Multiphoton imaging of tumor biomarkers in situ using highly oriented conjugates of single-domain antibodies and quantum dots 利用单域抗体和量子点的高定向偶联物原位多光子成像肿瘤生物标志物

High-quality imaging for immunofluorescence diagnosis in medical practice requires a high sensitivity of labeling and discrimination between the fluorescence of immunostaining probes and tissue autofluorescence. Multiphoton microscopy with excitation in the near-infrared spectral region, far from the region of excitation of tissue autofluorescence, enables deep imaging of biological tissues. CdSe/ZnS quantum dots (QDs) conjugated to single-domain antibodies (sdAbs) are efficient diagnostic nanoprobes with diameters <;12 nm and the two-photon absorption cross section exceeding 49,000 GM. Such nanoprobes ensure clear discrimination of tumor areas from normal tissue with the ratio of sdAb-QD emission to the autofluorescence for two-photon excitation >40 times higher than that for one-photon excitation.

Digital assistance services for emergency situations in personalized mobile healthcare: Smart space based approach 个性化移动医疗中的紧急情况数字援助服务:基于智能空间的方法

Recent progress in technologies of the Internet of Things (IoT) enables advanced scenarios for mobile healthcare (m-Health). In this paper, we consider the service intelligence in m-Health (so called smart services). On the one hand, such a service utilizes the telemedicine approach when existing healthcare services (located in hospital) are delivered to remote patients (out of hospital). On the other hand, construction and delivery of a smart service benefit from additional intelligence attributes, beyond a fixed set of healthcare services of a given medical information system. For this promising class of m-Health services, we introduce reference scenarios of personalized assistance for a mobile patient with focus on emergency cases. Our development employs the smart spaces paradigm: patients, medical personnel, healthcare services, and other participants operate within a common networked computing environment and interact by sharing information and its semantics. We contribute design solutions to construct services as smart m-Health spaces.

Problems of investigations in sphere of electromagnetic fields impact on biological objects 电磁场对生物物体影响的研究问题

Problems of the investigations in the sphere of electromagnetic fields (EMF) impact on biological objects (BO) in the TEM-cell have been considered. Researches on BO exposed to EMF (cell cultures, plants, worms, insects, animals) have been systematized. Features of devices for climatic (temperature, humidity, CO2 and nutrients) and electromagnetic exposure needed for research of specific BO have been described. The need for temperature control and video monitoring during the research on BO exposed to EMF has been justified. The results presented may be useful for development of research methods for investigation of the EMF impact on specific BO, as well as for the development of technical requirements in order to provide devices useful for such studies.

Multifunctional clinical decision support system based on clinical practice guidelines 基于临床实践指南的多功能临床决策支持系统

A multifunctional system for clinical decision support (SACRAL) is presented. The system supports the decisions based on recommendations from clinical practice guidelines and individual risk factors, generates reports, provides statistical data analysis, automatic generation of quality registry and audit which includes quality indicators. SACRAL is implemented in a general hospital, has 5 modules “Cardiology”, “Oncology”, “Neurology”, “Surgery” and “Preventive Medicine”), including 52 scenarios. Proposed concept of multifunctional clinical decision support system with automated register generation and clinical audit has proved its practical value for routine use in a general hospital. It can be used universally for various clinical domains and any clinical practice guidelines.

Bioengineering system for research on human emotional response to external stimuli 用于研究人类对外界刺激的情绪反应的生物工程系统

Multichannel biotechnical system is designed to study human emotional reactions to external stimuli by recording and analyzing speech samples test or electroencephalogram signals. The system allows you to create complex stimuli for the perception that the subject uses a visual, acoustic or olfactory system. For registration of biological signals earmarked voice recording and electroencephalogram (EEG). Since stimulus material acts on three different channels of human perception, in the biotechnical system includes means of visual, audio-visual and olfactory stimulation. All system components are integrated into the control unit shown in the form of a personal computer with pre-installed special software. A feature of the system work scenario is the registration sample speech only after the marked changes in the electroencephalogram caused by the perception of the appropriate stimulus. This approach increases the credibility of the sampling of speech, which can then be used to form the rules of recognition of types of emotions. With this system were formed multimodal database comprising 280 different russian speakers phrases and corresponding EEG recording. The goals of experiments are studies attribute spaces are invariant to the physical nature of the signal and adequately reflects the emotional response of the subject, and testing the hypothesis of invariance of attributes allocated to the national characteristics of speech signals. In order to study the dynamics of emotional response developed a method that allows to explore a variety of discrete portions of biological signals. The technique is based on the idea of shifting the window, which is carried out static analysis. For each window are used as classical methods of calculation of the spectrum of the signal, and the methods of nonlinear dynamics. System showed enough high accuracy interpreting both the EEG pattern (83%) and with the speech samples (96 %) of the multimodal base emotions and objects taken from German base Emo-DB (92%). Evaluation of different emotional states a person can be used in the diagnosis of psychosomatic and psychomotor disturbances.

An ontological-based monitoring system for patients with bipolar I disorder 基于本体的双相I型患者监测系统

Our aim is to provide a patient monitoring system that integrates a Clinical Decision Support System (CDSS) and an Electronic Health Record (EHR) that assist psychiatrists and primary care physicians to tackle existent health needs of mental illness related to the treatment and management of bipolar I disorder (BDI). Our monitoring system consists of an EHR system based on the Health Level Seven Reference Information Model (HL 7-RIM) and an ontological-based CDSS leveraging the Semantic Web capabilities. Based on the evidence-based clinical guidelines and patients' health records, the monitoring system is developed to encode and process this information and subsequently to assign recommendations of choices and alerts to clinicians for improved mental health care. Considering the clinical guidelines germane knowledge, as well as issues of patient's health record, the monitoring system can support a personalized decision-making for bipolar I disorder longitudinal course. We propose AI-CARE as an online monitoring tool that may offer useful guidance in clinical practice.

Development of automated methods for the critical condition risk prevention, based on the analysis of the knowledge obtained from patient medical records 基于对从患者医疗记录中获得的知识的分析,开发危重状况风险预防的自动化方法

This paper describes the methods of development of ontologies and ontological models in medicine. A four-level model of knowledge representation is suggested. Algorithms for prevention of critical condition risks and complications are developed on the basis of ontological methods of knowledge representation. The work is based on the model-theoretic approach to representation of medical knowledge. The knowledge is represented through partial atomic diagrams of algebraic systems, as well as representation of patient's case data via Boolean-valued models. Ontology and ontological model of the “spinal deformity and degenerative diseases of the spine” subject domain have been developed. The ontology model contains: a) universal knowledge that is true for all patients, b) data on specific patients, and c) estimated (fuzzy) knowledge that is used for recommendations for doctors. Estimated knowledge is a set of probabilistic hypotheses on the possibility of emergence of patient's critical condition or complication. An algorithm for generation of estimated (fuzzy) knowledge, based on the analysis of medical records, has been developed. A software system for generating recommendations to prevent and reduce the risk of patient's critical condition has been implemented. The software system has been tested on the data of patients with spinal deformity and degenerative diseases of the spine.

The knowledge is represented through partial atomic diagrams of algebraic systems, as well as representation of patient's case data via Boolean-valued models. Ontology and ontological model of the “spinal deformity and degenerative diseases of the spine” subject domain have been developed. The ontology model contains: a) universal knowledge that is true for all patients, b) data on specific patients, and c) estimated (fuzzy) knowledge that is used for recommendations for doctors. Estimated knowledge is a set of probabilistic hypotheses on the possibility of emergence of patient's critical condition or complication. An algorithm for generation of estimated (fuzzy) knowledge, based on the analysis of medical records, has been developed. A software system for generating recommendations to prevent and reduce the risk of patient's critical condition has been implemented. Remote monitoring of children with asthma, being treated in multidisciplinary hospital 对在多学科医院接受治疗的哮喘患儿进行远程监测

The increase in the prevalence of allergic diseases requires the active implementation of the capabilities of modern technology in the process of providing health care for children with asthma. The paper describes the system for remote monitoring for asthma patients receiving treatment atthe Scientific Center of Children Health. We describe the method of estimating of the state of patients, the method of selection of medication therapy, the knowledge base and the rules implemented in it, interaction with the mobile application. The use of the system is necessary to improve disease control and prevention of exacerbations and improve quality of life as a sick child and his family.

Principles of organization and control of multielectrode neuro-electrostimulation device 多电极神经电刺激装置的组织与控制原理

The basic principles of the multielectrode neuro-electro stimulation device and its control program are shown. Device includes a spatially distributed current pulses field generation unit that works with help of cathodes and anodes multielectrode arrays, first lead ECG registration unit, and the control unit. The control unit allows changing the electric field structure and biotropic field parameters in real-time mode. The device receives and transmits data to PC via USB. Laboratory analysis of device treatment capabilities shows that the device may be used in treatment process of prenosological conditions and in otorhinolaryngology, ophthalmology, neurology, psychiatry, and for rehabilitation of soldiers.

Feasibility of a capnometry device for respiratory biofeedback in patients undergoing coronary artery bypass graft surgery 冠状动脉搭桥手术患者呼吸生物反馈测量装置的可行性

Background: Psychophysiological patient's state during hospital stay influences coronary arterial bypass graft (CABG) surgery outcomes. Biofeedback is used in the treatment of posttraumatic stress disorder and anxiety disorders. The aim of the current study was to evaluate feasibility of respiratory biofeedback (RB) in patients undergoing CABG surgery during hospital stay. Methods: Before CABG surgery all patients (n=93, aged 40 to 77 years) were divided into three groups after performing capnography and trial course of RB. The first group (usual care or USU group, n=48) consisted of patients who refused additional psychological support and RB, the second group consisted of patients who agreed to get psychological support (PSY group, n=25), and the third group were patients who accepted RB with psychological support (RB group, n=20). The task of the RB training for the participants was to maintain end-tidal partial pressure of carbon dioxide 10-20 % above the initial level. Results: RB was chosen by 21.5 % of the patients, psychological support was chosen by 26.9 % of the patients. Half of the patients (51.6 %) refused the offered additional programs. There were breathing frequency reduction and expiratory time increment in the RB group in contrast to other two groups at discharge. The RB group demonstrated a significantly shorter length of postoperative hospital stay (mean 9.5 [SE 1.0] days) than the USU group (13.9 [0.7]) and the PSY group (12.4 [0.8]). Conclusion: RB appears to be a feasible method of rehabilitation of the patients undergoing CABG surgery during hospital stay. Future randomized research is needed to examine effectiveness of the method.
