Abstract Healthcare workers are more vulnerable to burnout due to the fact that they often face stressful situations that involve high levels of risk and emotional involvement, especially if they are exacerbated by patients they provide care to. The activity in the medical field is not only physically demanding, but also socio-emotional, being necessary a better understanding of the socio-emotional aspects present in the employment relationship. The health sector is facing continuous changes and reforms, periods of uncertainty when the fulfilment of the psychological contract is less probable. We propose the description of a relational model between the psychological contract with work engagement, burnout, organizational support and the intention to leave the organization for the medical staff. The results show that organizational support influences work-related outcomes and emphasizes the importance of examining organizational contexts, such as contract status, to understand the influence of support on work involvement and burnout.

Abstract Platelet indices have been linked to the severity and prognosis of coronary artery disease, but a very small number of studies assessed them under dual antiplatelet therapy after percutaneous coronary intervention (PCI). The aim of this study was to evaluate changes in mean platelet volume (MPV), platelet distribution width (PDW) and platelet-large cell ratio (P-LCR) in these patients. A number of 437 patients with stable coronary artery disease, 131 patients with non ST segment elevation acute coronary syndrome and 151 patients with ST segment elevation myocardial infarction were included in the study. There was no difference between the three groups regarding platelet indices prior to PCI. Follow-up data was available for 181 patients, at a mean follow-up time of 69 (46-98) days. MPV, PDW and PLC-R were similar to those prior to PCI in all three groups. Regarding the P2Y12 inhibitor, clopidogrel or ticagrelor, there was no difference at follow-up between platelet indices irrespective of the indication for PCI.

Abstract Hand pain is a common complaint in the hemodialysis patient on arteriovenous fistula. The problem is related to its etiology: nervous or vascular, which requires different therapeutic solutions. Ischaemic etiology involves a deviation of the distal flow in the anastomosed vein, with the decrease of the distal flow and the installation of distal ischaemia. Nerve etiology involves compression of the median nerve in the carpal tunnel. The purpose of the paper is to present a case study of diagnosis and treatment of median nerve compression in hemodialysis patient.

Abstract The use of psychostimulants is more and more common lately, especially in the medical guild. The term “burn-out” is an occupational phenomenon, not a disease, and 25-60% of doctors in different specialties have the burn-out syndrome. The method was to complete a questionnaire-type test by 204 medical residents. The results shows us that 72.1% (147) of the respondents drinks coffee, 25.9% of them smokes cigarettes during hospital hours and 13.7% consumes energy drinks. Furthemore 5.9% drinks alcohol during shifts. Among of all, those who are consumers of at least one psychostimulant substance, the risk of them having at least one of the symptoms of burnout is 1.3 times higher than those who do not consume anything at work (RR-1.3453, p = 0.0152). Psychostimulant substances are consumed to improve the efficiency and resistance at work. It’s important to manage the time in order to avoid extreme fatigue and distress.

Abstract Being considered a rare form of malignant neoplasm, stromal tumours are part of sarcomas group, they account for ~5% of all sarcomas (1), which develop from the supporting tissues of the body and have as a starting point special cells located in the wall of the gastrointestinal tract, called interstitial cells - described by Cajal. Although this type of tumour formations poses the problem of diagnosis, when it is made early and surgical and oncological treatment is prompt, the prognosis improves considerably.

Abstract Nowadays, at work, many people spend more than half of their entire day working in a sitting position. Maintaining this position for a long period of time is a risk factor for health. The results of various studies have shown associations between maintaining a prolonged sitting position with morbidity due to chronic diseases, such as cardiovascular disease, diabetes, metabolic syndrome, obesity, etc. And, as we have already mentioned in the first part of this study, dental technicians are part of this category of staff, who carry out most of their specific activity, in a sitting position.

Abstract The most common causes of non-bilious vomiting in infants are hypertrophic pyloric stenosis and pylorospasm. Differential diagnosis between the pathologies establishes the correct treatment. The aim of our study is to present the management of the pathologies cured in our department. The longer the duration of vomiting symptoms the higher muscle thickness was shown. Dehydration and lethargy was associated with higher muscle thickness. There were no intraoperative or postoperative complications recorded in our cases. Ultrasonography is the gold standard in imaging technique but it requires an experienced radiologist. In case of negative physical examination and ultrasonography, but high suspicion of the diagnosis a fluoroscopy may be required. The correct diagnosis avoids unnecessary surgery combined with general anesthesia.

Abstract Numerous studies in the last decades report a frequent association of marginal periodontal disease with some systemic metabolic disorders, such as diabetes or obesity. The prevalence of these diseases is growing alarmingly both globally and in our country. Recent data from the literature confirm that advanced forms of periodontal disease can influence the evolution of type 2 diabetes. This may provide a premise for better glycemic control.

Abstract The alarming spread of the novel Coronavirus necessitated the cessation of elective therapeutic procedures in most health-care facilities. This strategy has limited the spread of the virus, but with a huge socio-economic impact. For this reason, the resumption of elective surgery in the context of the coronavirus pandemic is a difficult, but necessary process. Addressing this delicate situation requires interdisciplinary collaboration, so as to ensure high quality medical care for all patients, with consideration to protection of the staff involved in the care of the surgical patient.

Abstract Orbital pseudotumor represents a benign inflammatory disorder of the orbit, with unspecified etiology, whose clinical presentation can mimic the ocular pathology of Basedow disease, namely Graves’ ophthalmopathy, the two of them representing two of the most common orbital conditions. Imagistic methods and laboratory analyses can establish the diagnosis through orbital MRI images specific for the orbital pseudotumor, especially by dosage of the TSH receptor antibodies (TRAb) which will have increased values in Basedow disease with ocular damage. In both situations, the treatment is represented as a first therapeutic line by administration of corticotherapy in high dosage orally or intravenously, with favorably evolution, which is also observable in the case of our patient who was diagnosed with orbital pseudotumor based on MRI images, with associated thyroid dysfunction with negative TRAb, in which the pulse therapy with Solumedrol has determined the improvement of ocular symptoms and signs.
