Meta-analysis of the epidemiology of microvascular complications in people with Type 2 diabetes in mainland China 中国大陆2型糖尿病患者微血管并发症流行病学荟萃分析

Objective: This study aims to estimate the overall prevalence of microvascular complications of type 2 diabetes in mainland China, and to identify any associated risk factors. Methods: A systematic literature search was conducted to collect the prevalence information of microvascular complications over the past 5 years (2014 -2019) in CNKI, Wanfang, PubMed and the Cochrane Library. A meta-analysis was conducted using R3.4.3 following normal transformations given the skewed raw data. Due to high heterogeneity, a random effect model was selected. A meta-regression analysis was performed to investigate risk factors.

Diabetic Peripheral Neuropathies (DPNs) from Basic and Clinical Aspects 糖尿病周围神经病变(DPNs)的基础和临床

Diabetes has been one of the crucial diseases worldwide, which has to be controlled adequately for long years. It has three diabetic complications of micro-angiopathy such as neuropathy, retinopathy and nephropathy. Among them, Diabetic Peripheral Neuropathies (DPNs) are most prevalent to manage in primary care setting. In this article, recent topics concerning DPNs are introduced [1]. DPNs have a variety of symptoms and signs, then DPNs are often described in plural forms [2]. DPNs are classified into two categories, which are local and general. The former includes mononeuropathy and multifocal neuropathy. The latter includes diabetic polyneuropathy (DPN) and others. DPN has Distal Symmetric Polyneuropathy (DSPN) and Diabetic Autonomic Neuropathy (DAN) [2]. For examples, DSPN shows bilateral numbness of extremities and DAN shows Orthostatic Hypotension (OH).

When a diabetes treatment is changed, patients are eager to see their FBG and A1c changes as a result on the new treatment. We examined these changes in 20 subjects with Type 2 diabetes for 1 week prior to starting Lysulin and then for the following 30 days. While both FBG and A1c drop in less than 1 month, the change in A1c was more rapid than the change if FBG. Possible reasons for the differences between FBG and A1c are presented

Pharmacists can be part of the healthcare team to help people with diabetes and prediabetes to better health. Obesity and type 2 diabetes are a worldwide epidemic and a problem that can be helped dramatically with lifestyle changes (diet and exercise) combined with treatment with drugs and supplements. Lysulin is a patent-pending nutritional supplement that contains lysine, zinc and vitamin C and has been shown in double blind placebo controlled studies to help people with prediabetes and type 2 diabetes to better glycemic control and lower their HbA1c. The pharmacist needs to be aware of this breakthrough and inform his customers of the availability of this new product for improving their health. In addition, HbA1c testing at the pharmacy or at home can be very useful in helping people with diabetes know how well their diabetes management program is working.

Gene therapy has entered a new era with the dawn of CRISPR/Cas9 technology which though were always available in nature but rediscovered to tame into a real-tlife genome editing tool. With the modernization upsurge and changes in ways the “homo sapiens” survived on this planet from hunger to current era of exuberance has led to multiple metabolic issues like type-2 diabetes. Notwithstanding the rapid emergence of medication to suppress the hyperglycemia and insulin resistance associated with this menace, need has defi nitely emerge to fi nd more personalized and curative dimensions to therapeutics of type-2 diabetes mellitus.

Diffuse Lymph-Nodes Microangiopathy as Concurrent Cause of Immunodeficiency in Long-Term Insulin-Dependent Diabetic Patients 弥漫性淋巴结微血管病变是长期胰岛素依赖糖尿病患者免疫缺陷的并发原因

Immune abnormalities have been demonstrated in vitro models in genetic (type1) autoimmune (type 2) and metabolic (type 1 and type 2) Insulin-Dependent Diabetes Mellitus (IDDM). However, the precise reason for increased susceptibility to frequent and protracted infections in diabetic patients is still unclear, despite a multitude of in vitro studies which have focused on the metabolic and functional modifications of immune cells Diabetes microangiopathy, which is a peculiar alteration of the disease, has been extensively described in the retina, renal glomeruli, skin, skeletal muscles, peripheral nerves, and other organs but not in lymph nodes. We report our histological and immunohistochemical observations in lymph-nodes removed in multiple sites during autopsy from four patients with long-term IDDM, severe lymphocytpenia and several infectious diseases during their life.The peculiar microangiopathic modifications made by presence of hyaline substance thickening basal membrane of thin vessels and capillaries appear concurrent with lymphodepletion of B and T cells dependent areas of lymph-nodes and with jointed marked reduction of Follicular Dendritic Reticulum Cells (FDRC). Indeed microangiopathy further compromise the traffic and diapedesis of T and B lymphocytes may prevent the transformation of endothelial cells into FDRC with severe immune failure of lymphoid follicles.The histological and immunohistochemical data in this study could provide additional insights into the complex problem of the immunodeficiency in diabetic patients.

The Role of Hormones in the Regulation of Glycogen Metabolism in the Clawed Toad Xenopus Laevis (Daudin) 激素在爪蟾糖原代谢调节中的作用

The hormonal regulation of amphibian glycogen metabolism was studied in Xenopus laevis as a typical member of the anurans (tailless amphibians).The main focus of this study was given to the effects of various hormones on the glycogen/glucose balance in adult toads. We determined biochemically the liver and muscle glycogen contents as well as the blood glucose and lipid levels for a number of hormones and also diabetes inducing substances. Additionally, we examined ultrastructure changes in hepatocytes induced by the various treatments, and also investigated the activity of carbohydrate-relevant enzymes by histochemistry. With one exception, the liver glycogen content of Xenopus remained basically unchanged by the treatments or was even slightly enhanced. Only human chorionic gonadotropin, through which the vitellogenic response is triggered, prompts a significant decrease of liver glycogen in females. Under the same conditions the male liver glycogen content remained stable. Muscle glycogen contents were not affected by any of the treatments. Blood glucose and lipid levels on the other hand were elevated considerably in both sexes after application of either epinephrine or cortisol. The ultrastructural examination revealed a proliferation of Rough Endoplasmic Reticulum (RER) in hepatocytes from epinephrine treated toads of both sexes as well as from HCG treated females. By histochemistry, we detected an elevated glucose-6-phosphatase activity in the hepatocytes from toads treated with either epinephrine or cortisol. These treatments also led to enhanced glycogen phosphorylase activity in males, and to a slightly elevated glyceraldehyde-3-phosphate dehydrogenase activity in females. Our results show that the hepatic glycogen is extremely stable in adult Xenopus. Only vitellogenesis causes a marked utilization of glycogen. Since the blood glucose levels are elevated in epinephrine or cortisol treated toads without the liver glycogen being affected, we conclude that either protein and/or lipid metabolism are involved in carbohydrate metabolism in Xenopus laevis.

With one exception, the liver glycogen content of Xenopus remained basically unchanged by the treatments or was even slightly enhanced. Only human chorionic gonadotropin, through which the vitellogenic response is triggered, prompts a significant decrease of liver glycogen in females. Under the same conditions the male liver glycogen content remained stable. Muscle glycogen contents were not affected by any of the treatments. Blood glucose and lipid levels on the other hand were elevated considerably in both sexes after application of either epinephrine or cortisol. The ultrastructural examination revealed a proliferation of Rough Endoplasmic Reticulum (RER) in hepatocytes from epinephrine treated toads of both sexes as well as from HCG treated females. By histochemistry, we detected an elevated glucose-6-phosphatase activity in the hepatocytes from toads treated with either epinephrine or cortisol. These treatments also led to enhanced glycogen phosphorylase activity in males, and to a slightly elevated glyceraldehyde-3-phosphate dehydrogenase activity in females. Our results show that the hepatic glycogen is extremely stable in adult Xenopus. Only vitellogenesis causes a marked utilization of glycogen. Background and Purpose: Sleeve gastrectomy is a major therapy of morbid obesity, but recent reports suggest that its effects on weight loss are improved when patients increase their muscular activity, while a weight regain may occur in more than 30% of patients after 40 months. Exercise is an effective mean of preventing weight regain. In a recent preliminary study, we reported that exercise training targeted at the level of maximal lipid oxidation (LIPOXmax) improved weight loss and weight stabilization at 30 months. This study extends this previous one and aims at describing the kinetics of weight loss on 60 months after sleeve in people performing or not a LIPOXmax training and to see whether this variety of training exerts a satietogenic effect in sleeve patientsas has been evidenced in nonoperated obese subjects.

Study of Vitamin D Status and its Correlation with Glycated Haemoglobinin Type 2 Diabetes Mellitus 维生素D水平及其与2型糖尿病糖化血红蛋白相关性的研究

Introduction: Few published researches have surveyed the correlation between Vitamin D status and glycaemic control in type 2 diabetes mellitus (T2DM). The present study was conducted to investigate the status of vitamin D and its correlation with glycated haemoglobin in type 2 diabetes mellitus.Method: A cross-sectional single centre study was conducted in 2440 patients with T2DM attending the Diabetes Centre at King Fahad Armed Forces Hospital, Jeddah, Saudi Arabia between January 2018 and December 2018. Eligible patients were 20 years or older.Results: There were 2440 patients with T2DM. Vitamin D deficiency (25-OHD<50 nmol/l) was found 47.5%. Patients with HbA1c<7% were younger than patients with HbA1c 7%-9% and >9% (55.3 ±16.0 vs. 58.4 ±15.2 vs. 57.1 ±15.4 respectively, p<0.0001). The mean 25-OHD concentration was statistically significant lower in patients with Hba1c>9% compared to patients with Hba1c<7% and 7%-9% (49.7 ±27.0 vs. 61.8 ±31.4 vs. 56.9 ±28.8 respectively, p<0.0001). The frequency of vitamin D deficiency was statistically significant higher in patients with Hba1c>9% compared to patients with Hba1c<7% and 7%-9% (40% vs. 48% vs. 61% respectively, p<0.0001). The frequency of vitamin D deficiency was upward across HbA1c groups as age advanced with highest frequency of vitamin D deficiency was found to be statistically significant in HbA1c>9% compared to HbA1c< and 7%-9% groups in the age group 50-59 years and ≥60 years with males most frequently predominant than females in all age group associated with HbA1c 7%-9% and 9%. HbA1c was significantly positively correlated with age whereas 25-OHD concentration was significantly negatively correlated with age.Conclusions: We report vitamin D deficiency and its inverse association with Glycated Haemoglobin in type 2 Diabetes Mellitus.
