Skeletal abnormalities include radiohumeral synostosis, anterior bowing of femurs with neonatal fractures, medial bowing of ulnas, narrow iliac wings, arachnodactyly with distal tapering of digits, camptodactyly, and scoliosis. Other occasional abnormalities are hydrocephalus, atrial septal defects, vaginal atresia, renal defects, multiple hemangiomata, and preauricular tags. Auditory dysfunction may also occur.

Neonatal screening for phenylketonuria (PKU) has created an unexpected problem as females with PKU are reaching childbearing age. Surveys have revealed that maternal phenylalanine blood concentrations above 1200 mumol/L are associated with microcephaly, mental retardation, congenital heart defects, and intrauterine growth retardation among their offspring. It is estimated that as many as 3000 hyperphenylalaninemic females may be at risk for producing these fetal abnormalities. To examine this problem, the North American Maternal PKU Collaborative Study has been developed to evaluate the efficacy of a phenylalanine-restricted diet in reducing fetal morbidity. Preliminary findings indicate that phenylalanine restriction should begin before conception for females with PKU planning a pregnancy. Dietary control should maintain maternal blood phenylalanine levels between 120 and 360 mumol/L and should provide adequate energy, protein, vitamin, and mineral intake. Pregnant hyperphenylalaninemic females who achieved metabolic control after conception or by the 10th week of pregnancy had a better offspring outcome than anticipated.

OBJECTIVETo determine whether rebreathing of expired air is a plausible lethal mechanism of sudden death in infants sleeping face down on sheepskins.DESIGNCase reports of infants who died on sheepskins combined with a controlled study of effects of their respiratory microenvironment at death.SETTINGResearch laboratory.MATERIALSRabbits used experimentally to assist in simulation of an infant's respiratory microenvironment. Rabbits breathed into sheepskin through the airway of an infant mannequin.INTERVENTIONNone.MEASUREMENTS/MAIN RESULTSRebreathing of expired air was documented by carbon dioxide analysis of airway gas. Arterial blood gas analysis showed hypoxemia, hypercarbia, and acidosis in all experimental rabbits but not in controls. Rebreathing expired air was lethal for three of four experimental rabbits.CONCLUSIONSInfants sleeping prone on a sheepskin, with their faces straight down, experience potentially lethal rebreathing of expired air. Avoidance of the prone position would markedly reduce the risk of rebreathing expired air. The pronounced decrease in sudden infant death syndrome in southern New Zealand that followed a campaign to eliminate prone sleeping may have been attributable to reduced fatal rebreathing, as deaths of infants sleeping face down on sheepskins were common before the campaign.

Sir .—The otherwise useful report by Abadco et al 1 in the September 1992 issue of AJDC is marred by repeated use of the incorrect term Streptococcus viridans . This, of course, does not refer to a specific genus and species but rather to a large group of α-hemolytic streptococci, correctly designated as viridans streptococci (nonitalicized). Nevertheless, the authors are correct in discounting these organisms as pathogens when isolated from bronchoalveolar lavage specimens, as they serve only to indicate that oral flora have contaminated the lavage specimen.
