Decrease Treatment Burden following the Application of ADCES7 and WHOICOPE in a Diabetic Frail Elderly Lady with Multimorbidity 应用ADCES7和WHOICOPE减轻多病糖尿病体弱老年妇女的治疗负担

We are living in a world of aging, and the elder population is increasing rapidly. According to the World Health Organization (WHO), the number of people aged 60 or older is about 1 billion in 2019. The numbers are expected to rise to 1.4 billion by 2030 and 2.1 billion by 2050 [1]. Elderlies are frail and they are prone to many diseases, including psychological disorders. Diabetes is also a global problem, and its prevalence increases after middle age [2]. To help clinical health providers to get better pictures of the frail diabetic elderly subjects holistically, many programs or models have been developed. One of the most used assessment and management tools is the integrated care for older people (ICOPE) guideline developed by the WHO [3]. ICOPE was set with person-centered goals, screening for loss in a range of domains of intrinsic capacity and assessing health and social care needs to develop a personalized care plan. The Association of Diabetes Care & Education Specialists 7 Self-Care Behaviors (ADCES7 Self-Care Behaviors®) is also a person-centered approach to diabetes by acknowledging the whole person in the context of their life and relationships which form the framework for diabetes care to deal with people with diabetes [4]. By using the ICOPE and ADCES 7 Self-Care Behaviors®, we were able to evaluate the patient rapidly and further help manages their problems. Here, we present a frail diabetic elderly subject with multimorbidity who had recent medical deterioration. This case is also a good demonstration of holistic care to minimize the disease and treatment burden in a primary care setting.

Use of Next-Generation Sequencing Techniques in the Accurate Diagnosis of Neural Crest Cell-Derived Tumors 新一代测序技术在神经嵴细胞源性肿瘤准确诊断中的应用

The increasing adoption of next-generation sequencing (NGS) techniques in daily practice has led to the incorporation of molecular information in the diagnosis and management of cancer patients. Taking advantage of these new tools could clearly impact the outcome of cases we present the case of a 39-year-old woman who attended the emergency department for fever and dysphagia. Computed axial tomography revealed a solid lesion in the subcarinal region. Histologically, the mass showed a tumor with proliferation of spindle cells containing elongated, hyperchromatic nuclei and brownish pigment in the cytoplasm. On immunohistochemical staining, the cells were positive for S-100, SOX10, P16 and HMB 45 and negative for CKAE1/AE3 and actin. Consequently, given the location and clinical findings, the mass was tentatively diagnosed as locally advanced esophageal mucosal melanoma. Further genetic testing with an NGS panel identified EWSR1-ATF1 fusion, resulting from translocation t(12;22)(q13-14;Q12). These new data were combined with all the available information, and the patient was diagnosed with clear cell sarcoma, which was treated with radical-intent radiotherapy. The use of NGS platforms to determine the genetic profile of the tumor facilitated the correct diagnosis of this rare neoplasm with melanocytic differentiation, an entity that presents a broad differential diagnosis with other neural crest cell-derived tumors. As a result, the patient received proper treatment based in precision medicine in line with the new molecular findings described.

Introduction and Importance: High-flexion prostheses can be associated with a greater risk of early aseptic loosening of the femoral component than standard prosthesis. Case presentation: We describe two patients in whom high-flexion total knee arthroplasty (TKA) was performed using the anterior referencing (AR) method that resulted in early aseptic loosening of the downsized femoral component. Conclusion: Excessive bone resection of the posterior femoral condyle by additional downsizing of the femoral component during high-flexion TKA using the AR method could lead to early failure of the femoral component.

Perceptions and Experiences of Rare Diseases among General Practitioners: An Exploratory Study in NI 全科医生对罕见病的认知和经验:NI的一项探索性研究

Objective: To gain an understanding of GP’s perception and experience of rare diseases. Design: This is an exploratory study with GPs from Northern Ireland. Setting: An online survey was hosted within Smart Survey, available for 6 weeks in early 2019, which queried: GPs career to date, interaction with rare disease patients, challenges faced, the exchange of information with patients, priorities for the future, support for such patients and finally training. The survey contained open-ended and closed-ended questions and both qualitative (narrative) and quantitative analyses were carried out. Participants: Sixty-six GPs engaged with the survey with 15 completing the survey. Results: Many frustrations were mentioned by GPs when caring for such patients including finding a diagnosis, lack of specialist services and lack of awareness of rare diseases. Seventy-nine percent of GPs responding did not feel adequately trained to care for patients with a rare disease, while 93% were interested in undertaking training to enable them to improve their care for rare disease patients. Conclusions: More effective and convenient ‘rare disease’ focused training programs would help GPs manage patients with rare diseases. Enabling efficient, effective communication between GPs and individual medical specialists is of paramount importance to give GPs access to information on how to effectively manage patients living with a rare disease. Awareness should be raised for effective support mechanisms such as charities and support groups for rare disease patients.

Evidence Based Physical Medicine in Internal Medicine 内科循证物理医学

Introduction: The teaching of physical examination is based on medical textbooks. In the era of technology and evidence-based medicine, clinician should be able to perform and accurate physical examination. In this review, we propose a new way of doing physical examination with accuracy. Methods: We searched PubMed from 1980 to 2022. We included studies that compared physical examination with imaging, electrophysiological studies and intraoperative findings. Results: We have identified a bare minimum for head to toes examination. The content is vital signs, eyes inspection, mouth inspection, lymph nodes palpation, thyroid palpation, lungs examination (inspection, percussion and auscultation), heart examination (inspection and auscultation), abdomen (inspection and palpation), vascular examination (dorsalis pedis and posterior tibial artery palpation), neurologic examination (eyes movement, facial strength, tongue, thumb rolling test, ankle reflex, gait). Conclusions: Physical examination of human major systems can be done within 10 minutes with accuracy.
