Postoperative Pain and Microbiological Evaluation After Using 810nm Diode Laser Disinfection in Single Rooted Teeth with Necrotic Pulp and Periapical Radiolucency: Randomized Control Clinical Trial 810nm二极管激光消毒单根牙髓坏死及根尖周放射率术后疼痛及微生物学评价:随机对照临床试验

Purpose: This study aimed to evaluate the treatment outcome after using diode laser disinfection of root canals in necrotic teeth with periapical radiolucency. Subjects and Methods: Thirty patients of age range between 20 and 50 years old were involved in this study. They were divided randomly into two groups (15 teeth each). Group I: Access cavity preparation was performed and microbiological sample (SI1) was taken. The root canal was prepared using ProTaper rotary instruments up to #F4 and irrigated with 2ml of 2.6% sodium hypochlorite, this was repeated after each instrument use with total volume 14ml. Smear layer removal with 17% EDTA (5ml) was performed, saline solution (5ml) was used as a final irrigant then, the canal was irradiated with 810 nm diode laser in a continuous wave mode with a power of 1.5 w then, microbiological sample (SI2) was taken. Group II: The same steps were followed as in group I up to the use of saline solution (5ml) as a final irrigant, then microbiological sample (SII2) was taken. The treatment outcome was evaluated clinically (pain recording using Visual Analogue Scale (VAS) at 6,12,24,48 and 72hours) and microbiologically (reduction in number of CFU of microorganisms). Results: There was no statistically significant difference between pain scores in the two groups. Group I showed statistically significant higher mean percentage reduction in bacterial and fungal counts than group II. Conclusion: 810 nm diode laser can be used as an adjunct to conventional root canal treatment in necrotic teeth with periapical radiolucency. Comparison between Chitosan Hydrogel and Recaldent Paste in Enamel Re-mineralization of Induced Enamel Demineralized Lesions 壳聚糖水凝胶与复钙糊剂对诱导脱矿病变牙釉质再矿化效果的比较

Purpose: This study aimed to compare between the effect of chitosan and GC MI paste plus on re-mineralization of demineralized enamel in-vitro. Subjects and methods: Extracted upper premolars from orthodontic patients. Sample premolars were examined one by scanning electron microscope with energy dispersive X-ray spectroscopy to determine the Ca/P ratio before demineralization and the other by Fourier transform Infrared spectroscopy to determine the degree of conversion of bonds. After demineralization, two premolars samples were selected; one was examined by scanning electron microscope with energy dispersive X-ray spectroscope and the other was examined by FTIR to determine the degree of conversion of bonds. The remaining premolars were divided equally into 3 groups: control group (GI) premolars were remineralized using artificial saliva only for 14 days, (GII) premolars were re-mineralized by chitosan hydrogel for 14 days and (GIII) premolars were re-mineralized by GC MI paste plus for 14 days. Results: They were 46 premolars and after both negative and positive control, they were divided into 3 groups 14 premolar each where, GI showed least re-mineralization occurred, where the peripheries of prisms were lost. GII chitosan group showed the most re-mineralized results as it was not only increase in minerals but also thickness of enamel by addition of minerals’ layer on the surface and GIII GC MI paste plus showed average re-mineralization process. Statistical results: GII showed the highest mean % of re-mineralization followed by GIII and then GI. Statistical difference was significant. Conclusion: Chitosan is better re-mineralizing agent than GC MI paste plus. Evaluation of Cytotoxic Effect of Betanin Nanoparticles against Squamous Cell Carcinoma Cell Line Compared to Doxorubicin 与阿霉素比较,甜菜素纳米颗粒对鳞状细胞癌细胞的细胞毒作用

Aim: to evaluate the anticancer effect of nano sized betanine particles (betanine NP) on squamous cell carcinoma cell line compared to doxorubicin (DOX) by measuring apoptosis through caspase 3. Material and Methods: Three groups of the human tongue squamous cell carcinoma cell line (SCC 25) were created, and two of them were treated with DOX and betanine NP. The third group served as a negative control and was not given any treatment. To evaluate the concentration of caspase 3 using the ElISA technology, different quantities of betanin NP and DOX were administered to SCC25 at 48- and 72-hour intervals. Results: Caspase 3 levels in the current investigation were 501.69.93, 543.86.71 pg/ml for DOX and 336.913.1, 405.53.5, and respectively for betanine NP in 48h and 72h intervals. Additionally, morphological analysis was performed to demonstrate the cells’ apoptotic alterations. There was a highly statistically significant difference between samples of various materials, as revealed by the ANOVA test for Comparison between groups using the ELISA technique for caspase 3 detection at 48h and 72h (p0.001). These findings suggest that DOX has a stronger apoptotic effect on SCC25 cells than betanin NP. Conclusion: Our findings explained that DOX and betanine NP can induce cancer cell death against SCC 25 cell lines by increasing the concentration of caspase 3. DOX has higher apoptotic effect on SCC25 cells than betanin NP according to ELISA technique.

Antibacterial Effect of Nutmeg Extract on Cariogenic Microflora in a Group of Egyptian Children 肉豆蔻提取物对埃及儿童龋齿菌群的抑菌作用

Purpose: to evaluate the antibacterial effect of Nutmeg natural extract on cariogenic microflora and compare its effect with chlorhexidine (CHX) in a group of Egyptian children. Subject and methods: A total of 60 normal apparently healthy children aged between 6-12 years from both genders were involved in this study and equally divided into two groups (n=30) regarding to the type of the received mouthwash: group 1; included children who rinsed with 1% Nutmeg extract containing mouth wash (Tested group); group 2; included children who rinsed with 0.12% CHX containing mouth wash (Control group). Saliva samples were collected from each enrolled child at the base-line (before the use of mouthwash), and at 7 th day after the use of mouthwash. Mitis salivarius bacitracin and Rogosa agar media were used to determine Streptococcus mutans (S. mutans) and Lactobacilli count respectively. Results: Significant reduction in S. mutans and Lactobacilli count in the two studied groups after mouth rinsing for 7 days. However, CHX showed a significant reduction in S. mutans and Lactobacilli count when compared to Nutmeg extract. Conclusions: Nutmeg extract has a significant positive antimicrobial effect against S. mutans and Lactobacilli microorganisms but the use of CHX mouthwash has a higher antimicrobial effect when compared to Nutmeg extract.

Evaluation of the Addition of Injectable Platelet Rich Fibrin to Xenograft in Management of Periodontal Intraosseous Defects. “Randomized Controlled Trial” 可注射富血小板纤维蛋白异种移植治疗牙周骨内缺损的疗效评价。“随机对照试验”

Purpose: Platelet rich fibrin (PRF) has been widely and successfully used in periodontal regeneration. Injectable PRF (i-PRF) is a liquid form of PRF containing more leucocyte and growth factors. I-PRF was used with great success in management of gingival recession and implant therapy. So the aim of the study was to evaluate the benefit of adding i-PRF to xenograft in management of periodontal intraosseous defects. Subjects and Methods: Twenty four patients (age range 36 59 years), with stage III periodontitis with at least one intrabony defect, probing pocket depth > 5 mm and radiographic evidence of vertical bone loss, were enrolled in the present study. The patients were equally divided into two groups, the test group (i-PRF and xenograft) and the control group (xenograft only). Probing depth (PD), clinical attachment level (CAL.), bone defect depth and bone density were recorded at baseline and after 6 months. Results: The results showed a statistical significant reduction in PD, gain in CAL, bone fill and change in bone density from preoperative to 6 months in each group separately. There was a statistical significant difference between test and control groups with mean and standard deviation (SD) of PD reduction (4.2±0.4; 3.3±0.7) and CAL gain (3.8±0.4 ; 2.9±0.8) respectively in favor of the test group. Regarding bone fill, there was no statistical significant difference between both groups with mean and SD (2.6±0.7; 2±0.8) respectively. As for the increase in bone density, there was a statistical significant difference between both groups with mean and SD (17.5±4.9 ;10.9±6.8) respectively. Conclusion: We can conclude that adding i-PRF to bone grafting material might have a beneficial impact in treatment of periodontal intraosseous defects. 目的:富血小板纤维蛋白(PRF)在牙周再生中得到了广泛而成功的应用。可注射PRF (i-PRF)是一种含有较多白细胞和生长因子的液体形式的PRF。I-PRF在治疗牙龈萎缩和种植治疗方面取得了巨大的成功。因此,本研究的目的是评估添加i-PRF异种移植治疗牙周骨内缺损的益处。研究对象和方法:24例(年龄36 ~ 59岁)伴有至少一处骨内缺损的III期牙周炎患者,探测袋深度> 5mm,有垂直骨丢失的影像学证据。患者平均分为两组,试验组(i-PRF和异种移植物)和对照组(仅异种移植物)。在基线和6个月后记录探查深度(PD)、临床附着水平(CAL)、骨缺损深度和骨密度。结果:术前至6个月,各组患者PD降低、CAL增加、骨填充、骨密度变化均有统计学意义。试验组与对照组PD降低的均值和标准差(SD)差异有统计学意义(4.2±0.4;3.3±0.7)和CAL增益(3.8±0.4);实验组分别为2.9±0.8)。在补骨方面,两组间差异无统计学意义,均数和标准差为(2.6±0.7);分别2±0.8)。在骨密度增加方面,两组的平均值和SD值分别为17.5±4.9和10.9±6.8,差异有统计学意义。结论:在植骨材料中加入i-PRF对治疗牙周骨内缺损有良好的效果。Codex: 15/22.04 azhardentj@azhar.edu.eg http://adjg.journals.ekb.eg DOI: 10.21608/adjg.2022.111166.1461口腔医学与外科科学(口腔医学,口腔颌面外科,口腔病理学,口腔生物学)

Injectable PRF (i-PRF) is a liquid form of PRF containing more leucocyte and growth factors. I-PRF was used with great success in management of gingival recession and implant therapy. So the aim of the study was to evaluate the benefit of adding i-PRF to xenograft in management of periodontal intraosseous defects. Subjects and Methods: Twenty four patients (age range 36 59 years), with stage III periodontitis with at least one intrabony defect, probing pocket depth > 5 mm and radiographic evidence of vertical bone loss, were enrolled in the present study. The patients were equally divided into two groups, the test group (i-PRF and xenograft) and the control group (xenograft only). Probing depth (PD), clinical attachment level (CAL.), bone defect depth and bone density were recorded at baseline and after 6 months. Results: The results showed a statistical significant reduction in PD, gain in CAL, bone fill and change in bone density from preoperative to 6 months in each group separately. There was a statistical significant difference between test and control groups with mean and standard deviation (SD) of PD reduction (4.2±0.4; 3.3±0.7) and CAL gain (3.8±0.4 ; 2.9±0.8) respectively in favor of the test group. Regarding bone fill, there was no statistical significant difference between both groups with mean and SD (2.6±0.7; 2±0.8) respectively. As for the increase in bone density, there was a statistical significant difference between both groups with mean and SD (17.5±4.9 ;10.9±6.8) respectively. Conclusion: We can conclude that adding i-PRF to bone grafting material might have a beneficial impact in treatment of periodontal intraosseous defects. Comparative Evaluation of Smear Layer Removal by Apple Vinegar Using Laser Activated Irrigation and EndoVac System: Randomized Control Trial 激光活化灌洗与EndoVac系统去除苹果醋涂抹层效果比较:随机对照试验

Purpose: This study was designed as randomized control trial to assess the effect of laser activation versus EndoVac systems with 5% apple vinegar irrigant on smear layer removal from root canal wall using scanning electron microscope. Material and Methods: Thirty three single rooted teeth were selected and prepared using Universal ProTaper rotary files and irrigated using sodium hypochlorite. Samples were randomly alienated into three groups (11 samples each) in proportion to method of activation of apple vinegar which used as final irrigant: Group I: Laser activated irrigation using 810nm diode laser at 1.5 Watts power in pulsed mode, Group II: EndoVac system used according to manufacturer instruction, and Group III (Control group) irrigating needle was used for irrigation, the amount of irrigant and time of application were adjusted according to each irrigating method. Scanning electron microscopic analysis was performed to evaluate smear layer of each group at different root canal levels. Results: No significant difference between tested groups in coronal and middle thirds but there is significant difference in apical third and total. In apical third, there is statistically significant difference between laser activated and EndoVac compared to control group, while no significant difference was found between EndoVac system and laser activated group (P-value <0.05). Conclusion: This in-vitro study demonstrated that using of 810 nm diode laser activation and EndoVac system with apple vinegar are effective in removing smear layer compared to conventional irrigation, particularly at apical third of root canal system. Cytotoxic Effect of Different Concentrations of Chitosan and Propolis Nanoparticles on Periodontal Ligament Stem Cells 不同浓度壳聚糖和蜂胶纳米颗粒对牙周韧带干细胞的细胞毒作用

Purpose: The purpose of this study was to assess the cytotoxic effect of different concentrations of Chitosan Nanoparticles (CNPs) and Propolis Nanoparticles (PNPs) on periodontal ligament stem cells using MTT assay. Materials and Methods: Immature, impacted third molars were used to isolate stem cells. Cells were seeded in 96-well culture plates. Groups were divided into six experimental groups: Group I: 17% EDTA, Group II: 1% propolis NPs, Group III: 2% propolis NPs, Group IV: 1% chitosan NPs, Group V: 2% chitosan NPs, Group VI: cultured cells not receive any material. Cytotoxic effect of these materials on stem cells was evaluated using MTT assay at 24, 72,120 hours and 14 days. Results: In all assay intervals; 2%CNPs group showed the statistically significant highest median value and control group showed the statistically significant lowest median values. After 24 and 72 hours; there was no statistically significant difference between 2% PNPs and 1% CNPs groups followed by 1% PNPs group. Both EDTA and control groups; showed the statistically significantly lowest median values with no statistically significant difference between them. After 120 hours; there was no statistically significant difference between 2% PNPs and 1% CNPs groups in median value followed by 1% PNPs then EDTA groups. After 14 days; there was no statistically significant difference between 1% PNPs, 2% PNPs and 1% CNPs groups in median value followed by EDTA group. Conclusion: The ability of 2% Chitosan NPs to promote periodontal ligament stem cell viability makes them a promising irrigating solution. The cell viability of 2% Propolis NPs was comparable to that of 1% Chitosan NPs, which was better than EDTA. Regenerative Potentiality of Concentrated Growth Factors with Partially and Completely Amputated Pulp Tissue (Histological Study) 浓缩生长因子在部分和完全切除牙髓组织中的再生潜能(组织学研究)

Purpose: This study aimed to investigate the effect of concentrated growth factors (CGF) with partially and completely amputated pulp tissue on the histologic outcome of dentin-pulp complex regeneration in immature dogs’ teeth with non-infected pulp and apical tissue. Materials and Methods: In this study fifty six immature teeth in four mongrel dogs were involved and simple randomization design was used. The teeth were divided into four groups; group (I): the teeth were left with no treatment (negative control), group (II): the teeth underwent complete pulp extirpation where blood clots filled the root canals (positive control), group (III): the root canals were packed with CGF after complete pulp extirpation and group (IV): the pulp was partially removed leaving the last 1–4mm and the root canals were packed with CGF. The animals were euthanized at 1and 3 months and samples were processed for histological analysis. Results: The best results appeared in CGF treated groups, where the immature roots continued their development in a way simulating the normal and this was represented histologically in closure of the root apex, increasing the root canal wall thickness and ingrowth of soft tissue into the root canals. However, the pulp architecture and pulpdentin relation appeared better when the pulp was partially removed. Conclusion: Dentin-pulp regeneration of the immature teeth can be promoted by CGF with faster root development and better pulp architecture when the apical 1–4mm of the pulp tissues are left. Assessment of Rate of Orthodontic Tooth Movement with Different Modalities of Ligations 不同结扎方式正畸牙齿移动率的评估

Purpose: This study was carried out to assess the effect of different types of ligatures on the rate of canine retraction. Materials and Methods: 14 participants requiring extraction of upper first premolars, were randomly assigned into 2 groups; group 1 had passive self-ligating bracket on the upper right canine, figure of 8 module ligature on the upper left canine and group 2 also had self-ligating bracket on the upper right canine, and slide-low friction elastic module on the left side. Canines retraction were done under 150 gm of force using a 9 mm nickel-titanium closed-coil spring .Alginate impression was taken at the onset of canine retraction(T0), and every month (T1T4). Study casts were fabricated and then scanned with a 3D scanner. Results: The statistical analysis showed no statistically significant difference in the rate of canine retraction between the 2 groups, and between the control and intervention sides within each group. Conclusion: The difference in rate of upper canine retraction between selfligation brackets, figure of 8 elastic module and slide –low friction ligature was not statistically significant.
目的:研究不同结扎方式对犬牵出率的影响。材料与方法:需要拔除上颌第一前磨牙的患者14例,随机分为2组;组1右上犬齿为被动自结扎支架,左上犬齿为8模结扎图,组2右上犬齿为自结扎支架,左侧为低摩擦滑动弹性模块。用9mm镍钛闭式弹簧在150gm的力下完成犬的收拔。在犬的收拔开始时(T0)和每月(T1T4)取海藻酸盐印模。制作研究模型,然后用3D扫描仪扫描。结果:经统计分析,两组间、各组内对照组与干预组间犬科内缩率差异均无统计学意义。结论:自定位托槽、8字形弹性模组和滑动-低摩擦结扎法在上犬牵出率上的差异无统计学意义。

Canines retraction were done under 150 gm of force using a 9 mm nickel-titanium closed-coil spring .Alginate impression was taken at the onset of canine retraction(T0), and every month (T1T4). Study casts were fabricated and then scanned with a 3D scanner. Results: The statistical analysis showed no statistically significant difference in the rate of canine retraction between the 2 groups, and between the control and intervention sides within each group. Effect of Pomegranate and Banana Extracts on Microshear Bond Strength of Resin Composite to Bleached Bovine Enamel: An In-Vitro Study 石榴和香蕉提取物对树脂复合材料与漂白牛牙釉质微剪切结合强度的体外研究

Purpose: To evaluate the effect of pomegranate and banana extracts (natural antioxidants) versus ascorbic acid (synthetic antioxidant) prepared in 10% and 15% concentrations on microshear bond strength (μSBS) to bleached enamel. Materials and Methods: A total of 56 bovine incisors were selected for the current study. Forty teeth were used for microshear bond strength and divided into 5 groups according to antioxidant type; Group 1: No bleaching + No antioxidant (negative control), Group 2: Bleaching + No antioxidant (positive control), Group 3: ascorbic acid, Group 4: pomegranate extract and Group 5: banana extract. Groups 3,4 and 5 were further divided into 2 subgroups according to concentration of antioxidant: 10% and 15%. Bleaching gel was applied according to manufacturer instructions. Antioxidants were applied for 10 min. Composite specimens were prepared and tested for μSBS using universal testing machine. Sixteen bovine incisors were prepared and tooth-restoration interfaces were assessed using SEM. Data were tabulated and statistically analyzed. Results: Negative control and 15% ascorbic acid recorded the highest mean μSBS values followed by 15% and 10% pomegranate groups. No significant difference was reported between 10% ascorbic acid and 15% banana. Lowest mean values were obtained with 10% banana and positive control groups with no significant difference between them. Conclusions: Pomegranate in both concentrations and 15% banana extract could partially reverse compromised bond strength. Use of 15% ascorbic is able to totally regain bond strength to bleached enamel. Banana extract in low concentration is not beneficial in improvement of bond strength after bleaching.
