Parvovirus infection and congenital leukemia: Is it a causal or casual? 细小病毒感染与先天性白血病:是因果关系还是偶然关系?

Upon arrival, she was not pale. She had low-grade fever, but other vital signs were normal. Cardiovascular and respiratory examination was unremarkable. Abdominal examination revealed a 28 weeks size uterus. The fetus was in longitudinal lie with cephalic presentation. Ultrasound scan confirmed the diagnosis of an IUD with no hydropic features. The couple was counselled regarding the diagnosis and bereavement counselling was given. She had an uneventful delivery following combination of mechanical and prostaglandin induction. She delivered a baby girl with birth weight of 1.33 kg. There was no dysmorphic features or skin lesion noted. Post mortem consent was obtained.

Psychiatric morbidities among attenders of an Islamic spiritual healing centre in Malaysia 马来西亚伊斯兰精神治疗中心参加者的精神病发病率

Introduction: Traditional medicine which includes Islamic faith healing centre is still an important place to seek treatment whether for psychiatric reasons or others. This study aims to determine the prevalence of psychiatric illness among attenders of an Islamic spiritual healing centre and to understand patients’ illness attribution and their religiosity. Methods: A cross-sectional study was done with a systematic random sampling to obtain the prevalence of psychiatric illness which was diagnosed using Malay version of Mini International Neuropsychiatric Interview (M.I.N.I). Religiosity was measured using Hatta Islamic Religiosity Scale 1996 (HIRS96) and illness perception using Brief Illness Perception Questionnaire (BIPQ). Results: The prevalence of psychiatric illness among the attenders was 53.5%. Depressive disorder were more prevalent (41.5%) followed by anxiety disorder (28.3%), bipolar disorder (16.8%) and psychotic disorders (5.9%). More than half (58.8%) of attenders attributed their illness to supernatural causes but there were no significant difference between those with psychiatric illness and those without in terms of their attribution to supernatural causes, p=0.197.Generally, the patients had  a higher religiosity score. Conclusion : This high prevalence shows a need for collaboration with the spiritual healing practitioners to improve delivery of treatment to patient and this would be a more holistic approach.

Prevalence and clinical characteristics of Metabolic Syndrome among Malaysian Hypertensive subjects using the International Diabetes Federation definition 根据国际糖尿病联合会的定义,马来西亚高血压患者代谢综合征的患病率和临床特征

Individuals with metabolic syndrome are at increased risk for developing cardiovascular disease and diabetes mellitus. This study was carried out to determine the prevalence of metabolic syndrome and clinical characteristics in hypertensive patients according to the criteria of the new International Diabetes Federation (IDF) definition. Hypertensive patients were recruited from the Medical Out-Patient Department, Kuantan Hospital. The five components of metabolic syndrome were examined which included blood pressure (≥130/85 mmHg), fasting glucose (≥5.6mmol/L), fasting triglycerides (≥1.7 mmol/L), high-density lipoprotein (HDL) cholesterol level (<1.03mmol/L in males and <1.29mmol/L in females), and abdominal obesity (waist circumference: men>90cm; women>80cm). Out of 139 hypertensive patients, there were 113 met all the selection criteria consisted of 61 male and 52 female subjects. The participants’ age ranged from 21 to 91 years (51.9±16.8 years; mean±SD), and body mass index 13.5-42.3 kg/m (27.5±4.9 kg/m). According to the IDF criteria, the prevalence of central obesity was 67.2% in men and 84.6% in women. Among the 113 hypertensive subjects over 21 years of age, 51 subjects or 45.1% had metabolic syndrome. The present data revealed that there was high prevalence of metabolic syndrome in Malaysian hypertensive subjects. This finding was supported by the fact of high prevalence of central obesity among the study subjects.

Depression, Anxiety and Sexual Dysfunction in Patients with Diabetes Mellitus With and Without Foot Ulcer 糖尿病伴或不伴足溃疡患者的抑郁、焦虑和性功能障碍

Introduction: Anxiety and depression are both associated with increased morbidity in diabetic population, especially those afflicted with foot ulcer, whereas Erectile Dysfunction (“ED”) and Female Sexual Dysfunction (“FSD”) are amongst the common complication found in diabetes. This study aimed to determine the prevalence of anxiety and depression as primary outcome and sexual dysfunction (“SD”) as a secondary outcome in diabetic patients with and without foot ulcer (“DFU”). Methods: A cross-sectional study was conducted in Hospital Raja Permaisuri Bainun Ipoh, Perak, where 164 diabetic patients were recruited for the purpose of this study. Hospital Anxiety and Depression Scale (“HADS”) were utilised to measure anxiety and depression. International Index for Erectile Function 5 (“IIEF-5”) and the Malay version of Female Sexual Function Index (“MVFSFI”) were used to detect SD. Simple and multiple logistic regressions were referred to in order to evaluate the factors associated with anxiety, depression and SD. Results: In the group with foot ulcer, 36.8% had depressive and anxiety risk respectively compared to without the foot ulcer, 37.5% and 38.5% respectively. Male gender (AOR=6.3,p=0.002,95%CI 1.975, 20.205) and having lower education status (AOR=6.0,p=0.018,95%CI1.36,26.59,) was associated with foot ulcer. High prevalence of SD found in foot ulcer group (52.6%,p=0.006). Those with depression (OR=4.421,95% CI 1.08,18.16,p=0.039) and male gender (OR=18.42,p<0.01,95%CI5.5,-61.62) predicts present of SD. Conclusion: Depression and anxiety prevalent were similar for both with and without diabetic foot ulcer. Several significant predictor factors were identified for DFU group, for anxiety and depression risk and for SD in a diabetes patient. Further studies are required to establish the directional nature of this relationship.

Short Communications Caring for Muslim Patients - Some Religious Issues 关心穆斯林病人的简短交流——一些宗教问题

ABSTRACT Islam is a universal religion and a comprehensive way of life that cannot be separated from patients. Muslim patients are not just passive recipients of medical decisions, but have their own religious views and beliefs about how they would like to be cared for by the medical profession. With the increasing Muslim population in the west, problems arise when a Muslim patient is admitted to a hospital with non-Muslim health care-giver, particularly related to dietary and nutritional issues. The health team should be aware of the religious prohibitions in Islam such as wine or alcohol, flesh of swine, reptiles, birds with talons, canine animals or scavenging creatures, intoxicants etc. The guidelines presented in this paper would enable the health provider to serve their Muslim patients in the most appropriate manner. 摘要伊斯兰教是一种普世宗教,是一种离不开病人的综合生活方式。穆斯林患者不仅是医疗决策的被动接受者,而且对他们希望如何得到医疗专业的照顾有自己的宗教观点和信仰。随着西方穆斯林人口的增加,当穆斯林患者与非穆斯林医护人员一起入院时,就会出现问题,尤其是与饮食和营养问题有关的问题。卫生团队应了解伊斯兰教的宗教禁令,如葡萄酒或酒精、猪肉、爬行动物、有爪子的鸟类、犬科动物或拾荒动物、麻醉剂等。本文中提出的指南将使卫生服务提供者能够以最适当的方式为穆斯林患者服务。关键词:穆斯林病人,医院饮食,禁忌食品

Major cellular events in peripheral nerve regeneration: A brief overview 外周神经再生中的主要细胞事件:综述

ABSTRACT Injury to a peripheral nerve leads to degeneration of the segment distal to the site of lesion, a process referred to as Wallerian degeneration. During Wallerian degeneration, axons and myelin sheaths undergo degeneration and are phagocytosed by macrophages and Schwann cells. The Schwann cells proliferate and the endoneurial tubes persist, together the whole structure is known as the band of Bungner. Within few hours, the damaged axons in the proximal stump initiate a regeneration response, with formation of new growth cones. During Wallerian degeneration, neurotrophins, neural cell adhesion molecules, cytokines and other soluble factors are upregulated to facilitate regeneration. The recovery of the target in mammals is often variable, but almost never complete. In humans, scar tissue forms at the site of lesion and this often results in poor recovery of the target. The major events underlying this regenerative process is highlighted and discussed in this review. KEY WORDS: Wallerian degeneration, Nerve regeneration, Nerve injury

Inflammatory Pseudotumour of Stomach in an old lady: a rare case report 一位老太太的炎症性胃假性肿瘤:一例罕见病例报告

ABSTRACT Inflammatory pseudotumour (IPT) is a rare benign solid tumor in adults and children. The prevalence, etiology and pathogenesis of this condition are still uncertain. Despite the use of modern laboratory techniques and imaging, it is often difficult to make the diagnosis of IPT. Besides, occasionally the nonspecific morphological appearance and clinical presentation of the mass may mimic other more common primary or secondary neoplasms. IPT is commonly encountered in the lung and mediastinum. Other sites include abdomen (liver, pancreas, stomach, omentum), retroperitoneum, pelvis (bladder) and extremities in children. We report a rare case of gastric inflammatory pseudotumour in a 65-year-old female patient. Clinical presentations and its management along with review of literatures are presented.
