Efficacy of Moringa Leaf Extract in Management of Bone Healing after Implant Placement 辣木叶提取物在种植体植入术后骨愈合中的作用

Purpose: This study aimed to evaluate the effect of using topical Moringa leaf extract on healing after delayed dental implant placement in posterior maxilla. Subjects and methods: A total of twelve patients with missing maxillary premolars and molars were involved in this study. They were randomly divided into two groups; Group I (test group; n = 6) patients that received delayed implant placement with the use of topical Kaempferol (Moringa olifera leaf extract) gel and Group II (control group; n = 6) patients that received delayed implant placement only for replacement of missing teeth. Clinically, pain was evaluated for each patient by using visual analog scale (VAS) and swelling was assessed in form of mild, moderate and severe. Assessment of healing progress was conducted by modified gingival index and probing depth at 1st, 3rd and 6th month after implant placement. Results: A statistically significant difference was recorded in VAS assessment for 1st week follow up in both groups (P=0.026), regarding swelling no statistically significant difference between studied groups (P=0.065). Considering the values of modified gingival index and probing depth, the study group showed lower values than of control group during the follow up periods of the study however, this difference was non-significant (P= 0.240) for both parameters. Conclusion: The use of Kaempferol topical gel with delayed dental implant placement offer a new promising natural, safe and effective remedy for management of healing process. 目的:观察辣木叶提取物对上颌后牙延迟种植体愈合的影响。对象与方法:选取12例上颌前磨牙及磨牙缺失患者作为研究对象。他们被随机分为两组;第一组(试验组;n = 6)局部使用山奈酚(辣木叶提取物)凝胶延迟种植的患者和II组(对照组;延迟种植仅用于替换缺牙的患者6例。临床采用视觉模拟评分法(VAS)对患者进行疼痛评估,肿胀程度分为轻度、中度和重度。分别于种植后第1、3、6个月通过改良牙龈指数和探探深度评估修复进展。结果:两组随访第1周VAS评分差异有统计学意义(P=0.026),肿胀评分差异无统计学意义(P=0.065)。在研究随访期间,研究组改良牙龈指数和探诊深度均低于对照组,但两项指标差异均无统计学意义(P= 0.240)。结论:山奈酚外用凝胶治疗延期种植体是一种自然、安全、有效的治疗方法。法典:15/21.10 azhardentj@azhar.edu.eg http://adjg.journals.ekb.eg DOI: 10.21608/adjg.2021.48189.1317口腔医学与外科科学(口腔医学,口腔颌面外科,口腔病理学,口腔生物学)

Evaluation of Dental Implant Stability: A comparative Study of Tapered vs Cylindrical Implants 牙种植体稳定性评价:锥形种植体与圆柱形种植体的比较研究

Purpose: This study was carried out to evaluate stability of tapered and cylindrical implants in partially edentulous patients. Materials and Methods: The current study was done on 14 implants that were alienated randomly into two groups: Group I: treated with tapered implant, Group II: treated with cylindrical implant. Results: Clinical results: Modified Gingival Index (MGI) revealed a non-significant statistically difference on comparing results of 3 months and 6 months postoperatively (p=0.254 at 3 months, p=0.606 at 6 months). Modified Plaque Index (MPI) revealed a non-significant statistically difference on comparing results of 3 months and 6 months postoperatively (p=0.298). Probing Depth(PD) revealed a nonsignificant statistically difference on comparing results of 3 months and 6 months postoperatively (p=1 at 3 months, p=0.591 at 6 months). Stability: the mean value increased by time from baseline to after 6 months in Group I (61.57±6.4 to 71.57±6.08), however, the mean value increased from 63.14±7.17 to 74.57±5.53 in Group II. Implant stability revealed nonsignificant statistically differences for p-values (P 0.05). Radiographic results: Bone Density (BD):The statistical analysis between the two groups in the mean percent change in bone density revealed a non-significant statistically difference between the 6 months results. It was 1.88±1.37% for the first group and 11.54±15.63% for the second group (p=0.20).Conclusions: Conical and cylindrical implants have comparable biological effects during the healing process. The properties of the bone site can influence the stability of the implant. Comparative Evaluation of Fresh Aloe Barbadensis Plant Extract and Mineral Trioxide Aggregate as Pulpotomy Agents in Primary Molars 新鲜巴巴多斯芦荟提取物与三氧化二矿骨料在第一磨牙切髓剂中的比较评价

Purpose: A comparative evaluation of Aloe Barbadensis plant extract and Mineral Trioxide Aggregate as pulpotomy agents in primary molars. Materials and methods: Thirty eight Children, aged between 4 to 8 years old of both sexes, 20 girls and 18 boys were included in this study. Pulpotomy was made in primary molars, it was divided into two groups. The first group included 32 teeth indicated for pulpotomy with NeoMTA and the second group included 32 teeth indicated for pulpotomy treated with Aloe vera .Clinical and radiographic follow up were made for each child after 1,3 and 6 months. Results: NeoMTA showed higher clinical success results than Aloe vera after 3 and 6 months,Chi square test shown a significant difference statistically between groups in all of its clinical findings and in the overall clinical and radiographic success 6 months after treatment. Conclution: Upon comparison between Aloe vera and NeoMTA as a pulpotomy medications NeoMTA showed better results clinically and radiographically.
目的:比较巴氏芦荟植物提取物和三氧化二矿骨料作为第一磨牙切髓剂的疗效。材料与方法:选取年龄在4 ~ 8岁的男女儿童38名,其中女孩20名,男孩18名。在一磨牙处行截髓术,分为两组。第一组32颗牙行NeoMTA牙髓切开术,第二组32颗牙行芦荟牙髓切开术,分别于1、3、6个月进行临床及影像学随访。结果:NeoMTA在治疗3个月和6个月后的临床成功率高于芦荟,卡方检验显示,治疗6个月后,NeoMTA的所有临床表现和总体临床和放射学成功率在组间具有统计学差异。结论:将芦荟与NeoMTA作为髓切开术药物进行比较,NeoMTA在临床和影像学上均表现出更好的疗效。

Effect of Rapid Prototyped Upper Complete Denture on Retention and Patient Satisfaction 快速成型上全口义齿对固位及患者满意度的影响

Purpose: This study aimed to evaluate the effect of rapid prototyped upper complete denture on retention and patient satisfaction. Material and methods: Eight completely edentulous patients were selected for this study with ages ranged from 40-50 years. All patients had received two complete dentures one fabricated by conventional heat compressed method and the other fabricated by rapid prototyping technology. Patients were classified into two groups, four patients each. Group I received the conventional denture first then the rapid prototyped denture while, Group II received the rapid prototyped denture first then the conventional denture. All patients were instructed to wear their dentures for equal periods of 1 month where retention and patient satisfaction were measured at insertion and after 1 month. After an interval of two weeks, as a wash out period, patients received their other denture. Results: The results of this study showed that there was a high statistical significant difference between the conventional and the rapid prototyped denture regarding retention and patient satisfaction while there was no significant difference between both groups, group I which received the conventional denture first and group II which received the rapid prototyped denture first. Conclusion: within the limitation of this study, it was concluded that retention and patient satisfaction of maxillary rapid prototyped denture were significantly higher than that of the conventional denture. The rapid prototyped dentures can be used as an alternative effective satisfactory treatment for completely edentulous patients rather than conventional dentures but furthermore studies are needed. Clinical and Radiographic Evaluation of Mineral Trioxide Aggregate, Laser and Antioxidant Mix as Pulpotomy Agents in Primary Molars 三氧化矿骨料、激光、抗氧化合剂在磨牙切髓术中的临床及影像学评价

Purpose: The purpose of this study was to access post-operative clinical as well as radiographic success of Mineral Trioxide Aggregate (MTA), Diode Laser and Antioxidant mix as pulpotomy agents for human primary molars. Materials and Methods: A total of 60 primary molars that had deep carious lesion and need pulpotomy treatment from 38 healthy and cooperative patients, aged 5-8 years old were chosen for this study. After removal of the coronal tissue and depending on the type of radicular pulp medicament, the teeth were divided randomly and equally into three groups; Group I (MTA), Group II(Diode Laser) and Group III (Antioxidant mix). Treatments in all groups were completed with Glass Ionomer restoration then stainless steel crowns. Results: The cumulative clinical and radiographic success rates after 6 months follow up were 100%, 90% and 85% for the MTA, Diode Laser and Antioxidant mix pulpotomy groups respectively. Conclusion: MTA, Lasers and Antioxidant mix used for primary teeth pulpotomy had a more advanced success rate on follow-up. Statistically, no significance difference was found between the three study groups in the overall clinical and radiographic success rate.

Conclusion: MTA, Lasers and Antioxidant mix used for primary teeth pulpotomy had a more advanced success rate on follow-up. Remineralization of Early Carious Lesion Using Natural Agents Versus Bioadhesive Polymers 天然药物与生物胶粘剂在早期龋病再矿化中的应用

Purpose: The study was established to compare the remineralization effect of natural agents versus bioadhesive polymers with or without PH cycling using a microhardness test. Materials and Methods: Forty extracted premolars were used. The roots of the teeth were removed, then the crowns of teeth were cut in mesiodistal direction to expose the labial and lingual portion using a diamond disc so that 80 specimens were obtained. Each tooth-half was fixed with superglue on the custom made acrylic block. The enamel surfaces were polished to produce flat enamel surfaces. 80 specimens were divided into four groups (A) according to the remineralizing agent used. Group A1 for eggshell nanoparticles, Group A2 for clove extract, and Group A3 for carbopol, and Group A4 for carboxymethyl cellulose (20 each). Each group was subdivided into two subgroups (10 each) according to PH cycling either with (B1) or without (B2). Then, the specimens were examined before and after demineralization and after treatment agent with Digital Display Vicker Microhardness Tester. Results: The results showed that the highest microhardness mean values were found with eggshell nanoparticles followed by carbopol, and then carboxymethyle cellulose then clove extract. Conclusion: Natural materials have remineralizing potentials and could successfully replace synthetic products.

Evaluation of CD44 Expression in Malignant Salivary Gland Tumors CD44在恶性唾液腺肿瘤中的表达

Purpose: The aim of this study was to assess the presence and correlation of CD44 in selected malignant salivary glands tumors (SGTs), carcinoma ex pleomorphic (Ca ex PA), mucoepidermoid carcinoma (MEC) and adenoid cystic carcinoma (ACC) histopathological variants. Material and Methods: Thirty-five formalin fixed paraffin embedded blocks of both normal and malignant salivary gland specimens were included. Immunohistochemical analysis was employed to assess the expression of CD44. Results: In the present study, comparing all groups revealed that the highest value was recorded in MEC, followed by ACC, then Ca ex PA, while the lowest mean was recorded in normal salivary tissue. ANOVA tests indicated that all groups had statistically significant differences (p=0,00). Comparing all subtypes of ACC revealed a significant difference between solid pattern and cribriform tubular patterns. Also, there is a significant difference between the high and low grades MEC. Conclusion: According to the current study, the pattern of expression of CD44 was correlated with the histopathological variants and serve as tumor behavior indicator.

Influence of Occlusal Plane Orientation on Sound Production and Electromyographic Activity in Complete Denture Wearers 咬合平面方向对全口义齿佩戴者声音产生和肌电活动的影响

Purpose: The aim of the study was to evaluate the effect of two different occlusal plane orientations (Ala tragus line and Retro molar pad) on electromyographic activity of masseter and anterior temporalis muscles and sound production in complete denture wearers. Material and methods: Ten completely edentulous patients’ ages ranged from 55-65years. All patients had received two dentures first denture was made according to Ala tragus line (Group І) and second denture was made according to retro molar pad (Group ІІ). Electromyographic activity was evaluated for both groups after one week, one month and three months with one week washing period between the two groups. While Speech analysis was evaluated after one week and one month for both groups. Results: The results of this study showed that no significant differences between both groups regarding to Electromyographic activity and phonetics. Conclusion: Based on the results of this study no single technique seems completely accurate to determine the exact occlusal plane. A combination of different landmarks along with a good clinical assessment should be considered in order to determine the location of the occlusal plane in edentulous patients.

Effect of Application of Natural versus Synthetic Desensitizing Agents on Dentin Permeability 天然与合成脱敏剂对牙本质渗透性的影响

Purpose: This in vitro study was established to compare the effect of natural versus synthetic desensitizing agents on dentin permeability. Materials and Methods: Class V cavity preparation was done on the labial surface of 40 premolars for cervical dentin assessment, while for coronal dentin; class I cavity was prepared on the occlusal of 40 premolars. Cavities were etched with phosphoric acid then classified according to treatment either by Bio active glass, Pro-Argin, Moringa extract or untreated (control). The dentin permeability was done using dye penetration test with Evans blue dye. The depth of dye penetration in dentin was examined using stereomicroscope. Results: The results showed that the lowest statistically significant mean depth of dye penetration values were found with Bio active glass followed by Pro-Argin, then Moringa extract. Conclusion: Among the three treatment modalities, Bio active glass and Pro-Argin can be considered as effective materials in the management of hypersensitivity

Survival and Adherence of Apical Stem Cells to Root Canal Dentin after Conditioning with Apple Vinegar 苹果醋调理后根尖干细胞在根管牙本质上的存活和粘附

Purpose: To evaluate the survival and adherence of apical stem cells to root canal dentin after conditioning with apple vinegar. Materials and Methods: Stem cells from apical papilla of immature third molars were isolated. Survival of cells was evaluated by MTT test. Adherence of cells was evaluated by level of fibronectin expression and scanning electron microscope (SEM) observation. Thirty dentin discs from premolar teeth, extracted for orthodontic purposes, were immersed in 1.5% NaOCl then washed by PBS and finally divided into five groups regarding the final irrigating solution and time of its application. Group I: EDTA for 5 minutes, Group II: Apple vinegar for 5 minutes, Group III: Distilled water for 5 minutes, Group IV: EDTA for 3 minutes and Group V: Apple vinegar for 3 minutes. Cellular morphology was evaluated by SEM. For that part of the study, twelve root segments were longitudinally sectioned into two halves, and then immersed in 1.5% NaOCl, then rinsed with PBS. Finally, they were divided into three groups regarding the final irrigation. Group I: EDTA, Group II: Apple vinegar and Group III: Distilled water. Samples were rinsed and prepared for examination by SEM. Results: The biocompatibility of tested solutions was arranged in descending manner as following: Apple vinegar > EDTA > cells in media, distilled water and saline > NaOCl. Apple vinegar for 5 minutes released highest amount of fibronectin with statistically significant difference from other groups. Conclusion: apple vinegar promoted survival and attachment of SCAPs better than EDTA.
