Patients suffering from either severe mental illness or alcohol use disorder thus lose 13 to 30 years of life expectancy. The leading cause of death is insufficient detection, diagnoses and treatment of cardiovascular disease. Several interventions aimed at reducing the risk of cardiovascular disease have been developed and tested in randomized controlled trial studies. The interventions tested so far has not shown to have a long-term impact on the health of these patients. Only a few studies have investigated this particular patient group’s perspectives and recommendations on how to prevent and treat risk factors for cardiovascular disease. The aim of this study was to learn about the patients’ perception of physical health problems, have them identify potential barriers for lack of treatment and prevention of cardiovascular disease, and to obtain knowledge for future interventions. The study was designed as a qualitative study and consisted of semi-structured interviews with patients. Two main themes, motivation and continuity/flexibility, and a number of subthemes were identified. Patients wanted healthcare professionals to be more aware of somatic health and that physical health could be better integrated into psychiatry and in the alcohol treatment facility and the possibility of flexible courses adapted to the individual patient.

Co-Designing Health Care Solutions with Patient Representatives and Clinicians in a Large Acute Hospital Setting: Process and Engagement 在大型急性医院环境中与患者代表和临床医生共同设计医疗保健解决方案:过程和参与

The benefits of involving patients and wider members of the public as partners in care are being increasingly recognised internationally. Co-design is one of the methods reported to promote patient-based health service improvements and offers a participatory approach to engage patients and citizens in solving health care challenges. However, current limitations are levelled at this corpus of work, indicating a lack of sustainability and substantive evidence of any known associated processes that can yield sustainable longer-term patient benefit. This service improvement project was underpinned by a Human Centred Design (HCD) methodology incorporating the Design Council’s process Discover, Define, Develop and Deliver [1]. This assisted in providing a participatory framework of co-produced work over a twelve-month period with three clinical pathway teams, Stroke, Children and Young People (CYP) and Learning and Developmental Disabilities (LD.) Meeting specific project objectives, patient-based projects were developed using a toolkit and Collaborative Action Plans that steered involvement throughout. Evaluative results elicited three themes, generating a product idea together, acknowledging the contribution of all, barriers and challenges. Within this, the clinicians and patient representatives reported the value of having a safe space to carry out experienced based work with their respective patient representatives. Additionally, they reported the chosen HCD framework guided the process of engagement determining co-produced health care solutions to patient derived challenges. Conclusions are drawn that suggest further work and research is required to testbed the ‘how to’ processes associated with successful co-design in health and social care. This could provide an empirical basis for the value and process associated with sustainable human centred design required at both a micro and macro level of healthcare.

Effectiveness of Comfort Theory Model Based on Holistic Care for Hospitalized Children with Sickle Cell Disease Crises 基于舒适理论模型的镰状细胞病危重期住院患儿整体护理效果研究

Background: Children’s experience of hospital when suffering acute sickle cell crises can be improved by a comfort approach to nursing care. Aim and Design: The true experimental design approach was used to investigate the efficiency of comfort theory in improving the comfort level model for children with SCD crises. Sample and setting: A convenience sample was taken from children admitted at pediatric inpatient department of Benha University Hospital and Specialized Children Hospital, Benha city, Egypt during sickle cell crises. Data collection tool: Data collection was undertaken in a number of ways, including a demographic information form, patient Comfort Behaviors Checklist, Comfort Daisies Scale, and General Comfort Questionnaire, both pre- and post-theory intervention, based on the nursing process. Results: When compared to a control group, significant differences were observed in children with SCD crises post-comfort theory application. There was also a positive correlation between the children’s state of comfort and post-comfort theory intervention. An association was also noted between children in secondary education and at a higher economic level in comfort state at second observation, which was deemed to be the best comfort achievement. Conclusion and recommendations: Nursing care modelled on comfort theory can significantly improve children’s experience of hospitalization during sickle cell crises. The nursing of children should be holistic, and evaluate their physical, emotional, and psychological needs when addressing their level of comfort.
