rely on religious taboos to maintain control. The authors expand on this theory by exploring Sigmund Freud's concepts of "totem" and "taboo," examining how these ideas might explain the behavior of authoritarian regimes. They also refer to Carl Jung's theory of the Shadow and the collective sense of inferiority. The article concludes with suggestions on how to counter the spread of authoritarian regimes. The authors advocate for a balanced approach that focuses on finding ways to coexist with authoritarian individuals. They draw parallels between the different neurophysiological functions of the brain's two hemispheres and the potential coexistence of multiple geopolitical blocs. The authors reflect on a country that recently shifted from far-right, populist, national governance back to a liberal democratic pattern. They also propose a tool to record personal civic feelings, which can help assess whether a particular region is moving toward or away from autocratic governance.

The article takes the view that the conflict in Gaza, involving Israel and Hamas cannot be viewed objectively by assuming that the conflict started on October 7th, 2024. Rather is the conflict a reflection of the pattern of such events and related actions by Israel as the occupying power over 75 years since the declaration of the state of Israel on May 24th, and indeed related conflicts in Palestine prior to that declaration. The article briefly examines to origins of Zionism as an ethic nationalism in the rise of nationalisms in the final 30 years or so of the 19th century. It remarks in particular on the rise of and persistence of Revisionist Zionism and its European neo-liberal roots. It explores the development of Zionism and how it has morphed into a political instrument, going well beyond a benign ethnic nationalism. However, aside from documenting many of the repressive actions of the successive government of Israel since the inception of the state and arguing that Israel’s action following October 7th have been disproportionate, the article explores two potential permanent and practical solutions to the successive conflicts in Palestine: a two-state solution and a confederal solution, both to be guaranteed internationally. The aim of any settlement, it os argued, must provide security and integrity for the state of Israel and the establishment of a similarly recognized and protected Palestinian state. A variety of sources, including Jewish sources, have been used and referenced.

This article discusses "extreme" Prisoner's Dilemma games, and questions Nash equilibrium as a solution concept in such situations. A slight reformulation of the concept is suggested, securing that a more reasonable solution is chosen. A SMALL WARNING TO THE READER: This article is special, not everything is true. That is, there are fictional parts. The reader is left for deciding what is fictional and what is science. Perhaps we could categorize the paper as a quiz where the objective is to rule out fictional parts from the content which at least is meant to be scientific.

The collector and brushless electronic commutation machines have gained widespread utilization in industrial applications. Among these machines, internal permanent magnet synchronous motors (IPMSMs) have become increasingly popular due to their numerous advantages, including high torque/current and torque/inertia ratios, robust construction, high efficiency, and reliability. However, the incorporation of position sensors in IPMSMs has introduced challenges concerning their application, performance, mass production, and cost. Consequently, the implementation of sensorless control techniques has become essential in drive systems and various applications. This paper proposes a backstepping control approach for achieving speed sensorless control of IPMSMs, employing an extended Kalman filter (EKF). The first step involves developing a comprehensive nonlinear dynamical model of the IPMSM in the direct and quadrature (d-q) rotor frame and obtaining its corresponding state-space representation. Subsequently, backstepping controllers for rotor speed and current tracking are designed to ensure precise tracking and anti-disturbance performance. These controllers are integrated into the field-oriented control (FOC) scheme. The Lyapunov stability theorem is employed to guarantee the asymptotic stability condition of the backstepping controller. To address the challenge of estimating immeasurable mechanical parameters of the IPMSM and accurately tracking the system states, an EKF is designed. This filter enables the estimation of mechanical parameters and achieves high steady-state precision within a finite time. The effectiveness of the proposed methodology is demonstrated through simulations conducted under various dynamic operating conditions, including sudden torque load changes, command speed changes, and parameter variations.
