Screening for Attention Deficit Hyperactivity Disorder among a Group of Allergic School-Aged Children 一组过敏学龄儿童注意缺陷多动障碍筛查

Patients and Methods: A group of 120 children with physician diagnosed allergies and another 120 age and sex matched healthy children were enrolled. Patients were recruited from the Pediatric Allergy, Immunology and Rheumatology Unit, Children’s Hospital, Ain Shams University, during the period from May 2022 to November 2022. Diagnostic and Statistical Manual of Mental Disorders (DSM-V) criteria for ADHD, Conners scale and strengths and difficulties questionnaire (SDQ) were measured through valid translated questionnaire. Medical records were revised for the results of absolute eosinophil count (AEC), total IgE level and skin prick test (SPT) if available . Results: Patients included 77 (64.2%) males and 43 (35.8%) females with ages ranging from 6 to 12 years. Allergic diseases were distributed as follows: 50% with urticaria, 35% with allergic rhinitis/asthma and 35% with atopic dermatitis. Allergic patients had higher scores in SDQ subscales, total difficulties (p=0.001), emotional problems (p=0.006), conduct problems (p=0.007), hyperactivity (p=0.001) and conners’ subscales oppositional (p=0.045), inattentive (0.001), hyperactive (p<0.001), anxiety (p<0.001), perfectionism (p<0.001)

Preschool wheeze among a retrospective cohort of Egyptian children 埃及儿童学龄前喘息的回顾性队列研究

the developed world, with even more frequency and severity in the developing countries. Epidemiological studies on this subject in Egypt are lacking. We sought to investigate the frequency and different phenotypes of preschool wheeze (PSW) among a group of Egyptian children and their relation to different risk factors. Methods: A retrospective cohort study was conducted on 1209 children, aged 6-10 years, recruited from the Outpatients’ Clinics of Ain Shams University Hospitals from March 2019 to March 2021. Children with underlying chronic disease or suspected to be immunodeficient were excluded. The main study tool was a survey composed of 30 questions and presented to the parent/caregiver via direct interviewing by the investigator and included sociodemographic data details of wheezing episodes before the age of 6, wheezing risk factors and comorbidities. Skin prick test (SPT) was done for the prolonged early, persistent early and late onset wheezers, while pulmonary function tests were done for the currently asthmatics patients . Results: History of PSW was positive in 298/1209 (24.6%) of the participants. According to the onset, duration and course of wheezes, preschool wheezers were classified as transient wheezers (10.4%), prolonged early wheezers (3.9%), late onset wheezers (3.2%) and persistent wheezers (7.1%) relevant to the whole cohort. Fifty percent of PSW were multi-trigger wheeze while 39.3% were episodic viral wheeze. Twenty-five patients out of 148 wheezers (16.9%) who underwent SPT were found to be atopic . Conclusion: PSW is commonly reported among Egyptian children, paralleling international reports with transient and persistent early wheezing being the most common phenotypes in Egypt.

of the CC16 gene was suggested to increase the risk of developing Background : The dysregulation of CC16 protein secreted by club cells (Clara cells) was reported in acute respiratory distress syndrome and Chronic obstructive pulmonary disease. We sought to investigate serum and urinary CC16 in asthmatic children in relation to asthma exacerbation and quiescence and correlate it to pulmonary function test results. Methods: This prospective controlled study was conducted in the Pediatric Allergy, Immunology and Rheumatology Unit, Children’s Hospital, Ain Shams University on 40 atopic asthmatic patients, 6-12 years old, and 40 matched healthy controls, during the period from March 2020 to August 2021. Patients were enrolled consecutively during an asthma flare and were followed up to be re-evaluated at asthma quiescence. Patients were subjected to clinical assessment, skin prick testing to common aeroallergens, pulmonary function testing, and measurement of serum and urinary CC16 by ELISA during asthma exacerbation and quiescence . Results: Serum and urinary CC16 levels in patients during asthma exacerbation (median 243.5 and 137.5 ng/ml, respectively) and quiescence (median 112.5 and 55 ng/ml) were significantly higher than the levels in matched controls (median 15 and 13 ng/ml). Moreover, serum and urinary CC16 levels were higher during exacerbation than during quiescence (z=-5.214 and 4,941 respectively, p values < 0.001). Urinary CC16 showed significant inverse correlation with best FVC% of predicted (r=-0.408, p= 0.009), but no significant correlation was found with age, BMI, age of onset of asthma, disease duration . Conclusion: Serum and urinary CC16 levels seem to be elevated in asthmatic children especially during asthma exacerbation. Measurement of CC16 in urine might represent a non-invasive option that can replace blood sampling. Further wider scale studies involving timely measurements of CC16 are needed to efficiently assess the role of CC16 as a biomarker of asthma exacerbation .

Patients were enrolled consecutively during an asthma flare and were followed up to be re-evaluated at asthma quiescence. Patients were subjected to clinical assessment, skin prick testing to common aeroallergens, pulmonary function testing, and measurement of serum and urinary CC16 by ELISA during asthma exacerbation and quiescence . Results: Serum and urinary CC16 levels in patients during asthma exacerbation (median 243.5 and 137.5 ng/ml, respectively) and quiescence (median 112.5 and 55 ng/ml) were significantly higher than the levels in matched controls (median 15 and 13 ng/ml). Moreover, serum and urinary CC16 levels were higher during exacerbation than during quiescence (z=-5.214 and 4,941 respectively, p values < 0.001). Urinary CC16 showed significant inverse correlation with best FVC% of predicted (r=-0.408, p= 0.009), but no significant correlation was found with age, BMI, age of onset of asthma, disease duration . Conclusion: Serum and urinary CC16 levels seem to be elevated in asthmatic children especially during asthma exacerbation. Measurement of CC16 in urine might represent a non-invasive option that can replace blood sampling.
