Multi-System Inflammatory Syndrome in Children (MIS-C) is a new diagnostic entity that appeared with the Covid-19 pandemic, and that only concerns the pediatric age. Early diagnosis and early immunomodulatory treatment lead most of the time to a complete resolution of the pathology. We present a case of MIS-C, whose main clinical feature was neck pain due to inflammation of retropharyngeal tissues. The use of anakinra was critical for healing.

Background: Postpartum hemorrhage (PPH) is a leading cause of severe maternal morbidity and mortality worldwide which leads to massive blood loss. Coagulation abnormalities in response to severe trauma or infection are a latent cause that might aggravate PPH. Case Presentation: A 39-week menolipsis of a 26-year-old puerpera appeared lacking amniotic fluid and uterine infection after examination. During the cesarean section, the patient manifested fever, massive hemorrhage, and shock. The low coagulation of the PPH patient was diagnosed by thromboelastography (TEG) guided with heparinase (type I). According to the sequential monitoring via the TEG guided assay, the coagulopathy and hyper-heparinization were obviously shown. Concurrent protamine correction for the patient’s coagulation abnormality gradually resulted in a stable condition after 4 hours of emergent treatment. This setting revealed that TEG-guided determination of endogenous heparin and subsequent infusion of protamine effectively reverted the syndrome of PPH. Conclusions: This is an investigation of the PPH syndrome with infection patient suggests that hyper-endogenous heparinization should be clinically taken into consideration for low coagulation.

Methods: This was a multicenter, open-label, single-arm, clinical trial which included adult patients with nasal obstructive symptoms due to chronic rhinitis/rhinosinusitis and a minimum Visual Analog Scale (VAS) score of 25 mm and/or a minimum raw Nasal Obstruction Symptom Evaluation (NOSE) score of 2 on any of items 1, 2, 3, or 5. Enrolled subjects were instructed to apply the study device bilaterally to the external nasal nerve at least three times per day for 30 seconds until the 2-week followup, then twice per day and as needed thereafter. Subjects were evaluated at Days 3, 14, and 30 following the initial baseline visit. The primary efficacy endpoint was the change in nasal obstructive symptoms from baseline to Day 30 as measured by the VAS and NOSE instruments. Safety was assessed by the incidence of device-related Adverse Events (AEs).

Carney Complex Syndrome is a rare genetic disorder that affects multiple organs and systems, including the endocrine glands, heart, and skin. The transmission of the syndrome is autosomal dominant, the gene involved in the mutation, in more than 60% of cases, is the PRKAR1A gene. We aim to present a 38-year-old female diagnosed with Carney syndrome since 2011. During April 2022, according to the cardiac screening plan, she underwent a transthoracic echocardiography, which did not show any cardiac tumors. However, in December 2022, the patient reported symptoms like dyspnea and palpitations. Immediately a new transthoracic echocardiography was performed, which revealed a large oval mass with heterogeneous echotexture and irregular margins in the left atrium, with a broad base of implantation at the interatrial septum, measuring 5 × 5, 5 cm. The mass cause obstruction to filling because was engaging in systole in the left ventricle. Additionally, a PFO, a common anatomical variant of interatrial septum, was found. The patient underwent cardiothoracic surgery to remove the myxoma and close the PFO via right mini thoracotomy, few days after. The surgery was successful, and the patient had good results in subsequent follow-up. In this article we aim to focus attention on the need to perform echocardiographic follow-up in a short time in patient with a familiar history of neoplasm involving myxoma, to preserve the patient from major compliances, especially reduce the risk of sudden cardiac death.

A 28-year-old male was referred by his primary care physician for workup of possible oral candidiasis. He had been diagnosed with asymptomatic HIV-infection 18 months earlier and had undetectable viral load under antiretroviral treatment. The patient reported a history of multiple unprotected sexual encounters over the preceding 3 months. Physical examination showed painless and confluent whitish mucous patches (about 2 cm long) with erythematous border on the left soft palate which did not scrape off with a tongue depressor (Panel A). A biopsy specimen from the damaged mucosa was obtained and revealed hyperplasia of the epithelium and a dense inflammatory infiltrate in the corion, composed mainly by plasma cells (Panel B and C). Immunohistochemistry highlights numerous treponemal spirochetes, brown chromogen (Panel D and E). Treponemal tests (EIA and TPPA) were reactive and Rapid Plasma Reagin test (RPR) was positive (titer, 1:32). A diagnosis of syphilis was made. The patient was initially treated with intramuscular penicillin G benzathine. At 3-month follow-up he had complete resolution of palate lesion.

Elderly burn patients are at increased risk for wound healing complications, as their injuries are more likely to become deep and wound healing is impaired. Furthermore, higher age is a known risk factor for burn wound infection. New biologic matrices are promising tools in the armamentarium of wound care specialists in application of hard-to-heal wounds. We present a case of an elderly burn patient with chronic wounds that were ultimately healed using marine omega3 wound matrix for wound conditioning and meek micrografting for closure. As the patient general health status did not allow surgery under general anaesthesia, skin grafting was performed bed-side under topical anaesthesia.
