Opiekun dziecka jako sprawca urazu nieprzypadkowego w obrębie uzębienia. Część I. Identyfikacja zjawiska w aspekcie środowiskowym

Introduction. The problem of non-accidental injury has long been analyzed as a public health problem generating high social and economic costs. Aim. The aim of the study is to present views on the etiology of occurrence of the phenomenon of child abuse and its identification in order to take effective interventions interrupting the phenomenon of improper handling of a child. Material and methods. The publications in the PubMed scientific database were reviewed by applying the search criteria for the following keywords in Polish and English: “non-accidental injuries”, “children and adolescents”, “child abuse”, “identification of the phenomenon”, “perpetrator’s psychological profile”, “improper handling of the child”, “social and economic costs”, “public health”. Results. Medical community plays an important role in diagnosing non-accidental injuries. For a dental team, that may be dealing with physical violence injuries, correctly taken medical history and compliance of information provided by the child’s caregiver with visible head and neck post-traumatic injuries is the key factor to differentiate the origins of the trauma. By recognising the multifaceted risk factors in regards to child abuse (resulting due to the environment, the structure of the family, the life situation of the parent or directly related to the child), medical team will be required to intervene to stop child abuse. Conclusions. The authors of studies from various countries agree that despite many publications on the occurrence of the phenomenon of improper handling of a child, there is a need to constantly work on the competences of professionals in the child’s environment – including the dental environment, which can and should effectively respond to violence against children.

Zapalenie przyzębia u kobiet ciężarnych z cukrzycą i jego kliniczne implikacje – problem wielodyscyplinarny. Przegląd piśmiennictwa

Periodontitis is a multifactorial bacterial infection that causes destruction of tissues surrounding the tooth. Its relationship with numerous systemic diseases was confirmed in vast studies. Concerns of maternal and fetus wellness are raised by gestational diabetes mellitus and prepregnancy diabetes causing need for regular periodontal control. The paper is a review of available literature published in medical databases (PubMed, EMBASE, MEDLINE) and in Polish dental journals in the years 1999-2018. Diabetes is one of the risk factors for periodontitis occurrence and its exacerbation. In pregnant women there may be an exacerbation of the already existing periodontal disease. Periodontitis in pregnant patients with diabetes is associated with the risk of many complications for both the mother and the fetus. Pregnant women with diabetes should be under constant control of a gynecologist, diabetologist and dentist. Periodontal care in such patients should focus on rapid diagnostics of periodontal diseases and the implementation of appropriate treatment. Future mothers should be educated that oral health can affect pregnancy, childbirth and the health condition of mother and the child.

Wpływ ultradźwięków pulsacyjnych o niskim natężeniu na przebieg leczenia ortodontycznego

Acceptable speed of tooth movement in orthodontics is main limiting factor in treatment time. Although there are many devices and accessories which helps to move teeth – big forces in small period can cause complications like tooth resorption. The paper aimed to verify low-intensity pulsed ultrasound as method to accelerate orthodontic movement. A literature review from 2002-2020 using data bases like PubMed and Medline was performed. LIPUS orthodontics, LIPUS in dentistry, low-intensity pulsed ultrasound. 35 articles associated with the aim of this review were chosen and analyzed. Low-intensity pulsed ultrasounds start biological sequence on molecular level. Reactions on cellular structures accelerate bone healing and reduce negative effects and complications during treatment. Low-intensity pulsed ultrasounds have positive impact on bone wounds healing, treating developmental bone defects, cartilage and soft tissues reactions, tooth resorption, teeth tissue and periodontal regeneration and on orthodontic treatment acceleration.

Deep penetration fluoridation for caries prevention and treatment: the use of Tiefenfluorid® junior in children. Case reports 预防和治疗龋齿的深度渗透氟化:铁芬氟®junior在儿童中的应用。案例报告

The use of fluoride, a healthy diet and good dental care habits are the pillars of the fight against caries. At a time when the anti-fluoride movement is on the increase and raises doubts even amongst the dentists and dental hygienists, the dental environment should be based on the credible scientific evidence regarding the effectiveness and safety of therapy in their clinical management. The article presents the clinical cases in which the agent for deep penetration fluoridation Tiefenfluorid® junior (Humanchemie, German) was used in the treatment and prevention of carious lesions in children. The visual-tactile method was used for detecting caries lesions. The International Caries Detection and Assessment System – ICDAS II was used for classifying dental caries. Carious lesions activity were assessed using visual-tactile method at control visits every 3 months within 12 months. Fluoride is effective in the treatment of initial caries in enamel, and the agent based on silver and fluorine SDF (Silver Diamine Fluoride) effectively inhibits tooth decay in the dentin. The disadvantage of SDF is the black color of the teeth. In contrast to common fluoride compounds, the solution with fluoride Tiefenfluorid® junior (Humanchemie, German) becomes non-toxic due to the reaction with the second application solution with calcium hydroxide. Furthermore, the process takes place deep in the funnels of the dental enamel, not on the tooth surface. The deep penetration fluoridation can be used for the caries prophylaxis, including the mineral fissure sealing, as well as for the treatment of initial caries lesions. In the treatment of initial caries in small children, it is an alternative to the silver nitrate impregnation and for the fluoride varnishes.
