Optical Properties of Zirconia Monolithic Crowns Constructed Using Speed Sintering Cycle 快速烧结制备氧化锆单片冠的光学性质

Purpose: This study examined the effect of change in sintering time on the optical properties of monolithic zirconia crowns using ZirCAD Prime. Materials and methods: A Sound premolar was scanned using optical oral scanner. The scanned premolar was digitally prepared to receive zirconia restoration using Exocad software following standard preparation guidelines for ceramic restorations. 3D printed resin master die was produced. An impression was made for the 3D printed master die using polyvinylsiloxane impression material then poured using epoxy resin. The procedure was repeated to obtain 16 epoxy resin models. Sixteen crowns were produced by CADCAM system from ZirCAD Prime blank shade A2 with 16 mm thickness. Samples were divided into two groups according to sintering time (n=8). Group (A): crowns were sintered using normal sintering cycle for 9h and 50 min. Group (B): crowns were sintered using speed sintering cycle for 2h and 26 min. After sintering the internal surfaces of both groups were abraded with 50 μm Al2O3 particles. Optical properties including Translucency Parameter (TP) and change in color (ΔE) of samples were determined using spectrophotometer. Obtained values were statistically analyzed. Results: Regarding change of color evaluation mean ∆E values were below 3.0 which is considered “clinically imperceptible”. There was also no significant difference in translucency parameter between both groups. Conclusion: ZirCAD Prime speed sintering cycle will have no perceived effect on color and translucency of the final restoration.

The Effect of Silver Diamine Fluoride on Microleakage and Shear Bond Strength of Glass Ionomer 氟化二胺银对玻璃离聚体微漏及剪切粘结强度的影响

Purpose: This study was conducted to evaluate the impact of silver diamine fluoride (SDF) on Glass ionomer restoration (Fuji IX) microleakage and shear bond strength. Material and Methods: The present study was carried out on 40 primary extracted human molars. The primary molars were equally divided into 2 groups (Group I, Group II). (Group I) including: 20 primary molars were used for microleakage testing under stereomicroscope at x40. (Group II) including: 20 primary molars were used for shear bond strength testing using Instron Universal Testing Machine. Each group was subdivided into 2 subgroups; subgroup 1 which been treated with SDF then restored by Glass ionomer restoration and subgroup 2 which was restored with Glass ionomer without SDF. Result: In microleakage analysis comparing both groups in both surfaces, the highest mean value was recorded in glass ionomer without SDF group in the occlusal surface (758.77±69.65), followed by glass ionomer without SDF group in the gingival surface (643.74±83.77), then glass ionomer with SDF group in the occlusal surface (126.06±10.70), with the least value was recorded in Glass ionomer with SDF group in the gingival surface (109.12±5.58). In shear bond strength analysis, the higher mean value was recorded in Glass ionomer with SDF group (9.38±1.12), while that recorded in Glass ionomer without SDF group was (1.97±0.16). Conclusion: The results of the current analysis suggest that the use of SDF minimized microleakage and increased intensity of shear bond strength for Glass ionomer restoration.

Evaluation of Retention for Maxillary Complete Denture Constructed by Conventional and 3D Printing Techniques 传统与3D打印技术对上颌全口义齿固位效果的评价

Purpose: Evaluating retention of maxillary complete dentures fabricated by threedimensional (3D) printing technique compared to conventional technique. Subjects and Methods: Ten completely edentulous patients were selected from the outpatient clinic of Removable Prosthodontics Department, Faculty of Dental Medicine for Girls, Al-Azhar University. Each patient received maxillary and mandibular dentures fabricated by conventional technique besides maxillary denture fabricated by 3D printing technique. All patients were divided equally into two groups; First group patients received conventional dentures first, then 3D printed ones. Second group patients received 3D printed dentures initially and conventional dentures later. Results: Regarding denture type, it was found that 3D printed dentures showed statistically significant higher retention at (P ≤ 0.05) compared to conventional dentures. Furtherly, pair-wise comparisons between follow up times revealed that there was a statistically significant increase in mean retention values after one month as well as from one to three months. Conclusion: Utilizing 3D printing technique in complete denture fabrication yielded better retention compared to conventional technique. Nevertheless, regardless of denture type, retention improves over time.
目的:比较三维打印技术与传统技术制作的上颌全口义齿固位效果。研究对象与方法:选取爱资哈尔大学女子口腔医学院活动义齿科门诊全牙缺牙患者10例。除采用3D打印技术制作上颌义齿外,均采用常规技术制作上颌和下颌骨义齿。所有患者平均分为两组;第一组患者首先使用传统假牙,然后使用3D打印假牙。第二组患者先使用3D打印义齿,后使用常规义齿。结果:在义齿类型方面,3D打印义齿固位高于传统义齿,差异有统计学意义(P≤0.05)。此外,随访时间之间的两两比较显示,1个月后和1至3个月期间的平均留存值有统计学上的显著增加。结论:采用3D打印技术制作全口义齿,固位效果优于传统义齿。然而,无论假牙类型如何,固位都会随着时间的推移而改善。

Effect of Propolis and Castor Oil Cement as Pulpotomy Materials on the Properties and Osteogenic Differentiation of Stem Cells Isolated from Human Exfoliated Deciduous Teeth (SHED) 蜂胶和蓖麻油水泥对人脱落乳牙干细胞性能及成骨分化的影响

Purpose: The aim of this research was to assess the effect of propolis and castor oil cement as pulpotomy materials on the stemness properties and osteogenic differentiation potential of stem cells isolated from human exfoliated deciduous teeth (SHED). Materials and methods: SHEDs were isolated from the pulp of five deciduous anterior teeth indicated for extraction. SHEDs were cultured with propolis and castor oil cement extracts. SHEDs cultured in MTA extracts and basal media were used as positive and negative controls respectively. Stemness properties of SHEDs were assessed in the form of proliferation, migration and the ability to enhance wound healing. Moreover, osteogenic differentiation potential of the isolated SHEDs was evaluated. Results: No significant statistical difference between propolis and castor oil cement regarding cell proliferation was detected. SHEDs cultured in Castor oil cement showed higher migration ability than those cultured in propolis. The ability to enhance the healing of the wound was more in castor oil cement group rather than in the propolis group. Finally, castor oil cement induced osteogenic differentiation while no formation of calcified nodules was noticed in the presence of propolis. Conclusions: Because of the limitation of the current research it was came to conclusion that Castor oil cement may have greater potentials as a pulpotomy material than propolis. Assessment of Pain Sensation and Release of RANKL In Gingival Crevicular Fluid Accompanying Laser Application In Orthodontic Treatment: A Randomized Controlled Clinical Trial 激光在正畸治疗中对牙龈沟液疼痛感觉和RANKL释放的评估:一项随机对照临床试验

Purpose: This study was made to evaluate the level of receptor activator of nuclear factorkappa B ligand (RANKL) in gingival crevicular fluid (GCF) and pain perception during orthodontic tooth treatment to evaluate the efficacy of low-level laser therapy (LLLT). Subjects and methods: A number of 10patients (age range: 15-20) requiring extraction of maxillary first premolars as a part of orthodontic therapy were selected randomly. A split-mouth technique was used. The test side received (LLLT) from a semiconductor (aluminium galliumarsenide) diode laser. The laser was irradiated on days 0, 2, 7, and14. The canine distalization was achieved with a force of 150 g per side using nickel titanium closed coil spring. GCF samples were collected from canines on days 0,7,14 and 30 using perio paper point #35. Enzyme-linked immunosorbent assay (ELISA) was used to assess levels of RANKL. Pain was assessed for one week from the intervention using a visual analogue scale(VAS).Gingival index and pocket depth were assessed at baseline and after 30 days. Results: There was no significant difference between RANKL concentrations in the two groups at base line, 7 and 14days. After 30 day; Laser side showed statistically significant lower mean RANKL (p=0.011) than control side. For pain: non significant difference was found between the two groups during the tested periods. Conclusion: LLLT showed no additional benefits over conventional canine retraction regarding pain and RANKL release. However, the laser group showed the least RANKL level at the end of the study which denotes biostimulatory effect of laser on bone cells. Pain was assessed for one week from the intervention using a visual analogue scale(VAS).Gingival index and pocket depth were assessed at baseline and after 30 days. Results: There was no significant difference between RANKL concentrations in the two groups at base line, 7 and 14days. After 30 day; Laser side showed statistically significant lower mean RANKL (p=0.011) than control side. For pain: non significant difference was found between the two groups during the tested periods. Conclusion: LLLT showed no additional benefits over conventional canine retraction regarding pain and RANKL release. However, the laser group showed the least RANKL level at the end of the study which denotes biostimulatory effect of laser on bone cells. Purpose: To evaluate marginal fit as well as fracture resistance of zirconia implant abutment supporting two types of metal free CAD/CAM restorations. Materials and methods: Twenty ready-made ZrO2 abutments resembling lower first premolar squeezed and stabled to its conforming titanium dummy implants were implanted in epoxy resin blocks. Samples were divided into two groups in relation to the material utilized for the fabrication of the copings; group (I) (n=10): zirconia copings, group (II) (n=10): breCAM.BioHPP copings. All copings were machined using CAD/CAM system. Samples were exposed to a fatigue procedure for 20,000 cycles. Marginal fit was determined using digital stereomicroscope. Fracture resistance was registered using universal testing machine. The samples were overloaded till last fracture occurred and load at fracture was documented. Analysis of data was examined using accessible software programme (SPSS 18). Results: BreCAM.BioHPP copings, group (II), registered a statistically significant higher mean vertical marginal gap value (122.08±25.05μm) in addition to higher mean failure load value (2828.47±735.4N) compared to zirconia copings, group (I) (27.06±3.2μm) (416.06±22.16N) respectively. Conclusions: Material type used for the fabrication of the copings influences marginal fit and fracture resistance. Zirconia coping samples showed higher marginal fit and lower fracture resistance than breCAM.BioHPP coping samples.

Assessment of Dimensional Ridge Changes After Augmentation Using External Oblique Ridge with or without Computer Surgical Guide 有或无计算机手术指导下使用外斜脊隆胸后尺寸脊变化的评估

Purpose: The current study was conducted to evaluate, clinically and radiographical-ly, the results of defective ridge augmentation by external oblique ridge graft with or without the use of computer-generated surgical guide. Patients and methods: Twelve patients with alveolar ridges defect were included in this study. The patients were divided into 2 groups: Group A; The alveolar ridges defect augmentation from external oblique ridge graft using Khoury technique with computer- designed generated surgical guides. Group B; The alveolar ridges defect augmentation from external oblique ridge graft using traditional Khoury technique. Clinical and radiographic follow up was performed for 6 months. Treatment changes were evaluated for each group and a comparison was done between the 2 groups. Data were analyzed using paired t-test for each group and student t- test to compare between the two groups. Results: There was a significant increase in bucco-lingual/palatal ridge dimensions between the two studied groups. While the minimal bone distances to the nerve differ significantly (p<0.05), the computer guided group had greater amount of bone remaining around the nerve than non-guided group. Conclusion: Computer assisted surgical guides allow for accurate precise osteotomies providing safe and quick surgery that reflects positively on the postoperative clinical outcomes.

Purpose : This study was carried out to evaluate the effect of biostimulation on the regenerative response of immature teeth with necrotic pulp and apical periodontitis in dogs. Materials and Methods: Seventy-two root canals were employed in the present study as follows: Sixty root canals for experimental groups and twelve as positive and negative controls. After the induction of pulp necrosis and apical periodontitis, roots were divided into two groups, group I: Regenerative Endodontic Therapy (RET) with subsequent biostimulation (seven sessions at 808 nm diode laser at output power of 300 mW for 90 seconds) and group II: RET without biostimulation. The groups were followed up for 1, 2 and 3 months (subgroups A, B&C). The maturation of the roots was assessed both radiographically and histologically. All data were statistically analyzed. Results: the application of biostimulation in group I demonstrated marked increase in root length, thickness and decrease in apical diameter compared to group II however, it was statistically significant only in subgroup C (12.43% versus 7.66%, 33.09% versus 17.81% and 38.12% versus 23.35 % at third month) respectively (p<0.05). Furthermore, it showed histologically the highest score of vital tissue infiltration and least inflammatory scores which was statistically significant. Effect of Diode Laser and Iontophoresis with Aqueous Extract of Nigella sativa and Fluoride-Based-Agent on Dentin Permeability 二极体激光及离子导入黑草水提液及氟基剂对牙本质渗透性的影响

Purpose: Evaluation of the effect of diode laser and iontophoresis with aqueous extract of Nigella sativa and fluoride-based-agent on dentin permeability. Materials and Methods: Thirty-six lower premolars were used to prepare 72 samples and were randomly divided into three intervention groups 24 per group; Group A1 Samples were treated with aqueous extract of Nigella sativa, Group A2 Samples were treated with fluoride-based-agent, Group A3 Control group were left without treatment. The main groups were subdivided into two subgroups of 12 samples, according to surface treatments: Group B1 was treated with diode laser and Group B2 was treated with iontophoresis. Results: Permeability results revealed that there was statistically significant difference between the study groups with the lowest mean depth of dye penetration values was found in group samples treated with aqueous extract of Nigella sativa with iontophoresis. Conclusion: Aqueous extract of Nigella sativa, a fluoridebased-agent with the application of diode laser and iontophoresis on each was effective in reducing dentin permeability, being an aqueous extract of Nigella sativa the most effective when using with iontophoresis.

Effect of Systemic Administration of Simvastatin on Dental Implant Stability 辛伐他汀对种植体稳定性的影响

Purpose: This study was designed to evaluate implant stability in partially edentulous patients by investigating the effect of systemic simvastatin administration on the bone regeneration around the dental implant. Patients and Methods: A total of 18 patients were divided into two groups. Group I:9 healthy patients underwent a one-stage titanium screw-type endosseous dental implant and Group П:9 patients on simvastatin therapy (20 mg daily) as a prophylaxis drug, underwent the same type of one stage titanium screw type of endosseous dental implant as the control group. Modified plaque index (MPI), Probing depth (PD) and Modified Gingival Index (MGI) were recorded. Implant stability was recorded immediately and after 6months. Results: Group II showed a higher mean implant stability (79.14±4.38) than group I (72.86±11.41) at 6 months with a non-significant difference Also, there is a nonsignificant difference of MPI, MGI, and PD between both groups after 3 and 6 months respectively. Conclusion: Simvastatin group showed higher implant stability than the control group. Simvastatin might have the ability to assist and enhance the osseointegration procedure for bone surrounding the dental implant.
